    Inspector highlights psych drug use among elderly

    WASHINGTON (AP) — Government inspectors told lawmakers Wednesday that Medicare officials need to do more to stop doctors from prescribing powerful psychiatric drugs to nursing home patients with dementia, an unapproved practice that has flourished despite repeated government warnings.

    So-called antipsychotic drugs are designed to help control hallucinations, delusions and other abnormal behavior in people suffering from schizophrenia and bipolar disorder, but they're also given to hundreds of thousands of elderly nursing home patients in the U.S. to pacify aggressive behavior related to dementia. Drugs like AstraZeneca's Seroquel and Eli Lilly's Zyprexa are known for their sedative effect, often putting patients to sleep.

    But the drugs can also increase the risk of death in seniors, prompting the Food and Drug Administration to issue multiple warnings against prescribing the drugs for dementia. Antipsychotics raise blood sugar and cholesterol, often resulting in weight gain.

    An inspector for the U.S. Department of Health and Human Services told the Senate Committee on Aging that the federal government's Medicare program should begin penalizing nursing homes that inappropriately prescribe antipsychotics, according to written testimony obtained by the Associated Press.

    The Centers for Medicare and Medicaid Services provides health coverage to nearly 80 million senior, poor or disabled Americans.

    HHS Inspector General Daniel Levinson proposed that Medicare force nursing homes to pay for drugs that are prescribed inappropriately, and potentially bar nursing homes that don't use antipsychotics appropriately from Medicare.

    A report by Levinson's office issued in May found that 83 percent of Medicare claims for antipsychotics were for residents with dementia, the condition specifically warned against in the drugs' labeling. Fourteen percent of all nursing home residents, nearly 305,000 patients, were prescribed antipsychotics. The HHS Inspector General's office Medicare claims during a 2007 six month period.

    Doctors are permitted to prescribe drugs for off-label uses, though it is illegal for drug companies to promote uses that haven't been cleared by the FDA. In recent years several pharmaceutical companies have paid huge fines to the Department of Justice in cases involving off-label marketing of antipsychotics.

    In January 2009, Eli Lilly & Co. Inc. agreed to plead guilty and pay $1.4 billion for illegal promotion of Zyprexa, including marketing to nursing home doctors. The company told its sales representatives to use the slogan "5 at 5," to persuade doctors that giving 5 milligrams of the drug at 5 p.m. would make dementia patients sleep through the night.

    AstraZeneca PLC has paid nearly $600 million in two separate settlements with federal and state prosecutors over alleged off-label promotion of its drug Seroquel.

      This is a very difficult issue to attempt to tackle. As a clinical pharmacist working closely with the geriatric population I am generally opposed to antipsychotic use in the demented patient. The problem we have though is that if they do not respond well to non-medical means to control aggressive and potentially harmful behavior then we are left with few choices. They do work, but at a cost. The risk for premature death from all causes increases slightly with their use. This has to be weighed against the potential harm they could cause themselves, other residents, family members, etc. Clearly, they are over prescribed but to attempt to limit their use through regulatory action could do more harm than good. My fear is that in their zeal to act to protect (whatever the motivation is to do so), regulations could be enacted that take an overly simplisic approach to solve this problem.

        Agreed. What's worse? Living with dementia & the extreme challenges that go along with that, including loss of quality of life? Or giving them something that will help calm them & make it more managable for everyone, including the patient, at the risk of a premature death. Dementia is like dying anyway - at least everyone involved would have some relief.

        agreed, also the article does not mention any alternatives. what does a nursing home do with an agressive or violent dementia patient? sure, you could assign 2 burly aides to be with them at all times, but how could we afford that?

        I've taken plenty of antipsychotics for bipolar disorder and they are awful. You cannot go to the bathroom no matter how much Miralax and prune juice you drown yourself with. Antipsychotics are to make a patient's condition better for OTHER people, and are NOT there to treat the patient. Be honest about it! Even in clinical trials, where people are nagged to stay on the pills, virtually all people drop out of the drug trial, because this particular class of drugs is so unpleasant. Akathisia, now that is even more hell than the constipation. You have the most horrible crawling feeling inside you. Do you really think anyone will care if a patient with dementia is in agony from constipation or their soul is crawling around inside? How would they even explain how horrible their lives are on the drugs? Look at the way that I write, and doctors STILL did not figure out that I have akathisia, when I say "I feel the most awful, insect like, crawling feeling inside of me." They are not for the patient - these drugs are to make it easier for the nursing home. CHEMICAL RESTRAINT.

      What good is it to have laws preventing long term physical restraint if it is allowed to have long term chemical restraint? That is what these drugs do. Everyone can think up examples where maybe there is no other choice, but chemical restraints are routine treatment. Come on, I am on Medicare, I took antipsychotics for bipolar disorder (awful drugs!!) and yet I am a blip in the antipsychotics stats. The other major group that takes antipsychotics besides nursing home residents? CHILDREN IN FOSTER CARE. 1/3 take antipsychotics. It is all about the money. There are few other psych meds that are still on patent.

      Bush senior was petitioned by congress to stop lobbying on behalf of Eli Lilly pharm when he was Vice President. Previously, he had been on the board of directors of Eli Lilly. He was specifically lobbying for them to be allowed to sell drugs illegal in the USA to other countries.

      As I have been told by State officials, "you give up all your rights when you enter a nursing home". My son was placed into a nursing home where he was given many drugs which have as a side effect, sedation. Then they don't need to provide therapy because he is "non-responsive". Nursing Homes are, from my personal experience, places to warehouse people, sedate them, and collect a bunch of Medicare, Medicaid, and other Social Security types of funding until they die. As my son's legal guardian, I never gave consent for him to be in a nursing home, nor would I, but the hospital placed him there over my objections -- basically they kidnapped him for their own benefit. Now my son is receiving Social Security benefits because he is a disabled dependent of mine, but I am still required to send $648 out of his monthly $698 to the nursing home, by Hearing Officer Decision -- but I finally got him out of the nursing home and into a Social Services home for the disabled -- a vast improvement -- but I still must send the $648 to the nursing home every month even though he is not there! It's all about the money and power, the inmates don't stand a chance -- stripped of all their constitutional rights.

      From observing geriatric patients diagnosed with dementia using Seroquel , I can tell you that drug does not have a desirable effect on them, but medical community is only interested in profits.

      The institution I worked at routinely gave them to anyone who was unhappy about being warehoused so they wouldn’t try to escape or sit in the lobby and badmouth the place to potential inmate's families. The only way anyone escaped psychotropics was by serious butt kissing.
