    Study: Colonoscopy cuts colon cancer death risk

    LOS ANGELES (AP) — Millions of people have endured a colonoscopy, believing the dreaded exam may help keep them from dying of colon cancer. For the first time, a major study offers clear evidence that it does.

    Removing precancerous growths spotted during the test can cut the risk of dying from colon cancer in half, the study suggests. Doctors have long assumed a benefit, but research hasn't shown before that removing polyps would improve survival — the key measure of any cancer screening's worth.

    Some people skip the test because of the unpleasant steps needed to get ready for it.

    "Sure, it's a pain in the neck. People complain to me all the time, 'It's horrible. It's terrible,'" said Dr. Sidney Winawer, a gastroenterologist at Memorial Sloan-Kettering Cancer Center in New York who helped lead the study. "But look at the alternative."

    A second study in Europe found that colonoscopies did a better job of finding polyps than another common screening tool — tests that look for blood in stool. Both studies were published in Thursday's New England Journal of Medicine.

    Colorectal cancer is the second leading cause of cancer death in the United States and the fourth worldwide. More than 143,000 new cases of cancers of the colon or rectum are expected in the U.S. this year and nearly 52,000 people will die from it, according to the American Cancer Society.

    Deaths from colorectal cancer have been declining for more than two decades, mostly because of screening including colonoscopies and other tests, the organization says. People of average risk of colon cancer ages 50 to 75 should get screened, but only about half in the U.S. do.

    A government-appointed panel of experts recommends one of three methods: annual stool blood tests; a sigmoidoscopy (scope exam of the lower bowel) every five years, plus stool tests every three years; or a colonoscopy once a decade.

    In a colonoscopy, a thin, flexible tube with a tiny camera is guided through the large intestine. Growths can be snipped off and checked for cancer. Patients are sedated, but many dread the test because it requires patients to eat a modified diet and drink solutions the day before to clear out the bowel. It usually costs more than $1,000, compared with a $20 stool test.

    Researchers at Sloan-Kettering previously showed that removing polyps during colonoscopy can prevent colon cancer from developing, but it was not clear whether it saved lives.

    The new study followed 2,602 patients who had precancerous growths removed during colonoscopies for an average of 15 years. Their risk of dying from colon cancer was 53 percent lower than what would be expected among a similar group in the general population — 12 patients followed in the study died, versus 25 estimated deaths in the general population.

    The study was not a randomized trial that's the gold standard in medical research. But Robert Smith, director of screening at the American Cancer Society, said it's the first direct evidence that removing polyps can reduce the risk of colon cancer death.

    "There's no question that these are findings that we can take to the bank," said Smith, who had no role in the research.

    The National Cancer Institute and several cancer organizations paid for the study.

    Government and private cancer groups also funded the second study in the journal, led by researchers in Spain. About 53,000 participants were given a colonoscopy or a stool blood test. Both tests found similar numbers of colon cancer cases — about 30 in each group.

    However, colonoscopies found advanced growths in twice as many people — 514 versus 231 of those given the stool test. Colonoscopy also found 10 times more people with less serious growths than the stool test did.

    Neither test proved very appealing — only a quarter of patients offered a colonoscopy had one. Similarly, only a third agreed to the offered stool test.

    The Spanish study is continuing and similar research in the U.S. and Norway that began recently is looking at the long-term impacts of colonoscopy.

    Stephen Raquet, of Mount Kisco, N.Y., finds the test reassuring even if the preparation is unpleasant. He had his first colonoscopy 13 years ago at age 41, earlier than usual because of a family history of colon cancer.

    The sudden death of his 45-year-old sister from the disease prompted Raquet to get checked out. He had a precancerous growth removed at Memorial Sloan-Kettering in 1999, and has had the test every three years since.

    During his last appointment four months ago, doctors said he can come back in five years.

    "It's given me peace of mind," said the 54-year-old business executive.

        Same thing happened to me at age 54. Treatment for stage 3 was hard. Major surgery , chemo and radiation. Would have been much easier if they had found it a few years before. But I ame alive, healthy, and cancer free now 15 years later.
        I believe Dr John Hoffman at Fox Chase Cancer Center saved my life.

      I can't believe what wusses we've all become. The prep is *not* "horrible, terrible" at all. You have to go on a low fiber diet for a few days before, so for those of you (like me) who eat a lot of fiber, it's an inconvenience.

      The day before, it's liquids only. Big deal.

      Then you have to drink this stuff which doesn't taste bad at all (they've improved it since the old days). And soon you're crapping your guts out. Again, big deal--it actually feels good to be cleaned out.

      As for the colonoscopy itself, you don't feel a thing. They give you something to "relax" you (I believe it's what's now known as "the Michael Jackson drug"), and the next thing you know, they're discusssing the results with you. You don't remember a thing.

      So stop being a wuss and get the test! And stop being squeamish about "anal things"--that's how we're designed for our greatest health. It's *natural."

        Really, it's a piece of cake. I've had two and I'd do it again in a minute.

        BTW, ask the facility where you'll be having it if you should bring something to eat when it's over. They make you hang out there a while afterward till the anaesthetic wears off, and you'll probably be pretty hungry while you wait. So bring a sandwich if they'll let you.

        The negative replies are simply stunning and illogical. The prep and procedure really are no big deal. In mine they found two polyps and instead of five years the doctor wants to do it again in three years.I'm thankful I had it done, unlike an acquaintance who's now undergoing Hell in expense and treatment for NOT having it done when the cancer could have been found in a very treatable early stage.

      I had one last year. Piece of cake. I dont know what all the fuss is about. So you dont eat for a day, big deal. Then they sedate you, and next thing you know, its over. They even give you a few photos that you can bring out when company is over.

      Laugh it up but: 2003 diagnosed colon /rectal cancer via this procedure/2004 loss all intestines/appendix,gall bladder and organs to be named later....radiation and chemo before and after..yes I do have a new friend attached to me for past 8 years..so....GET THE TEST and stop with the jokes...believe me dying is not funny. LIVE!

        Thanks for the honesty, my mom kicked the bucket with this and it was fricken nasty aweful pain train for three -four years. Maybe just maybe you helped someone wake the F up today. i started getting my test at 42 and will have it every 5years perDoc orders. Best of luck to you Realist.

        Congrats on surviving... My mom had a precancerous polyp removed after a colonoscopy... Unfortunately at the 3 month check up the doctor didn't do a pet scan to see if he " got it all" but rather went with his gut as he felt ..... Hey I'm SURE I got it all. Well he didn't and 3 months after that it had traveled to her liver and lungs, two years later we lost her. People need to not only get tested but keep an eye on their doctor as well.... A simple pet scan that was covered by her insurance would have saved her life.

        HOmeless.... I'm sorry about the loss of your mom... I'm there with you.

      The prep for the test was unpleasant, drinking the liquid the jight before, thank God I did the test early, I had the test at 41, they found a "polyp that was well on its way to being cancerous", per the doctor, if I had waited until 50 for the test, it may have been too late

      After putting off the "dreaded" colonoscopy for 4 years, I finally had mine done a few months ago. My gastroenterologist who performed it is great, had pills instead of the horrible liquid, was sedated, was under for about 45 minutes....did NOT feel a thing....came home, had a subway sandwich with fries and a drink, afterward had a nice long nap, and then fixed dinner for my husband!!!! Oh and my results were great....doctor does not want to see me for another 10 years!!!! Highly recommend having it done....in other words it's not a big deal :-)

        Two things wrong with your story here...first the pills that you took were recalled by the FDA as potentially harmful. The next time you get scoped, you will have to drink the salty drink. Second, no doctor on earth would recommend going home to eat the way you did. Way to follow doctors orders.

      Told a friend that i dreaded the prep. He asked if I would rather die of cancer. Obvious answer. Had it done and the doctor found two "minute" growths. Both turned out to be benign but who knows about in another 5 years. HAVE THE PROCEDURE.

      How is it horrible? You drink some stuff and go to the bathroom until your colon is empty. They sedate you, you fall asleep and don't feel a thing.

      My mom was diagnosed with colon cancer earlier this month. She found out by having a colonoscopy. Now by the end of the month she is cancer free!!! She had surgery and they removed some of her colon. I am only 28, but I already have my colonoscopy scheduled.

      As far as medical proceedures go, this test is not so bad. The sedation is the big plus -- didn't remember a thing for about an hour afterwards. Bring a designated driver with you!

      About time they said a preventive test actually prevents deaths from disease. That stuff they give you to cleanse the colon is truly awful, no getting around that. But, worth it if they find a cancerous or pre-cancerous polyp. The procedure is easy, you are knocked out on Verced, a wonderful medication for putting you in lala land. If you are over 50, don't be stupid, get one!

      My Mom didn't want a colonoscopy because it was "icky", and would only do a stool test. She was diagnosed with stage 4 colon cancer after complaining of stomach pain, and passed away 13 months later. Please get a colonoscopy!

      For all the people that feel a colonoscopy is unnecessary or think it is a money making scam, if you want to live, I highly recommend it.
      Yea, it is uncomfortable. Yea, it is not a lot of fun. Big deal. Having your colon cut in half because the cancer has eaten up most of it, or having it completely removed is much more painful. Carrying a bag around is not my idea of fun. After waking up from surgery the pain was intense. At that time, I almost would have preferred to have died. Now, here I am 2 years later. I get to visit with my grandkids.
      I had my second colonoscopy about 14 hours ago. You gotta look at this like an adventure. Make it entertaining. Had 8 months of chemotherapy. Now, the cancer has spread to my liver. Three tumors on it. Biopsy scheduled for next Tuesday. Then, more chemo.
      So what!
      If you choose death, your family will miss you.
      If my stupid doctors at VA would have let me get the colonoscopy procedure when I told that there was a problem, and not kept telling me the CT scan looked ok, the MRI was fine, the Sonogram was perfect, It would have been a tiny little polyp and been removed and I would not be going through this. No family history of cancer. Most of our elders live to be 90.+
      My wife was not military so we can't afford it for her. She is already 60. We live off $750 a month in my disability. We pay the mortgage and skip on electric and food. Now, we gotta find a way to save up enough for her to get checked, too.
      If you choose to die, it is your business. I don't. I have had both legs cut off for over 20 years. Clients don't pay for excuses, they pay for results. You don't get what you deserve, you get what you EARN! Fight like you can not die. Never quit. Never give up.

      I had my first colonoscopy this past year at 38, my father died of colon cancer. For those that dread the procedure whether its because of the prep or the actual procedure. Please take heed. The Prep, requires you to stay home that one day, I had no bad side effects at all, and it did wonders for cleaning out my system. As (no pun intended) for the actual procedure, I do not remember anything at all, no discomfort whatsoever. Plz your health is vital make an appointment, and let the gastroenterologist sweat the small stuff.

      Ten precancer polyps were removed that saved my life. I go every 2 years now and so glad I did this test. Your life is worth every dime whether or not you have insurance. Colon cancer is the worst way to die.

      Thanks a lot for the negative attidude throughout the article- "endured", "dreaded exam", "horrible and terrible". That'll sure prompt people to get checked.

      I have a very different opinion. I've already had two in my 40s, because of a close family history. Last one was a 1-day prep, very easy to complete. Basically nothing but clear liquids the day before, then start the laxatives around 3 pm. Go in the next day, get the procedure under anesthesia, wake up, have someone drive you home, and back to work the next day or the day after. (I was able to return to work the very next day). It's really not that bad. Yes, some people avoid the exam because of embarrasment, fear, and not wanting to take time off from work, but it is worth it. It can save your life. If you don't want to do it for yourself, do it for your kids.
