Berks food safety inspections: Mold build up along walls and floors of produce cooler found

Nov. 26—The Pennsylvania Department of Agriculture keeps records of food safety inspections at restaurants and food vendors.

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Onvo Travel Plaza-Bethel, 8602 Lancaster Ave., Bethel , Nov. 18: one violation. The handwash sink located in the counter area does not have water at a temperature of at least 100°F.

Speedway #06771, 328 E. Penn Ave., Wernersville , Nov. 18: six violations. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Coffee counter sink has a heavy dirt debris build up around drain line, and floor drain. The food facility does not have the original certificate for the certified food employee posted in public view. Corrected. Creamer in customer self serve dispenser, a refrigerated, ready to eat time temperature control for safety food in the coffee counter area, was date-marked by the facility, but was beyond the use by date and requires discarding. Corrected. Beverage container tops / lids submerged in melted ice water in consumer self service display container. Corrected. Ice machine chute, a food contact surface, was observed to have pink slime mold residue and was not clean to sight and touch. Walk in cooler fan box area of the food facility is extremely dirty, dusty, and in need of cleaning.

Turkey Hill Minit Market #295, 2600 Conrad Weiser Parkway, Womelsdorf , Nov. 18: five violations. Cheeses and sandwich meats in the under counter refrigerator, a ready to eat time temperature control for safety food in the kitchen area, was date-marked by the facility, but was beyond the 7 day use by date and requires discarding. Corrected. Observed deeply scored bain marie cutting boards not resurfaced or discarded as required. Observed prep table equipment, in kitchen area, with an accumulation of dirt, food splash residue, debris on non-food contact surfaces. Shelves under table top need cleaning. Also trash and debris under reach in under counter refrigerator. Front door located in the front area of the food facility has a large gap in center and does not protect against the entry of insects, rodents, and other animals. Store has heavy fly activity. Area under customer counter coffee sink has a build up of spilled debris/mold, old coffee filters. Corrected. Kitchen air vent area of the food facility is extremely dusty and in need of cleaning.

Blue Mountain Academy, 2363 Mountain Road, Hamburg , Nov. 16: two violations. Wall in the tray drop off drainboard area in the dish room is made of tiles that are missing and wood is exposed and is not a smooth, easily cleanable surface. Also several tiles along corner of wall are loose. Old unused french fry cutter must be removed from the top of the 2 bay sanitizing sink.

Red Lobster #218, 945 Woodland Road, Wyomissing , Nov. 16: two violations. Loose/broken door hinge observed on the 4-door Victory refrigeration unit at the cooking line. Door is not closing properly. Food employees observed in kitchen area not wearing proper hair restraints, such as nets, hats, or beard covers.

Turkey Hill Minit Market #040, 1660 New Holland Road, Kenhorst , Nov. 16: four violations. Front door located in the entry area of the food facility has a large gap and does not protect against the entry of insects, rodents, and other animals. Milk in customer reach in refrigerator was beyond the sell by date and requires discarding. Corrected. Observed various boxes of food and containers of ice cream stored directly on the floor in walk in freezer area, rather than 6 inches off of the floor as required. Splash guard of the soda fountain and creamer dispensing machines have a build up of splash residue.

Chuck E Cheese #539, 2721 Meridian Blvd., Wyomissing , Nov. 15: one violation. Working container (spray bottle) in kitchen area, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid.

Donut filling injector nozzles are missing covers. Covered with plastic until covers are ordered. Food facility inspection indicates evidence of heavy fruit fly insect activity in donut case and floor drain areas, but facility does have a pest control program.

Governor Mifflin School District Middle School, 130 E. Lancaster Ave., Shillington, Nov. 9: two violations. Observed wet wiping cloths in serving line area, not being stored in sanitizer solution. Sanitizer dispenser was not functioning properly. Buckets to be filled by hand. Corrected. Sanitizer test strips were expired (2016) and not reading accurately. Discarded and new container will be brought from other kitchen.

Governor Mifflin School District Senior High, 10 S. Waverly St., Shillington, Nov. 9: no violations.

Weaver Orchard Inc., 40 Fruit Lane, Morgantown, Nov. 9: 10 violations. Mold build up along walls and floors of produce cooler must be cleaned. Lights are not shielded or shatter proof area in the donut shed. Observed wet wiping cloths in deli area, not being stored in sanitizer solution. Corrected. Soup was not cooled from 135°F to 70°F in 2 hours after preparation. Large pots of soup placed in walk-in freezer were still at 122 degrees at 2 hours. Pots removed and placed on ice bath for cooling. Corrected. Observed tray of cooked turkey food cooling at room temperature on a rack in bakery, which is not a proper cooling method. Corrected. The food facility does not have a handwash sink located in the produce prep counter area and ice cream shed area. Food facility inspection indicates evidence of heavy fly insect activity in deli/bakery areas. Rear door to the outside, located in the cider processing area of the food facility is not self-closing and/or is being propped open. Also open hole at floor drain line in donut shed must be sealed to prevent pest access. Ceiling tiles missing in the Doughnut shed area, and need replaced. Working containers in deli/bakery kitchen, ice cream shed area, used for storing —chemicals, cleaners— taken from bulk supplies, were not marked with the common name of the chemical. Corrected. Out of compliance.

Heritage of Green Hills, 200 Tranquility Lane, Reading, Nov. 8: six violations. Countertop ice dispenser and juice dispenser in café kitchen has a splash debris/mold build up on the splash guards behind the dispensing chute. Soap dispensers for handwashing are improperly installed by food prep sinks on tavern line and main kitchen at soda box sink area. Also soap dispensers are missing or broken at 3 handwash sinks on cook lines in tavern line and main kitchen warewashing area. Observed vacuum packed fish thawing on trays in walk in cooler still in packaging that clearly states to remove from package for thawing. Corrected. Water, as observed at handwash sink in corner at tavern is not under pressure during inspection. Walk-in cooler ceiling at fan box and kitchen ceiling vent areas of the food facility are extremely dusty and in need of cleaning. Food facility person in charge does not have records to demonstrate routine inspection and service of backflow prevention devices and other water treatment devices. Filter for ice machine in kitchen has late date of service of 1/20 and soda line filter has no date.

Food dispensing utensil in bins of sugars/flours observed stored in the food and not with handle above the top of the food and the container. Also containers without handles being used as scoops laying in product. Food facility has an original certificate posted, but the location is not conspicuous for public viewing.

Gramia's Restaurant, 924 W. Philadelphia Ave., Boyertown, Nov. 5: six violations. A food employee was observed touching cooked bacon — a ready-to-eat food — with bare hands. COS. Prepackaged pies are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts. Prepackaged pies are not labeled to clearly indicate any "Big 8" allergen ingredients and / or the allergen warning statement. Observed the sides of the fryer, and the floor under the fryer, grill, with encrusted grease and old food accumulation. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Eggs, and meats are served raw or undercooked to the customer's request; however, a written consumer advisory (on the menu, table tent, or placard) is not provided to the consumer. The Hobart meat slicer, a food contact surface, was observed to have food residue and was not clean to sight and touch. COS.

Gino's Cafe, 400 W. Broad St. Unit D, Shillington, Nov. 4: six violations. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Corrected. Prepackaged take out desserts are not labeled properly with ingredients or allergens and reach in case does not state that ""ingredients/allergens are available upon request"". Wooden pizza paddle is splintered and chipped. Observed clean food equipment and/or utensils on clean pan rack and plastic cup shelf area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Corrected. Ceiling fan in rear kitchen area of the food facility is extremely dirty, dusty, and in need of cleaning. Walk-in freezer has a heavy ice damming and ice condensate build up on ceiling and fan box/line area that is covering boxes of food products.

Wilson School District Whitfield Elementary, 2700 Van Reed Road, West Lawn, Nov. 4: three violations. Plastic utensils used for cooking are chipped/deformed and misshapen from heat, and are not durable under normal use in the food facility. Corrected by discarding. Curtain is missing at entrance to the mechanical warewashing machine. The hot water used for sanitizing in the mechanical warewashing machine did not reach 180°F (or 165°F for stationary rack).
