New Cholesterol Screening Guideline for Kids Sparks Debate

All children ages 9 to 11 should be screened at least once for high cholesterol, according to new guidelines; although some say this advice may amount to unnecessary testing.

The new recommendations are intended to prevent heart disease, which is rare in children, but more likely to develop in adults who had high cholesterol as kids.

"The more we learn about heart disease and stroke in adults, the more we know that the process begins in childhood and progresses over time," Dr. Stephen Daniels, chair of the panel that wrote the guidelines, said in a statement. "By working with families, we can keep kids at a lower lifetime risk and prevent more serious problems in adulthood."

The new guidelines, issued by the National Heart, Lung and Blood Institute and approved by the American Academy of Pediatrics, are a change from earlier recommendations that advised cholesterol screening only for children with a family history of heart disease or high cholesterol. However, some experts disagreed today that a changed was needed.

But recent research has suggested such screening misses 30 to 60 percent of children with high cholesterol, Daniels said, and an increase in childhood obesity has lead to a much higher proportion of kids who have high cholesterol.

    Testing can lead to treatment

Some experts disagreed with the universal recommendation.

Testing children's cholesterol levels has no value if those with high cholesterol aren't going to be placed on medication, said Dr. Chad Teeters, a cardiologist at the University of Rochester Medical Center.

And there is no evidence that treating kids with medications for high cholesterol prevents or slows the progression of heart disease later in life, Teeters said. Moreover, starting kids on drugs that they may need to take for their entire lives could have deleterious side effects, Teeters said.
And although the screening test — which would involve a blood sample or finger prick — is relativity inexpensive, the cost to apply it nationwide would likely be expensive.

"You're probably talking about an immense amount of cost every year, with limited to no benefit, and the potential to cause harm," Teeters said.
"I don't think they're any benefit to testing just for the sake of testing at this time," he said.

The U.S. Preventative Service Task Force says currently there is not enough evidence to recommend for or against cholesterol screening in children and young adults up to age 20.

Lifestyle changes can lower cholesterol too

Most kids found to have high cholesterol would first be advised to change their diet and increase their physical activity, Daniels said. Only about 1 percent would qualify for treatment with medications.

But Teeters said guidelines already recommend that all children get between 30 and 45 minutes of exercise per day  and stay away from fatty and fried foods.

This is the best way to keep kids' hearts healthy, and "I don’t see [that] doing the test would change my recommendation," Teeters said.
The new guidelines recommend screening before puberty because during puberty, children will naturally experience an increase in cholesterol levels. Cholesterol levels in 9- to 11-year-olds are relativity stable.

The guidelines also recommend breast-feeding babies, and that parents emphasize  a diet low in saturated fat starting at age 1 to prevent heart disease risk factors. Children who are overweight and have at least two risk factors for diabetes (such as a family history of the condition) should be screened for diabetes every two years starting as young as age 10.

The recommendations are published online today (Nov. 11) in the journal Pediatrics.

I can't help but wonder what part the drug companies had in "helping" this panel come up with this recommendation. I'm sure even if only 1% of the young kids tested are found to need to go on medication, it would be a big boost to the bottom line of the makers of cholesterol lowering drugs. They are among the most expensive drugs out there, and I'm sure the drug companies would be thrilled to have these people on them for the next 70 years of their lives. I don't know how many people are ever to get off their cholesterol medication, but I'm sure not one of them. But, I can't help but feeling that childhood should be a fun time for kids. Let's not start ruining that by making them feel guilty every time they eat a slice of pizza.

In my opinion, you are so right. these medicines are blackboxed in Europe and they have to have labels with the dangerous side effects and diseases of the lungs they can cause in some people.

Best thing to do is throw the kids out of the house and tell them to go play in the yard with their friends or ride their bike.

This is just crazy! I feel like this is another effort on the big pharm companies to expand their market base and put kids on statins for life! We NEED cholesterol. It's is what makes our brains work! Kids have more of it because they are still growing! These people are making a case for creating a whole generation of mentally disabled people. Stop this madness!

Same panel that advocated stopping routine breast exrays for women and PSA testing for males. Gotta love the lunacy.

Thank you for not paying attention in your schooling. Apparenlty no one learned about a balanced diet.

Kids can only consume what their parents buy and bring into the home. The parent has no excuse to blame anyone for what their kids eat except the one looking back at them in the mirror.

Money spent on vigorous physical exercise would make testing irrelelvant.

This is the most erronious article I have ever seen. It is a known fact that cholestrol medicines have no effect on womens heart attacks at all. There is no benefit for a woman to take cholestrol medicine. whatsoever.
Many cardiologists have testified to this fact.
As far as heart attacks go, it is the INFLAMATION, not the CHOLESTROL levels that are the cause of the heart atttack. In fact people who reach their 90's usually have high cholestrol, some are in the over 300 range, and the ones with low cholestrol have long since passed on.
Cholestrol medicines can cause all sorts of diseases of the lungs, this is well documented by lung dr's. and in many medical journals. Many of them are black boxed in Europe, but not in America, the power of the drug companies. stops it . There are warning labels of the dangers of taking cholestrot meds in Europe.
In my opinion, no one should be taking cholestrol medicine unless they have had a heart attack and their levels are above 250. the old standard was 220. till the drug companies found cholestrol medicine and started to make billions on it.

Major players in this controversy could very well be the insurance companies and the pharmaceutical companies. One wants to hold back cost of providing health medications and the other wants to sell more medications! Think how the threshold for diagnosis for diabetes type 2 was higher 5 or ten years ago. Now that threshold is much lower and those that were not diagnosed with type-2 diabetes are now prescribed with diabetes medications.
Meanwhile about half a dozen of which have been found to cause health risks, not considering the after effects like bloating, water retention, weight gain.

Okay...I am 62 years old...when I hit 50 I had 3 heart attacks and outlived all the males in my family....I also was the oldest ever living family member on my Dad's side.. when all the diet stuff came up in my 30's I obeyed...less fat, no drinking, running. etc. I had two kids...then my first heart attack at 42....they grew up eating a heart healthy diet, lifestyle, etc. I demanded at 12 they both got screening...it was hard...the Pediatrician opposed it but went ahead....they both had very high cholesterol readings..and are controlled by diet, lifestyle, and a low dose of statin...they probably won't have the now 4 heart attacks, open heart twice, congestive heart failure, and just plain lucky to be alive. If I hadn't fought for their tests when I did, they may have had to go through what I did...or not live past 45.

This is definitely a campaign to increase sales of cholesterol reducing drugs. There are now plenty of studies that show statins have little effect in preventing heart disease and plenty of side effects that would be seriously enhanced when taken over a lifetime. Another consideration is that children are still developing and require a certain amount of cholesterol for brain development. Lastly the cholesterol that can be controlled through statins are not responsible for heart disease. The pharmaceutical industry has never been able to develop any drugs that can control the specific type of cholesterol that is responsible for heart disease. They needed to pay for all the research spent so they passed off statin drugs that control a different type of cholesterol as the solution to heart disease and were profiting quite well until the medical community including coroners for the last 2-3 years are discovering that statins are not stopping heart disease. So now they seek to use govt. to screen children and force the use of statins on said children to maintain the profit margin. The people that make up this panel need to be investigated to see if they receive money from pharmaceutical companies or even work or reside on boards of any of the pharmaceutical companies.

I threw away the pills and straightened out my diet. My eyes opened when I was diagnosed with MVP after 20 years of doctors ignoring and downplaying my symptoms. I'm thin, so I must be healthy, right? Doctors help you when you get so sick that it becomes apparent they fuc*** up and missed the issue. I hate every mother's son of them.

    • Centrist  •  6 mths ago
Dr. Chad Teeters is wrong. Knowing there is a risk factor pushes parents to structure the kid's activities and diet in a healthier direction. Might even make them aware of their own bad habits. Double plus.
Cost? What's the cost of premature stroke, heart disease, death? Then the ripple effect on close family and friends?

Did you take your statin today? Instead, did you eat all your vegetables. I agree the drug companies has something to do with this one.

I would prefer that we would send the money instead to poor countries and or give money to feed the poor children in Canada who go to school without breakfast.
