7 huge Casualty spoilers for next week
Next week's Casualty episode will see Ruby make a shocking discovery about Lev's past, while Ethan faces up to his trauma, and David pops the question to Rosa.

1. Ruby makes a shocking discovery about Lev
Next week's episode will kick off with a flash-forward scene showing a distressed Ruby telling her boss Jan that she no longer wants to work with her colleague Lev. Strange, right? Well, when we cut back to three hours earlier, everything becomes clear.
After a strange incident with a patient, Lev horrifies Ruby by explaining that he was in a violent gang when he was younger, who organised crime.
Seeing Lev in a completely different light, Ruby takes her concerns to Jan – insisting that she can't work alongside Lev anymore.
2. Ruby can forgive – but she can't forget
Despite her insistence that she needs to be taken off any future jobs with Lev, Ruby starts to fear she has jumped the gun when she sees his softer side later on.
After noticing how he is to a young boy, Jan tells Jan she's happy to be on the truck with him after all. Lev is relieved that Ruby has given him another chance – until she lays out the terms and conditions moving forward.
Wanting to be clear, Ruby tells Lev that, while they are a team, they can never – and will never – be friends after what she has found out about him.
3. Ethan is haunted by the past...
Keen not to move forward in the wake of his split with Effie, Ethan throws himself to work once again – and bonds with a young girl called Abigail, whose father died a few months ago.
As Abigail waxes lyrical about her heroic father, Ethan correctly starts to suspect that he knows who her dad was: Big Al.
Fans will remember that Big Al was the brave firefighter who lost his life in the terror attack at the start of the series – and Ethan was there during his final moments.
Abigail's presence sends a traumatised Ethan into a spin as he debates over whether he should tell her what happened in Big Al's final hours – until the decision is taken out of his hands.
4. ...And makes a decision about his future
When Abigail asks outright if Ethan knew Big Al, he realises he can't lie to her anymore and confirms that he was there with him before the second explosion.
As the pair talk about his Al's moments, it's cathartic for both of them – with Abigail encouraging Ethan to take some time out and look after himself.
Realising that being at the scene of his heartbreak is too painful, Ethan makes a big decision in order to combat his survivor's guilt. What is it?
5. Rosa's mum arrives
Next week's episode will see Rosa and David take the ultimate test with their fake relationship when her glamorous mother arrives.
Viewers know that David and Rosa have agreed to go along with a farce romantic relationship for the sake of her mum, but there's been some telltale signs that they both want it to be more.
As an apprehensive David swats up ahead of Xiomara's arrival (by practicing his Spanish and everything), he gets a shock when she arrives at the hospital a day early. What will she make of David? We can give you a clue…
6. David learns some sad news
To David's relief, Xiomara takes an immediate shine to him – but it's clear that she's on a one-woman mission.
Alone, Xiomara interrogates David, she drops some heavy hints about him making an honest woman of her daughter – leaving him flustered.
Eager to get away, Xiomara manages to hold David's attention when she implies that she is dying and wants to see Rosa happy and settled before she goes.
As David processes the news, Xiomara piles on the pressure by giving him her old engagement ring – urging him not to take too long.
7. David pops the question
With Xiomara's words ringing in his year, David broaches the topic of fake marriage with Rosa, whose true feelings are starting to become all the more apparent.
However, not wanting to get in the way of a 'real romantic opportunity' for him, Rosa is reluctant to take such a big step for the sake of her mother.
Ignoring her concerns, David gets down on one knee and delivers a fake proposal anyway, but is this starting to become more real?
