FILE - In this May 21, 2013 file photo, Sen. Johnny Isakson, R-Ga. speaks on Capitol Hill, in Washington. With a majority of Medicare beneficiaries dealing with two or more chronic conditions, a group of lawmakers from both political parties Wednesday proposed a new approach. Isakson is the co-sponsor of the Better Care Program where teams of doctors, nurses and social workers would get a flat fee per Medicare patient, with few strings attached. The goal is to keep seniors as healthy as possible, avoiding hospitalizations. (AP Photo/Charles Dharapak, File) less
