Calories, Not Protein or Carbs, Are Key to Weight Loss: Study

SUNDAY, June 26 (HealthDay News) -- Curbing calories is the key ingredient for diabetics seeking to lose weight, and low-fat diets that are either high in protein or high in carbs are equally effective, researchers say.

"I think there are two key messages from this study," said study lead author Jeremy D. Krebs, a senior lecturer with the school of medicine and health sciences at the University of Otago in Wellington, New Zealand. "The first is that no matter what diet we prescribe, people find it extremely difficult to sustain the changes from their habitual diet over a long time. But if they are able to follow either a high-protein diet or a high-carbohydrate diet, they can achieve modest weight loss."

Krebs said this first message conveys flexibility and allows people to choose the approach that best suits them and "even to swap between dietary approaches when they get bored."

The second point "is that for people with diabetes, if they can adhere to either diet and achieve weight loss, then they do get benefits in terms of their diabetes control and cardiovascular risk," he added.

Krebs and his colleagues are scheduled to report their findings Sunday in San Diego at the American Diabetes Association meeting.

To compare the potential benefits of two popular dietetic approaches, the authors tracked nearly 300 overweight men and women between the ages of 35 and 75 who were on a new, two-year nutritional program.

To start, all the participants had a body mass index greater than 27, meaning they were moderately overweight, and all had type 2 diabetes.

The researchers randomly assigned the participants to one of two groups: a low-fat/high-protein group or a low-fat/high-carb group.

For the first half year, all attended twice-weekly group sessions led by a dietitian; for the following six months, sessions took place monthly.

Weight and waist circumference were measured at six months, one year, and two years. Kidney function and lipid (blood fats) profiles were also assessed throughout.

Food diaries indicated that total calorie intake went down in both groups. Ultimately, both groups lost a similar amount of weight and reduced their waist size in similar measure, the investigators found. And by the end of the two-year period, both groups had similar blood fat profiles.

Krebs and his colleagues concluded that their "real-world" experiment demonstrated that both approaches afford similar benefits, with the principal driving factor behind sustained weight loss being calorie reduction rather than either high-carb or high-protein consumption.

Lona Sandon, a registered dietitian and assistant professor of clinical nutrition at the University of Texas Southwestern Medical Center at Dallas, said the observations were "not at all surprising."

"This is pretty consistent with other research out there that has conducted other long-term comparisons in the general population," she said. "In the first six months you might see a little better benefit from a high-protein approach. But long-term, the initial benefits from a high-protein diet seem to diminish over time, and the two diets end up being essentially equivalent," Sandon explained.

"The bottom-line is that the issue for weight loss is calories," Sandon added. "Not where those calories come from. You need to create an energy deficit to lead to weight loss, and that happens by decreasing those calories. That's just been shown again and again."

Experts note that research presented at medical meetings is considered preliminary because it has not been subjected to the rigorous scrutiny required for publication in a peer-reviewed medical journal.

This is incorrect. I was diagnosed as prediabetic in December. Doc said to limit carbs to 150 a day or less. I have lost twenty pounds , and my fasting blood sugar is now at normal non diabetic levels of 80-90mg/dl. I am 5' 9" and weigh 146 pounds. This places me a a normal range BMI for a 52 y/o male.

how much calories needed by the body of a regular person in a day?
      • Casey 10 mths ago
depends on a lot of factors...activity level, gender, muscle mass, goals, etc.

Sorry...as a 10 year veteran in the fitness profession, that's not true...2,000 calories of pizza vs 2,000 calories of chicken breast will have DRASTICALLY differing effects on the body

Lowering caloric intake, and therefore losing weight, is certainly vital in controlling type II diabetes (type I rings in a whole different kettle of fish). But I am wondering about the sudden blessing on high carb diet for diabetics. Carbs are burned to produce glucose, and diabetics cannot metabolize glucose effectively - that is why they ARE diabetic.

I would be very interested in seeing the Joslin Institute's take on this - rather the holy see of diabetes management in the US. After expounding for decades that exercise and diet were essential to controlling diabetes (along with appropriate medication - the third horse in their "triumvirate" approach), other research in the 80s confirmed the Joslin model, and led us to results like the study in the story.

I have been mentioning this in every comment I write and will keep mentioning it..
America read what have you done...
In every state you go, Malls, Costcos, Movie Theaters, Fast Food chains, Casinos, Pizza Hut, Dominos, Subways, we see the same crappy food that these chains provide.
Large quantities poor qualities. There is no way on this beautiful earth that you are not going to see more and more and more obese young or old, the more they eat this kind of food the more their brains are stopping from functioning right, and they dont want to exercise..
These people who live day after day eating poor qualities of food their brain cells are damaged they dont think of exercising they think only of eating, because this crappy food is like drugs for them except it is food that has chemicals not nutrients....
What do they eat in all of these locations? Pizzas, Hot Dogs, Hamburgers, Sausages, greasy Pastas, fried chicken, fried onions, milkshakes full of sugar, Mc Donalds Big Macs, is this what we were born to eat? Where in the hell have you seen a country that sells the same crappy food in every state and every corner except here in this great country that invents miracles, but they cannot invent fast food chain that contains healthy fresh food like veggies lean meat and fruits, that our bodies require every day to function properly, they dont because it costs lots of money...
Its too late America, more Obese is on the rise, these Obese will not give a dam about their bodies or exercise they want to eat crappy fast food cause they dont have energy to eat healthy and exercise...
Live with it America, look at the obese on the rise and enjoy watching bodies that look like huge ugly monsters.......
But who is happy? The owners of these fast food restaurants who are making tons of money and this is what matters for them... Show them the money they will screw all these human beings who are fed worst than animals ...................

Just try keeping hunger at bay on a low fat diet. Very difficult. Get rid of the simple carbs, most grains, and stick with healthy fats and high quality protein. If you don't consume carbs, your body HAS to use fat for an energy source, including saturated fat. And you will produce much less insulin, which wreaks havoc on your body.
    • Gonzo  •  10 mths ago
Why are we still studying this? We are all fat because we consume more food and exercise less. What else is there to study? Let's research and provide ideas that will help the 1 billion people starving on our planet..

        So darn true!!!
        Input-output = FAT

It is such a simple formula that so many studies are wasted on. Cut down junk foods and start exercising; change a life style slowly and stop WHINING. Gene may be a contributing factor for some people, but laziness and sedentary are the major reasons for obesity since we all have to eat to live and some people eat healthily and some eat just junk foods.

Stop with all the useless study's. People who want to lose weight just experiment with what might work for you. I know from firsthand experience that atkins works. Currently I am on it and have lost an average of 4 pounds a week without exercising. I have bacon and eggs for breakfast and eat steak and chicken. I just had bloodwork done and my trigcerides rae down and my A!C sugar test is down to 4.9 % from what used to be 5.8. Blood pressure is down to normal levels, all without medications. So for me and my friends who who the low carb /high fat way, it works.
      • WhiteDove 10 mths ago
Great news, Roger! Being a veteran of Atkins, my last blood work showed high HDL (75), low LDL and low Trigcerides. My doc gave me a thumbs up. The best thing is that I finally can control my hunger.

Atkins got it right coz its the first diet man ever ate to survive because he was designed to eat animal protein and fat for survival. Grains are a herbivorous food.We started eating them because of closer settlements and towns and scarcity of food led us to store grains.(Convenience, availability of food). We are the only non herbivore species on the planet eating grains.Who changed this and why?. Why the good old cereal advocating food pyramid !!.Setup by people who are llobyied by vegetarians and who did not like eating meat ofcourse(check it up - Food Pyramid Myth).30 years on, the low fat/high carb/low protein diet model, and look higher no of obesity and diabetic cases. Higher no of cardiovascular and heart disease, cholestrol, cancers cases have risen.High protein and high fat are must because of their Amino Acid/Fatty Acids chains, they cannot be created by the body and some must be consumed daily in all meals.Nuts have some amino acids but if you put almond butter on top of a whole grain brain, the amino acids are complete for intake. Just as an example. But all meats/dairies come with all 9 amino acids so why over 60% of food should be carbs in our diet?.Amino acids are the main components for building and repairing our bodies and without that our body will get broken down, stagnant growth,degeneration..But still carbs can be of use in boosting how fast those amino acids/fatty acids can be processed. Although carbs are the prefered fuel when we are fed mother's milk (high carb/high fat)when we are babies, which has twice as many carbs than cow's milk, they are not the original fuel the body was designed to run on for survival. Most studies found the body was originally designed to run on FAT because of its stability and economy as a fuel and its readiness to break into fuel, a steady performer. Secondly fat is good because we were not designed to run at high speeds or hunt by running. We hunted for food by using our brains (again amino acids/fatty acids) and we used our brains to figure out how we can survive. So in my experience there is no diet except low carb/high protein/high fat that will work best. However if you ever need carbs, sure have them but ensure they are low carb/low Gi/Complex/ whole grain foods and in moderation. Ofcourse any diet will work so long as lower your caloric intake, and you will still lose weight. So this study did not show us anything new than what we already know. In fact we know more and we know better already its proven(aka atkins diet, aka south beach diet and the recent popular DUKAN diet)

How about a test for Low carb/High fat vs. Low fat/high carb? Betting the statistics would be alot different. Low Carb/high fat would win hands down in weight and lipid testing.
      • Christine 11 mths ago
Fat actually contains 2.5 times more energy per pound than carbohydrates. Excess energy in the body is stored as fat tissue. That's why fat tends to pack on the pounds a lot more than anything else is because of all that extra energy it has in it that the body then takes and stores as fat tissue.
      • WhiteDove 10 mths ago
I believe the hormone "insulin" is the biggest factor in storing fat - which is stimulated by excess carbohydrates. Fat does not produce an insulin response.

low fat, moderate carbs, high protien,4 inches off my waist 22 lbs lost and 9% lower bfi with exercise 4 times a week. all in 3months,(just sayin)!

Its all about moderation. I have lost 85lbs in the last year due to eating better food. What is better: 300 calories of fruit or 300 calories of candy?? Its not all about calories its about how your body breaks down those calories. Fruit takes time to enter the blood stream while candy will explode into your blood stream and create havoc. Ever ate spaghetti and felt like sleeping an hour later?? Get away from as much processed food as you can. Eat well and sooner than later, you will be skinny, healthy and feeling great. Don't rush weight loss neither! Take your time. It took many years to put it on, expect a few years to take it off.

You need to study some more if you really think carbs won't make you gain weight or belly fat..
    • sammidanonth...  •  11 mths ago
alpha glycerol phosphate is created when insulin converts glucose. Alpha glycerol phosphate binds fatty acids inside the fat cell to create tryglycerides that can't escape the fat cell whereas unbound fatty acids flow freely in and out of the cell. It takes insulin to create this situation, and insulin only arises when carbohydrates are eaten. A diet of fat and protein will not create this situation and can be eaten in large amounts, calories don't count because the body uses what it needs and the rest is excreted. E

I agree with Gonzo. You eat foods high in fat and sugar and you dont exercise.... gues what.... you gain weight. Eat food low in fat with a moderate amount of protein and higher cars and you exercise you loose weight. Also watch your calorie intake. We all know what is good for us and what is bad.
