Clay County’s Department of Health begins Back To School Immunization Drive Monday

The Florida Department of Health in Clay County begins their annual Back to School Immunization Drive today at the Clay County Fairgrounds. The drive will run Monday through Friday from 8 a.m. to 4 p.m. until August 12.

Children entering preschool and grades K-12 can receive diphtheria, tetanus, pertussis (whooping cough), measles, mumps, rubella, polio, varicella (chickenpox) and hepatitis B vaccines at no cost. Photo IDs and current immunization records are required.

According to a recent press release, 7th grade students require a T-DAP, and all students require an updated Florida Certification of Immunization (DH680) form documenting receipt of vaccinations.

“Per County resolution 2020/2021-102 AM2, there will be a $12.00 fee for any child receiving a DH680 or DH681 form without vaccine services,” they said.

College students are advised to check their college requirements for immunizations.

Children under 18 must be accompanied by a “natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor.”

A step-parent, grandparent, an adult sibling of the minor, an aunt, or an uncle can accompany a minor child with written consent from a natural or adoptive parent, legal custodian, legal guardian, or a person who possesses a power of attorney to provide medical consent for the minor that can be reached by phone.

Parents and guardians are encouraged to early as doors may close early due to high demand.
