Why Women Report Being in Worse Health than Men

When asked to rate their own health, women, on average, consistently report being in worse health than men do, and a new study from researchers in Spain says this is because women have a higher rate of chronic diseases — contradicting a previous theory that women's lower self-rated health is simply a reporting bias.

"In general practice, there has been this idea that women over-report health problems, or are more likely to say they are ill or pay attention to their symptoms than men," said first author of the study Davide Malmusi, of the Public Health Agency of Barcelona. "We wanted to test whether their differences in self-reported health could in fact be explained by the difference in the prevalence of chronic conditions."

The new findings were published Dec. 16 in the European Journal of Public Health.

    Self-reporting health

Malmusi and colleagues across Spain gathered data from Spain's 2006 National Health Survey, which included data from face-to-face interviews with more than 29,000 people on their health. About half of the study participants were between the ages of 16 and 44; the other half was older.

The survey included the question, "Over the last 12 months, would you say your overall health has been very good, good, fair, poor, or very poor?" as well as a question on whether health problems had limited people's activities over the previous six months.

Of the women interviewed, 38.8 percent rated their health as poor or very poor, and 25.7 percent reported chronic limitation of activity. Of the men in the study, only 27 percent had poor self-rated health, and 19.3 percent reported chronic limitation of activity.

But when the researchers matched up the number of chronic conditions each person had with his or her health rating, the gender difference disappeared. Having a higher number of chronic conditions correlated with poorer self-rated health to the same degree in both genders.

For men and women with the same conditions, or the same number of conditions, women were no more likely to claim poorer health.

"There's been a longstanding debate about whether women's self-reported health is a reporting bias or not," said sociologist Ellen Annandale of the University of Leicester in the United Kingdom, who was uninvolved in the new work. "Some researchers argue that women might over-report health problems, and men might under-report. This study supports wider research that women's poorer self-reported health reflects underlying chronic health problems."

The root of chronic health problems

What the new study doesn't answer, Annandale said, is why women have a higher rate of chronic health problems. The data did reveal that women's higher rate of chronic problems can be most strongly attributed to five chronic disorders: arthritis, mental disorders, neck pain, headaches and back pain. But further research will be needed to explain why.

Malmusi said it is likely a mix of biological and social factors.

"Gender influences that way that people are treated and diagnosed in health systems," Annandale said. "It influences the kind of health conditions that men and women suffer from, the way people relate to their own bodies, and what kind of access to health care they have."

Understanding gender differences in health can help scientists and doctors find ways to better treat patients, she said.

"Women generally live longer than men, but in many countries that gap in life expectancy has been decreasing over time. One of the reasons for that is thought to be that men's health is improving, but women's is not."

Pass it on:  The fact that women generally rank their health poorer than men do correlates to a higher rate of chronic health problems, not a higher likelihood to complain about their health or pay attention to symptoms.

I hope doctors are reading this study article. Too many times older females get a patronizing pat on the shoulder instead of appropriate medical help.

Agreed. The second time I went to the doctor for a leaky blood vessel in my eye, he told me I was being emotional. Six months later I was diagnosed with macular degeneration by a different doctor. I guess this was due to being 'emotional' .

Women are more unhealthy because putting up with men makes us sick

Women live longer because they don't have to live with women. :)

lol, many are being mean on here, but I think that a good percentage of illness in women is because we are more exposed to children who are ill. Even a woman that doesn't have children will have more time spent with a child than a man who does not have children. Add in the fact that if a woman does not work and raise children at the same time; then society considers her lazy, with the overwork leading to poor health; and you have a good percentage of the cause.

That actually does make a LOT of sense!! Kids are great bacteria carriers, and women do interact with them considerably more.

Yes, but the study is based on CHRONIC illness, not bacterial. Overwork can definitely contribute to chronic illness, bacteria not so much.

Women self-report being in worse health than men because women actually have more chronic conditions than men? Gee, d'ya think? And part of the reason women are in worse health, with chronic conditions, is because most low-paying no-power crappy jobs don't come with health insurance, not every woman can afford to pay full price for medical care because she has children whose needs take priority, and if she works full-time outside the home and then comes home to take care of a house, a husband and a couple of children for another sixteen hours, she is exhausted, stressed out, and her mental and physical resources and immune systems are low.

That having been said, that which doesn't kill us makes us strong. It is precisely because we have to deal with the overwhelming demands of real life (while men, for the most part, come home, park themselves on the couch or behind the computer and don't help, or offer very little help), which, on one hand, gives us chronic conditions, on the other hand, provides us with the strength to outlive our husbands. (That and a case of pure cussedness.)

The article is nonsense. The "higher rate of chronic problems" is actually a higher rate of REPORTED chronic problems. So all the research really shows is that women report more problems because women report more chronic problems.

You must not have understood the article. It said that they do it because they are sicker. (extreme paraphrasing for the understanding impared)

women not only have to take care of their kids (husbands) house etc....but also have to work outside the home too sometimes. sometimes we are mistreated, abused, or not helped. its many factors, even health wise women have 9 months of pregnancy, labor pain or c section surgeries, a monthly period.......yet I BELIEVE WOMEN ARE SO STRONG BECAUSE THEIR IS NOTHING STRONGER THAN CREATING ANOTHER HUMAN BEING IN YOUR BODY, WHICH ONLY WOMEN CAN DO......so that we can all be here today!

Its cause we do all the work, while the men just bring home the paycheck, if that. If they cooked, and cleaned, and took the kids to all their activities on a daily basis they would get ill more often also.

I'm sure I'd complain more if I bleed from an orifice once a month for 5 days. That would suck.

    • John  •  Killeen, Texas  •  4 mths ago
A little premature to make statements like this with just one study. It is a fact that women go to see doctors more often then men so they may be more aware of their ailments then men are.

I am whinner but so are men, my husbands cries daily about all his health problems. I finally told him man its enough is enough think positive dude. Keep convicing yourself sick & you will be. The mind is very powerful tool. Think about enough & you will be. I told him eat right exercise would cure much of what ails him.

women do everything while men are turning into a bunch of pussys who are more than willing to let women do it all.

the answer is everything we do - for men, children, etc..., but not for ourselves...and we are more likely to keep going in spite of how we are feeling...Men are big babies when it comes to pain-That commercial earlier this year, with the sick man lying on the couch, saying "Ican't reach the remote.." sums it all up.
    • AG  •  4 mths ago
Woman get sicker because we are made to believe we have to be strong and take care of the men and children as well as others around us and we spend all our energy doing that then when we finally get time to settle down (which isn't often) our immune system becomes so weak we can't fight off the things that hit us.

Ok, how do they not understand that most women work a fulltime job and come home and take care of husband, kids, houses and pets? They work twice as much everyday as a man because most males don't do much around the house. Maybe mow a lawn once a week, or if you're really lucky help you with dishes or laundry once or twice a week. HOWEVER these men will claim they help thier wives with housework more than they actually do. I don't know of a single couple where the man actually does 50% of the housework/kid duty with thier wives. So women are basically being worked to death. That is the problem. How do these "Doctors" and "Scientists" not get that? AND for the guys....women will quote a lot of reasons why we seek divorces, but at the root of it all, past the money, or infidelity, etc. it really almost always comes down to the male partner doesn't help with the kids and the housework and the woman just can't take care of all of it so they may as well not have the husband around that they have to take care of like a child also. it's very frustrating when your spouse to be helps you when you are engaged and dating, and maybe at first in the marriage, but v ery shortly into the relationship most men just flat start thinking we are thier mama's and we get stuck picking up after them worse than the kids.
