Berks food safety inspections: Rodent activity detected in several areas of Spring restaurant

Sep. 2—Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Aug. 17 and Aug. 30, 2022, that were filed in the state database as of Sept. 1 at 5 p.m.

Berks

Turkey Hill Minit Market #061, 3057 Saint Lawrence Ave., Reading, Aug. 30: no violations.

D'lish Creations, 360 E. Wyomissing Ave. Unit F Pa 19540, Mohnton, Aug. 29: one violation. Test strips for testing multi quaternary sanitizer were expired as of 07/2022.

Domino's #4749, 3801 Perkiomen Ave., Reading, Aug. 29: eight violations. Floor in the unused restroom is cracked / damaged and is not a smooth, easily cleanable surface. Floor in the restroom in the back area is cracked / damaged near the toilet and is not a smooth, easily cleanable surface. Side door (east) located in the back area of the food facility has gaps on all sides and does not protect against the entry of insects, rodents, and pests. The door jam also has a gap at the bottom, on the handle side of the door. One set of lights is not shielded or shatter proof in the back storage area. Roof of the food facility does not protect from weather and entry of insects/pests at the roof vent in the back room. The roof vent is not closable and the ceiling tiles are missing in this room. Perimeter walls of the food facility do not protect from entry of insects, rodents, and other pests as evidenced by holes in the walls being plugged with steel wool and other materials. The light intensity in the walk-in cooler is not sufficient for proper cleaning. The light intensity in the back room/dry storage area is not sufficient for proper cleaning. Multiple ceiling tiles are missing in the back area and need to be replaced. There is exposed insulation in this storage area due to the missing ceiling tiles. Food facility inspection indicates evidence of rodent activity in all areas. Facility may need to increase the frequency of their pest control program. Old unused equipment and other items stored in the back area should be removed from food facility. Out of compliance.

Frank's Fine Swine & Beef At Antietam Market, 905 Byram St., Reading, Aug. 29: no violations.

Sunset Diner, 6560 Perkiomen Ave., Birdsboro, Aug. 29: eight violations. Loose or broken door hinges observed on the ice machine lid in the basement. Lid/door is not closing tightly to fully protect the ice in the bin. Wall behind the ice machine is very deteriorated and is in need of repair. Some insect control devices (fly strips) located in the kitchen area with potential to contaminate food, equipment, and / or utensil (near slicer and freezer storage). Large metal scoop at the ice machine is cracked and is not in good repair or condition. Old dirt and debris observed on the dry storage shelves in the basement. Plumbing system not maintained in good repair — hot water not working at the handwash sink in the dishwashing area. Screen door to the outside, located in the basement area of the food facility is not self-closing and was observed to be open during this inspection. The door does not latch properly either, so that it will stay closed. Door was closed during the inspection, but needs to be repaired. One set of lights is not shielded or shatterproof in the dishwashing area.

There is a gap between the 3-compartment sink and the wall that will allow water/debris to accumulate behind the sink, an area that can't be properly cleaned. Food facility does not yet have available sanitizer test strips to determine appropriate sanitizer concentration at the 3-compartment sink. Exit door located near the restrooms has a gap at the bottom right corner and does not protect against the entry of pests.

Cherry's Jamaican Delight Mff4 (Zlm-3669), 711 Penn Ave., West Reading, Aug. 25: one violation. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration.

BB's Grocery Outlet, 150 Morview Blvd., Morgantown, Aug. 24: one violation. Severely dented and distressed canned items observed in the baby formula section are intended for use or sale in the food facility. Corrected.

Douglassville Hotel, 8 Old Swede Road, Douglassville, Aug. 24: four violations. Observed clean food equipment and/or utensils in the clean dish area, stored uncovered or not inverted. Exit door located in the food prep area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. An open employee's beverage container was observed in the cookline area, a food preparation area. Corrected. Observed commercially processed food not heated to 135°F before being placed in the steam table for hot holding as required. Corrected.

Heritage Center Snack Bar, Red Bridge Road, Reading, Aug. 24: no violations.

Jimmy G's Beverly Hills Tavern, 710 Old Fritztown Road, Sinking Spring, Aug. 24: seven violations. Observed deeply scored chipped cutting boards not resurfaced or discarded as required at exterior bar. corrected. Floor behind ice machine has alot of debris/mold from overflowing water pump and spilled food that is also goind into wall that must be repaired. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Spilled food under shelves in walk in and some equipment. Also beer cooler shelving and lines have mold build up. Observed clean food equipment and/or utensils in dish rack and bar area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Also pans on clean rack with tape and label residue. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in bar reach in refrigerator equipment. Shelving units in reach in kitchen refrigerator, under cutting boards on prep table shelf and walk in cooler are chipped/rusted, peeling and are no longer durable and easy to clean. Loose rubber door gaskets observed on the bain marie cooling unit. Part is ordered. Prebagged bucket of raw shrimp is not date marked when bagged. Corrected.

Ihop #5502, 2701 N. Meridian Blvd., Wyomissing, Aug. 19: one violation. Wall (coving tiles) near the floor drain at the the dishwasher is deteriorated. End coving tiles missing from the walls at the left-hand entrance to the server area. Wall between the office and the closet is damaged near the floor and is in need of repair.

BF Clams, 336 Kennedy Ave., Mertztown, Aug. 18: no violations.

Bowers Hotel, 298 Bowers Road, Bowers, Aug. 18: two violations. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Corrected. Cut cucumbers and commercially prepared sauces marked "refrigerate after opening" observed at 52°F, rather than 41°F or below as required. Corrected.

Caboose Crew (Zse-6103) Mff3, 4800 Perkiomen Ave., Reading, Aug. 18: no violations.

Domino's Pizza, 1 W Penn Ave., Robesonia, Aug. 18: one violation. The food facility does not have the original certificate for the certified food employee posted in public view. This is a repeat violation.

Exeter Family Restaurant, 4800 Perkiomen Ave., Reading, Aug. 18: three violations. Observed liquid dripping from the soda lines that run through the basement ceiling. This is scheduled to be repaired tomorrow. Torn/damaged door gasket observed on the Coke refrigerator in the kitchen. Outside door located in the basement area of the food facility and the side door in the kitchen have gaps at the bottom and do not protect against the entry of pests. Some of the light bulbs in the kitchen are not shielded or shatter proof. Corrected.

Johnny & Hons Smokehaus, 924 W. Penn Ave., Robesonia, Aug. 18: three violations. Steam table drain hose is down into floor drain and does not have a proper air gap, also drain needs a cover. Frozen foods pulled and placed in walk in are not re-dated properly with use by date. Deli slicer blade and guard a food contact surface, was observed to have food residue and was not clean to sight and touch.

Sun Valley Acres Mff3 T-Xfy2108, 529 Woleber Road, Myerstown, Aug. 18: one violation. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid.

CD's Place Restaurant, 237 N. Reading Ave., Boyertown, Aug. 17: one violation. Holes observed at floor-wall juncture under 3-compartment sink.

Grocery Outlet Of Shillington, 4200 Shillington Plaza, Reading, Aug. 17: three violations. Observed dairy cooler fan, ceiling area, with an accumulation of dust, dirt, debris on non-food contact surfaces. Also floors and walls at door have mold/spilled product residue and must be cleaned. Rear door located in the warehouse area of the food facility has a large gap and does not protect against the entry of insects, rodents, and other animals. Door threshold is deteriorated at bottom of door area and must be repaired. Also pallets of product stored against walls of warehouse. Observed eggs in cardboard and paper wrapped cheese food stored in customer reach in cooler area, where it is subject to splash from dripping condensate water from shelf above product. Items to be moved from those shelves. Observed several snack food displays stored directly on the floor in aisle area, rather than 6 inches off of the floor as required.

Julianas, 776 Fritztown Road, Sinking Spring, Aug. 17: 11 violations. Raw animal foods were stored stacked in a pan above ready-to-eat foods in the walk in cooler. Corrected. Observed food stored in bain marie refrigerator unit area, where it is subject to splash from build up of condensation water in bottom of unit. Also dripping grease build up along ansul system piping above cook line. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Floors in bar area have build up of broken glass, old food debris and dead insects. Also dough mixing room has a build up of old flour and dough pieces under shelves and mixer. Raw shellfish is being stored in open containers of ice that has no drainage in bottom causing shellfish to be submerged in the water. Corrected. Ceiling vent area of the food facility pizza kitchen area is extremely dirty, dusty, and in need of cleaning. Deli slicer blade and guard, a food contact surface, was observed to have old dried food residue and was not clean to sight and touch also front counter slushie machine has slight mold build up around dispensing nozzle area. The food facility does not have a handwash sink located in the food prep / warewashing area. Rear prep room hand wash sink is not in an area that is easily accessible and easy to use. An unapproved insect control device located in kitchen/counter area with potential to contaminate food, equipment, and / or utensils. Food facility inspection indicates evidence of rodents/fruit fly insect activity in bar and dough mixing room areas. However facility does have an extermination program. Shellstock tags are not maintained for 90 days from the date the container is emptied. Ceiling tiles missing in the kitchen area around new vent and need to be replaced. Wall in the rear french fry cutter area, has a hole, or is broken and in need of repair.

Muhlenberg Township

China Fun, 3611 Pottsville Pike, Aug. 28: six violations. Cooked shrimp/raw shrimp in the bain marie were held at 47.1/45.7°F, respectively, rather than 41°F or below as required. Food (cooked chicken/raw beef} in the walk-in refrigerator is stored open with no covering. Food dispensing utensil in bulk food bins (rice/MSG) observed stored in the food and not with handle above the top of the food. Trash can exterior, a non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Fan guard is soiled. Front door to the outside, is not self-closing and/or is being propped open. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers.

Pizzariffic Inc., 3201 Pottsville Pike, Aug. 28: 10 violations. Shredded cheese, was held at 43.3 °F, in the LARGE bain marie, rather than 41°F or below as required. Facility has multiple items stored that are unnecessary and have not been used in the facility. An open employee's beverage container was observed in pizza dough area, a food preparation area. Food employee observed in 3 compartment sink, not wearing proper hair restraints, such as nets, hats, or beard covers. Corrected on inspection. Exposed food preparation (at pizza oven area) under dusty ventilation ducts, and subject to potential contamination. Rubber spatula observed to be cracked with groove present, which is not easily cleanable. Wall in the dough prep area, has a hole, and is in need of repair. The back room/storage area of the food facility is dirty, and in need of cleaning. Sump pump residual observed on floor. Large bain marie upper lids do not fit securely, allowing temperature loss and unprotected food storage. Observed wiping cloths being stored/soaked in very dirty solution. Corrected on inspection. Lower shelf of dough table, underneath grill table, underneath grill, und3erneath fryers have heavy grease/food residue build-up. Paint remains chipped on wall adjacent to bain marie/dough prep table. The wall is not easily cleanable.

China Wok, 3401 N. Fifth Street Highway, Aug. 22: three violations. Several pieces of raw chicken/beef were stored uncovered in the chest freezer. Corrected on inspection. Observed deeply scored cutting board not resurfaced as required. Food was held at 46.1°F, in the lower section of the bain marie, rather than 41°F or below as required.

Redner's Warehouse Markets, 3205 N. Fifth Street Highway, Aug. 22: three violations. Ice bin exteriors are not a smooth cleanable surface in areas. Observed soiled apron stored on baking pan cart. Corrected immediately on inspection. Brooms, squeegees shall be stored to prevent contamination of equipment.

Haiku Hibachi & Sushi, 3215 N. Fifth Street Highway, Aug. 19: four violations. Food dispensing utensil in teriyaki sauce observed stored in the food and not with handle above the top of the food. Corrected on inspection. Observed raw shrimp stored in 3-compartment sink area, where it is subject to splashes from utility washing. Corrected on inspection. Opened salad dressing/tofu was held at 54°F, in the small refrigerator, rather than 41°F or below as required. Corrected on inspection with transfer of items to alternate refrigerator. Old food residue, utensils bowl and bottles of soap present in the sinks observed in the handwash sink, indicating uses other than handwashing. Corrected on inspection.

Sonic Drive-In #6886, 4515 Perkiomen Ave., Reading, Aug. 16: five violations. Observed a spray nozzle for an open container of walk-in freezer cleaner laying in an open box of plastic wrap. Corrected. Observed a bag of onions leaning against a bottle of weed killer near the walk-in cooler. Corrected. Scoop is being stored with the handle in direct contact with the drink ice at the drive-thru window. Corrected. Observed a bag of onions stored directly on the floor near the walk-in cooler, rather than 6 inches off of the floor as required. Corrected. Commercially processed food (beef) was not reheated to 135°F before being placed in the steam table for hot holding. Temperature was 52°F. Corrected. Foods were held at 60 °F in the bain marie unit rather than 41°F or below as required. Manager will keep the condiments and cut vegetables in this unit for a maximum of 4 hours and will then throw away any unused portion until this unit can hold foods at proper temperature. The 4-hour hold time must be logged and monitored.

Heritage Of Green Hills, 200 Tranquility Lane, Reading, Aug. 15: one violation. Several wall corners in kitchen area have broken molding coverings and wall panels are open at ends and must be repaired.

Heritage Of Green Hills Health Care Bldg, 400 Tranquility Lane, Reading, Aug. 15: three violations. Pressure measuring device for the water supply line for hot water sanitizing in the mechanical warewashing equipment is not in increments of 1 psi and/or accurate to +/- 2 psi in a range of 15-25 psi. Gauge for sanitizing pressure is over 50 psi. Mop sink needs to be sealed at wall/sink junction. Installation date is not noted on all new filters for new Equipment.

Ihop #5502, 2701 N. Meridian Blvd., Wyomissing, Aug. 15: eight violations. There is a water line leaking in the ceiling above the dishwashing area. The repair is scheduled for tomorrow. Food employee observed working with exposed foods (bagging cheese) with fingernail polish and / or artificial fingernails. Corrected. Observed an internal piece of the ice machine hanging down into the ice. The back wall and upper surfaces in the ice machine, a food contact surface, was observed to have residue and was not clean to sight and touch. Paper towel dispenser empty at the left-hand server station handwash sink. Corrected. End coving tiles missing from the walls at the left-hand entrance to the server area. Wall between the office and the closet is damaged near the floor and is in need of repair. Wall (coving tiles) near the floor drain at the the dishwasher is deteriorated. Observed an accumulation of food debris on the shelves in the True refrigerator at the far end of the cooking line. Corrected. Dunkin Donuts (Walmart), 5900 Perkiomen Ave., Reading, Aug. 12: four violations. Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by observed old flying insects in pest control device in the front counter area. Handwash sink at the front counter not operational because of a drain issue. Observed deeply scored cutting boards not resurfaced or discarded as required. Manual warewashing equipment (sink sprayer) observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day.
