Ashburn Health Inspections: Samurai Sushi, Sense Of Thai

ASHBURN, VA — Several restaurants around Ashburn, Dulles, and Sterling recorded violations during recent inspections from the Loudoun County Health Department. The inspections occurred between Feb. 23 and March 1.

COVID-19 restrictions have relaxed around the state, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Here are the most recent inspections from around Ashburn, Dulles, and Sterling, as they were written by the Loudoun County Health Department.

  • Samurai Sushi Restaurant, 43150 Broadlands Center Plaza #162 Ashburn, VA 20148

    • Date inspected: Feb. 23

    • Total violations: 2

      • Handwashing facilities in the restroom observed without hot running water.

      • Ventilation hood observed with heavy accumulation of dust.

  • Yen’s Kitchen & Sushi Bar, 44640 Waxpool Rd #100 Ashburn, VA 20147

    • Date inspected: Feb. 23

    • Total violations: 4

      • Two out of the three hand sinks in the back kitchen are filled with employee personal belongings such as coats and sweaters. Hand sinks appear to not be in use.

      • Raw animal proteins were observed stored over ready to eat foods in the following places: - a container of raw ground pork stored on top of a container of noodles in the walk in cooler. - bag of raw diced chicken over cooked tofu in the walk in cooler - raw salmon and beef stored over lettuce leaves and basil leaves in the open air cooler next to the cook line.

      • Facility does not have established date marking procedures in place for prepared ready to eat TCS (time/temperature control for safety) foods. A bulk of the menu items are prepared daily however during today's inspection EHS observed several RTE TCS foods that were prepared on Monday (2/21) according to the PIC without date markings. The restaurant is closed on Tuesdays. Fried tofu, fried chicken, cooked lo mein noodles, cooked pork belly all observed without a date mark.

      • There is an sticky fly trap hanging from the ceiling over the cook line above exposed food and food preparation areas. There is also one hanging in the ware washing area.

      • Person in charge (PIC) unable to provide the signed copies of Form 1B (food employee reporting agreement).

      • Handwashing facility near the three compartment sink observed without paper towels.

    • Date inspected: Feb. 24

    • Total violations: 1

      • Observed quat sanitizer at 3 compartment sink recording concentration of less than 100 ppm.

  • Burger 21, 21305 Windmill Parc Dr #160 Dulles, VA 20166

      • A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees

      • Food items in the refrigeration unit across from the cook line second from the right are cold holding at improper temperatures.

      • Food items cooling in large buckets and large containers, preventing proper cooling.

      • Food, including containers of sugar and spices, are uncovered in the dry storage areas. Food items stored less than 6" from floor in dry storage areas and in refrigeration unit.

      • No sanitizer testing devices provided for the QAC solution at the 3 compartment sink.

      • Hollandaise sauce was observed hot holding at 130F.

  • Golden Path Academy, 21631 Ridgetop Circle #150 Sterling, VA 20166

      • Receiving temperature for cooked rice and beans in a plastic container and cooked corn registered at 86°F and 110°F respectively.

  • Eddie Merlot’s, 20350 Minot Dr Ashburn, VA 20147

      • Improper date marking in that several food items such as soups and dressings in the walk in refrigerator observed with no dates or improper date marking.

      • Sliced tomatoes, leafy micro greens, sour cream and dressings in two of the one door reach in refrigerator in kitchen registered at between 47°F to 50°F. As per the PIC said food were held in said unit overnight.

      • Cheese and dressings registered at 46°F in top and rear section of the eight drawer low boy refrigerator. Said unit observed not circulating cold air adequately to evenly cool the unit.

      • Two handwashing facilities in the kitchen observed without paper towels.

      • Establishment is unable to provide procedures for employees to follow when responding to vomiting or diarrheal events.

  • Farmwell Station Middle School, 44281 Gloucester Pkwy Ashburn, VA 20147

    • Date inspected: Feb. 25

    • Total violations: 1

      • Several old glue traps in dry storage area- located in the corner to the immediate right and under the rack on the immediate left of the door.

  • Super Chicken Dulles Inc., 45591 Dulles Eastern Plaza #194 Sterling, VA 20166

    • Total violations: 3

      • Food debris observed around the outside rim of the large white containers used to marinate whole chickens. Containers were in the clean stack by the back door.

      • Three prepared salads (TCS- time temperature control for safety foods) observed cold holding at improper temperatures in the serve line cold holding unit. Cole slaw 44F, pasta salad 45F, sliced tomato salad 44F. Cooked yuca (51F) cold holding at improper temperatures next to the fryer. The person in charge (pic) stated that it sits at room temperature next to the fryer during the lunch rush.

      • Bags of rice observed stored directly on the ground in the dry storage closet.

  • Sterling Middle School, 201 West Holly Avenue Sterling, VA 20164

    • Total violations: 1

      • Employee restroom observed without covered trash receptacle.

  • Pizza 360, 21760 Beaumeade Circle #130 Ashburn, VA 20147

      • No person in charge present at time of inspection

      • Test device for chlorine sanitizer expired 2019

      • Hand sink in kitchen is slow to drain.

      • An irreversible registering temperature indicator was not provided for the hot water mechanical ware washing operation.

      • The person in charge is not able to relate information on the primary symptoms of illnesses/diseases that are transmissible through food.

      • Person in charge (PIC) observed drinking from open cup in kitchen.

      • Cook observed garnishing the ready to eat food with sprouts using bare hands.

      • Handwashing sink next to the oven in the kitchen observed without paper towels.

      • Handwashing facility close to the three compartment sink observed without hot running water.

      • Raw animal food stored over ready-to-eat food in the refrigeration unit in that raw shell eggs observed stored above vegetables (carrots and celeries) in walk in refrigerator.

      • Improper cooling in that three containers of green curry registered at 48°F in the walk in refrigerator. As per the PIC, said food were prepared a day prior and placed in the walk in refrigerator.

      • Cooked pork registered at 45°F in low boy refrigerator in kitchen.

      • The prepared ready-to-eat (RTE) food in the low boy refrigerator in kitchen were held beyond the marked disposition date (past 7 days).

      • Containers of bleach and soap observed placed next to containers of vinegar in the shelf in kitchen

      • Several wet wiping clothes improperly stored between use in that wet wiping clothes observed left on prep tables throughout the kitchen.

      • In-use utensils improperly stored between use.

      • Cutting boards observed deeply stained and discolored

      • Floor tiles observed broken with water accumulation in gaps.

      • Ventilation hood observed with heavy accumulation of grease and dust.

  • Koyla Bar B Q Lounge, 45965 Regal Plaza Sterling, VA 20165

    • Date inspected: March 1

    • Total violations: 4

      • A spray bottle of orange tinted liquid was observed at the three basin sink by the back door

      • A container of Chick Peas that had been cooked on 2/25/22 was observed without a date.

      • Both handwashing sinks in the kitchen do not have hand drying provisions. One has no soap. One is being used for dumping, evident by food debris within the sink (rice).

      • The facility does have an employee health reporting policy, but it is not onsite to be verified.

  • Mediterranean Breeze Restaurant, 20693 Ashburn Rd #100 Ashburn, VA 20147

    • Date inspected: March 1

    • Total violations: 11

      • There was no verifiable employee health policy available.

      • There was an extension cord in the hand sink at bar.

      • Hand sink in kitchen the hot water was turn off.

      • Multiple gallons of milk with an expiration date of 1/27/2022.

      • Observed raw lamb chops store next to raw ground beef in walk in cooler.

      • Observed cut tomatoes 50F, diced tomatoes 46F, raw chicken 46F in prep cooler.

      • Mouska, pastitsis, and stuffed grape leaves with no date make for disposition.

      • Observed no asterisk on the menu to indicate what food item are raw or undercooked for Greek burger and house balsamic vinaigrette is made with raw eggs.

      • Observed in walk in cooler bottles water, raw chicken, and bottled beer stored on the floor.

      • Observed both prep coolers with build up dirt and grime.

      • Observed BBQ in kitchen/storage area not for restaurant use.

  • Rupa Vira’s Catering Services, LLC, 21760 Beaumeade Circle Ste 120 Ashburn, VA 20147

    • Date inspected: March 1

    • Total violations: 2

      • A time control for safety plan is needed for the small refrigeration unit across from the cook line, as well as the rice steamer by the entrance to the kitchen, to ensure that no food remains in the temperature danger zone for more than 4 hours.

      • No testing devices for the QAC sanitizer at the 3 compartment sink, chlorine test strips for the dish machine expired.

  • Reggio’s Treehouse, 19455 Deerfield Ave, Suite 101 Lansdowne, VA 20176

      • A spray bottle with clear liquid and no identifying information was found in the kitchen. (Diluted bleach solution for sanitizing)
