Man Drops 376 Pounds After Being Told to Write His Own Obituary

Adam Slack's weight-gain journey began with an injury to his lower back.

"I had been laid up on the couch for about five to six weeks," the 37-year-old from Mays Landing, N.J., told "Good Morning America." "I didn't do anything but eat."

"That's when it really started," he said. "100 pounds turned into 200 pounds, then 400 pounds."

By 2008, the married father of four weighed 585 pounds, had high blood pressure, suffered from type 2 diabetes and struggled to complete even routine tasks.

"I used to tie his shoes. I would help him put on his pants. Everyone made fun of him because of how big his weight was," his wife, Michelle, recalled.

By June 2008, Slack's unhealthy lifestyle caught up with him and he nearly passed out during his shift as a security guard at an Atlantic City casino. His supervisor called 911 and paramedics rushed Slack to the emergency room of a nearby hospital.

It was in that emergency room at the AtlantiCare Regional Medical Center that Slack's journey to weight loss began.

"A doctor walked into my room and the first words out of his mouth were, 'Uh, hello Mr. Slack, what would you like in your obituary?'" Slack told "GMA."

"I yelled at him and told him to leave the room," he said. "It was the darkest place, but it was also a rebirth."

Dr. Irineo Bustamante was the emergency room physician who issued the blunt death warning to Slack, telling him that the hospital would cover his obituary for him.

"It just came to my mind," Bustamante, who had never before said those words to a patient, recalled. "He didn't look good. I really had to do something."

Two hours after their first meeting, Bustamante returned to Slack's room, and saw that something in his patient had changed.

"For the first time, it clicked," Slack said. "If I had one doctor, I had 100 doctors tell me 'you need to lose weight,' but it was the bluntness of what he said."

What else clicked for Slack was a new regime of exercise and a healthier diet.

Gone were the days of lying on the couch and eating one dozen eggs, a full loaf of white bread and a half-pound bag of shredded cheese for breakfast alone. In was a commitment to walking the family's German Shepherd, Thunder, each day.

"You have to get hooked on something," Slack said. "For me it was walking. It was baby steps, step by step."

Within six months, Slack was down 110 pounds. Today, he has slimmed down to a size 34 and 209 pounds, nearly half the weight from where he began.

He came full circle when he began working as a security officer at the same AtlantiCare hospital where Bustamante first issued his blunt, but motivating, words.

Though Slack could recall every pound he had dropped since his 2008 emergency room visit, he could not remember the doctor who, he says, had saved his life.

Prompted by media coverage of Slack's weight loss, hospital officials tracked down Bustamante as the physician who treated Slack, and reunited the two men at the hospital on Nov. 15.

"I was touched," Bustamante said of the cards and mementos brought by Slack's family and friends to say thanks.

The most touching for both men, however, was a baseball given to Bustamante by Slack's son, whose baseball team Slack is now able to coach.

Slack's entire family has joined him in his new, healthier lifestyle. His wife, Michelle, lost 24 pounds herself.

It all hit home one day on the baseball field with his son.

"We had just won a real big baseball game and he went to go give me a hug and he clasped behind my back," Slack said. "That was the first time I realized that my son could ever give me a hug. It was a real emotional moment for me."

7 years ago I had a ER doc come into tell us my brother had died of a heart Attack. The ER doc was talking to my Mom and turned around and asked "who smokes in here" I said I did and he said "I will see you next" The look on Moms face and it being New Years he changed my life. I haven't had a smoke in 7 years and I lost 25lbs. Now I'm a 56 year old and I can breathe. Thank God for ER Docs who care.

I'm seriously glad you were able to quit, but, I'm sorry, that doc sucks BIG TIME. He was unprofessional, and his timing couldn't have POSSIBLY been worse. It had nothing to do with "caring", but more a sort of "god complex", note the small "g"

K.G.: I also want to add that the Hypocratic Oath says "First, do no harm"- that includes psychologically. To bad those ER docs licenses didn't come with a degree in sensitivity. There's a difference between knowledge and wisdom. The knowledge of both are questionable, and the wisdom of both is non-existent. Pat YOURSELF on the back- YOU did it- you and the REAL God. My belated sympathy to you, and my most deepest sympathy to your mom, who had to be already traumatized by the loss of one son, without the needless, heartless worry of the thought of losing another

@Suzanne C. People in the medical profession get tired of everything else not working. We are running against the ads that claim "magical effects" from pills without lifestyle change, no sacrifice except in the tiny print on the bottom of the screen or paper where it mentions that results were obtained with diet and exercise. We're also running against the tendency towards instant gratification. Yeah being healthy (thinner or cigarette free) will feel much better and everyone knows but that delicious high cal treat or cigarette is there now and satisfies current cravings. A lot of the reasons Americans are obese and unhealthy in other ways is cultural. I hate to say it. I remember trying as a nursing student to get a woman with COPD to drink more water (to thin the lungs' secretions to cough them up more easily) and to use the incentive spiromter to increase her lung capacity. She just wanted her inhaler or nebulizer more often than the doctor's orders allowed me to give to her.

How wonderful to read a good inspireing news story for a change! I wish the media would offer more of them.
The hardest part of taking off weight is keeping it off.
I wish this man the best. He's a true hero and so is the Dr. who scared him into doing something.
      • Josh 6 mths ago
Sometimes you just gotta say something. That doctor knew exactly what to say when he saw him and said it, even though he risked a lot saying it. He's a hero.
      • Dennis 6 mths ago
Hmmmm.... so we can all be heroes by telling morbidly obese people they are going to die?

In that case, I'm going to shoot for freakin sainthood........

Hey fatties watch out The Truth will be pouring like gravy into your pie holes...

So, I guess I'm a hero too because I weigh 153 lbs. and exercise 5 times a week? Amazing that you can be a hero just by being a normal person and not eating a loaf of bread for breakfast.

Wow! Good for him...inspiring story! Doubt he could afford any surgeries to do this, sounds like he did it the old fashioned way - eat less (and healthy choices) and exercise. It's possible to achieve weight loss without surgery and pills.
      • Riven 6 mths ago
What makes you think he couldn't afford surgery? Are all fat peole poor?

I'm so glad he made himself better. If I see an excessively large person out exercising I offer them a word of encouragement. When I was 17 I couldn't climb a small flight of stairs w/o wheezing. My dad, who had just recovered from quadruple bypass surgery could climb stairs faster than I could. That's what did it for me. A 17 y.o. outpaced, outclimbed by a 60 y.o....

It would be embarrassing to the obese person if a stranger randomly offered compliments for working out. It is just pointing out their flaws. But if it is someone you know well, then that is fine.
      • ... 6 mths ago
Paul, your intentions are great, but please stick to a nod and smile when you see larger people out exercising. My mother and a good friend are obese. Neither will go out exercising in public because they feel like everyone is looking at them.

I tell them that isn't true, but almost every time we're out for a walk, someone says something "encouraging" that actually comes across as patronizing. The best thing to do is to treat these people like you would anyone else.

I went through a few bad years where I was an alcoholic, really bad..... After I lost almost everything, I (31 at the time, not overweight), was told I'd be dead well before 40. A few moths later I hit some new lows, survived, and changed. My life is much different, I look like I'm 25 instead of 45 and drunk, and what my doctor said really hit home! Sometimes someone who knows what they're talking about has to be cold, blunt, and tell you how it is. It's 2 years later and I'm alive and healthy, but if it wasn't for him I would not be writing this right now.

I'm fat. But even as a fat guy who is losing weight by changing my diet (fruits, veggies, white meat, TONS of water.) and exercising, I just don't see how some people can eat a dozen eggs, who loaf of bread and pack of cheese in one sitting. I mean, three eggs, a slice of toast and 3 sausages would have me stuffed. It's not fair that I have this slow metabolism.

Excellent, he is a perfect example of what can happen when you have "Will Power" and Determination! He did this the healthy way, not unnatural, like having surgery. Congratulations, on your Great Weight loss!

That would scare me too. I'm glad it turned around for him. Good luck and bless to him and his family.
