"Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health," a rep for Moore reportedly told both People and TMZ in a statement. "She looks forward to getting well and is grateful for the support of her family and friends."
TMZ reported Moore was taken to the hospital on Monday night, after paramedics went to her home, following a 911 call at 10:45 PM.
Paramedics reportedly treated the actress for around 30 minutes after which they took her to a hospital, law enforcement sources told TMZ.
When speaking with Access Hollywood on Tuesday, Los Angeles City Fire Department spokesman Matt Spence confirmed the department responded to a 911 call last night at around 10:49 PM to an address in Beverly Hills.
He could not confirm it was Moore's address.
The spokesman told Access the call was for an "unknown illness."
One ambulance and one fire engine arrived at the scene and one individual, who the spokesperson did not name, was transported to a local hospital.
A rep for Moore did not immediately respond to a request for comment from Access Hollywood on Tuesday evening.
