Fredericksburg Restaurant Inspections: Fortune House, Osan

FREDERICKSBURG, VA — Health and restaurant inspections are back in full swing in the Fredericksburg area. During the most recent round of inspections from the health department, 22 different locations recorded violations, according to the Rappahannock Area Health District.

The most recently published inspections occurred between Feb. 9 and Feb. 15.

Throughout January, most of the health department's efforts were focused on training visits at restaurants around the region. Health department officials conducted virtual training sessions to help prevent the spread of COVID-19 during the omicron surge.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Osan Japanese Restaurant, 707 Caroline Street Fredericksburg, VA 22401

    • Date inspected: Feb. 9

    • Total violations: 9

      • Observed food employee wash hand in less than 10 seconds. PIC had food employee re-wash hands.

      • Observed food employee wash hands at 3 compartment sink. EHS reminded food employee of where to wash. Food employee washed hands at the 3 compartment sink.

      • No written agreement/statement from the supplier/aquaculturist stipulating that the salmon were raised (open water in net pens; or land-based operations such as a farm and tanks) and pellet fed.

      • Observed the reuse of single use/service articles throughout the establishment. Seaweed salad containers: 1. In the 2 door reach in cooler, one container of shreded carrots and one of shredded cabbage. 2. On the prep table next to the fryer, three containers of breading. 3. In the sushi bar reach in cooler, one container of cut greens. 1 gallon Mayonnaise bottles: 1. In the glass door reach in cooler, five gallons of Yakiniku sauce. During the inspection, the PIC discarded the food.

      • The following equipment are in need of repair: 1. The small reach in freezer in the sushi bar. 2. The 1 door reach in freezer is missing castor wheels.

      • The following are in of cleaning: 1. All shelves that are lined with cardboard. 2. The duct tape between prep table and prep unit.

      • The following are in need of repair: 1. Several lights are out in the kitchen. 2. Wall paint is peeling behind the hot water dispenser. 3. The floor throughout the kitchen.

      • The kitchen floor under and around equipment is in need of cleaning.

      • Observed a hat hanging over several stacks of plates (top plate was saran wrapped). PIC relocated the hat.

  • Fortune House, 282 Deacon Road Unit 108 Fredericksburg, VA 22405


    • Date inspected: Feb. 9

    • Total violations: 37

      • The person in charge has failed to ensure employees are complying with the requirements of the Regulations. Observed a lack of date marking, a lack of hand washing at required points during food preparation, and bare hand contact with ready to eat food, and soiled equipment.

      • Person in charge was unable to provide documentation of training relating to the employee health policy for this facility. EHS will provide an employee health policy when reviewing the report.

      • Per discussion with the person in charge, no procedures for responding to vomiting and diarrheal events were in place. The EHS will provide a handout when reviewing the inspection report.

      • Observed three uncovered tumblers with drinks in the kitchen near food. The person in charge relocated the drinks.

      • Observed a food employee rinse their hands with water. Hands were not effectively cleaned due to not applying soap.

      • Observed a food employee handle soiled dishes and personal belongings then proceeded to engage in food preparation without first washing their hands and donning gloves. Employee was asked to wash their hands.

      • A food employee was observed cleaning their hands at the three compartment sink. Employee was instructed to wash their hands only at the handwashing sink.

      • Observed a food employee handling cabbage and egg rolls with their bare hands. The items were discarded and the employee was instructed to use gloves or utensils.

      • Soap was not provided at both hand washing sinks in the kitchen. Person in charge provided soap.

      • Observed several damaged cans stored in the dry storage area. The person in charge relocated the cans.

      • Observed several uncovered containers of marinated raw chicken, cooked vegetables, cooked egg rolls, and cooked shrimp in the refrigeration units. A food employee covered the foods.

      • Observed shelled eggs and different types of raw animal foods stored in such a manner that may cause cross contamination. 1- shelled eggs and raw beef were stored over ready to eat food, 2- raw chicken was stored over raw shrimp.

      • The following food contact surfaces were observed soiled to sight and touch: the meat grinder, several containers, lids, tongs, and knives. Items were removed to be washed, rinsed and sanitized.

      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods.

      • Chlorine sanitizer solution in sanitizer bucket, to later be used on food contact surfaces, was measured at greater than 200 parts per million chlorine. Solution was diluted to approximately 100 parts per million.

      • Observed two bottles of bug spray and a bottle of Lysol spray stored near food and equipment. The person in charge relocated the bottles.

      • Observed the following: 1- tree nuts stored in a metal baby corn container, 2- several bulk containers in big unlabeled containers. The person in charge relocated the tree nuts and labeled the containers.

      • Observed the rear screen door of the facility not tight fitting. Main exterior door was broken. This may allow the entry of insects or rodents.

      • Observed the following: 1- two bags of meat in a metal pan stored directly on the floor in the walk-in cooler, 2- several containers of sauce on the floor in the dining room area, 3- several bags of meat stored on the floor in the walk-in freezer, 4- a bag of red onions stored directly on the floor by the exterior door.

      • Observed employees in the back of the house preparing food without proper hair restraints.

      • Observed several wet wiping cloths on the counter. The person in charge placed the wet cloths in the sanitizer bucket.

      • Observed the following: 1- single service cups stored in the rice cooker and in several containers being used for dispensing, 2- a knife stored between the prep table and the sandwich prep unit.

      • Observed several single service articles boxes stored directly on the floor in the dining room area.

      • Observed stainless steel food containers stacked while wet on the warewashing shelving units near the 3 compartment sink. Discussed requirements with the person in charge.

      • Observed single service containers were saved and re-used to store other food items. The person in charge discarded the items.

      • Observed a to go menu being used as a liner under cooked spring rolls in the prep unit. Observed shredded carrots, cooked shrimp, cooked wonton, and raw chicken stored directly in a non-food grade bags in the walk-in cooler.

      • Observed wooden shelves and several non-food contact surfaces throughout the kitchen with a non-smooth surfaces and not easily cleanable.

      • Observed the following: 1- The walk in freezer door was observed with a large accumulation of ice, 2- tape on the rice cooker handle.

      • Observed the following: 1- duct tape being used for labeling, 2- the blade on the meat dicer is broken, 3- wooden food contact utensils and equipment are being used.

      • Observed the following items in need of cleaning: 1- Fryer cabinets 2- the top of the fryers, 3- the interior of the refrigeration units, 4- foil used for shelf lining on the cook's line, 5- outside of the food containers, 6- storage shelves, 7- the interior of the chase freezer.

      • Observed the handwashing sink and toilet in the women restroom in need of deep cleaning.

      • Observed no waste receptacles near the handwashing sinks.

      • Observed the following in need of repair: 1- the hood system is collapsing, 2- chipped paint near the mop sink, 3- damaged ceiling tiles throughout the facility, 4- broken floor tiles in the walk-in cooler. 5- re-caulk three compartment sink.

      • Observed a ladder stored near equipment.

      • Observed the hood system and filters soiled with grime and debris.

      • Observed several employees' personal belongings stored next to food, single service articles, and equipment throughout the facility. The person in charge relocated the items.

      • Observed 0.0 foot candles of light inside the women restroom.

  • Carrabba’s Italian Grill, 1951 Carl D. Silver Pkwy Fredericksburg, VA 22401


    • Date inspected: Feb. 9

    • Total violations: 10

      • Observed the absence of paper towels at the bar hand washing sink. During the inspection the person in charge provided paper towels.

      • Observed the top interior surfaces of the ice machine soiled with mold like growth.

      • Observed the interior surfaces of the microwave heavily encrused and soiled.

      • Observed the following:1. Four containers of minestrone soup cold holding at unsafe temperatures (42-51 degrees F). Per discussion with the person in charge soups had been cooled and stored from the day prior. 2. One container of fresh sliced garlic in oil at the hot line (76 degrees F). During the inspection the person in charge discarded the containers of minestrone soup and the container of garlic oil for safety.

      • Observed two containers of heavy cream left on the counter to thaw out. EHS discussed with the person in charge approved thawing methods. Heavy cream was removed to the walk in cooler.

      • Observed several cutting boards heavily stained.

      • Observed caulking missing in parts of the handsinks of the dish washing area, prep area and 3 compartment sink area.

      • Observed the following: the non food contact surfaces of the floor mixer, counter mixer, and interior of the fryer cabinets heavily soiled to the sight and touch.

      • Observed the following: several cracked floor tiles throughout the kitchen, 2 blown out lightbulbs in the women's bathroom.

      • Observed the following in need of cleaning:floors underneath equipment throughout the kitchen area, ac and ceiling vents throughout the kitchen, walls behind and around the handsinks, walls behind the3 compartment sink and walls around the dish washing area area.

  • Kenmore Inn, 1200 Princess Anne Street Fredericksburg, VA 22401


    • Date inspected: Feb. 9

    • Total violations: 5

      • The facility does not currently have a person that is a certified food protection manager.

      • Observed the interior lip of the ice machine is in need of cleaning.

      • Observed 1 box of paper towels and 1 box of single service use lids stored on the floor in the dry storage area. During the inspection the person in charge moved the boxes off the floor.

      • Observed several plastic spatulas with the handle melted/warped.

      • Observed floors underneath equipment are n need of cleaning.

  • Always Sonshine Learn and Play, 11919 Rutherford Drive Fredericksburg, VA 22407

    • Total violations: 1

      • Observed boxes of cereal stored on the floor. The person in charge relocated the boxes.

  • Tropical Smoothie Cafe, 15 South Gateway Drive Suite 113, Fredericksburg, VA 22406


    • Date inspected: Feb. 10

    • Total violations: 16

      • The person in charge could not provide documentation of employee health training but was knowledgeable of health reporting requirements. Sample employee health policy provided during the inspection.

      • Observed a food employee lather their hands less than 10 seconds with hand soap. Instructed the employee to lather their hands for at least 10 seconds with hand soap. The employee rewashed their hands.

      • Observed container of various utensils soiled on the drainboard next to the sanitizer basin and the food thermometer soiled with debris. The container of utensils and thermometer were removed to the 3 compartment sink to be cleaned and sanitized.

      • Observed interior of the microwave soiled.

      • Observed significant accumulation of debris on the interior surfaces of the ice machine.

      • The food establishment is adding unapproved food additives in smoothies for consumption by customers. Ingredients included in the Collagen Peptides, multivitamin and probiotic support powders and energizer and fat burner tablets are not included on the FDA Approved Additives Status list. Informed the person in charge these products are not permitted to be used in beverage preparations. The food establishment may provide the products to customers in a single use container which they may add to their beverages upon request. The person in charge agreed to comply.

      • Observed packaged food in the walk in freezer stored under leaking water which has condensed and frozen. During the inspection the person in charge relocated the food under the leak to prevent contamination. Discontinue food storage under the leak until repairs are conducted.

      • Observed food employees wearing wrist jewelry.

      • Observed single use articles used to package food for guests stored with the food contact surface facing upward on the top of the large prep cooler under open shelving with noticeable dust accumulation. Discussed adequate storage to prevent contamination. Also observed packaged single use beverage lids stored inside the cabinet under the smoothie rinse sink. The person in charge relocated the items and discarded opened or heavily soiled sleeves of lids.

      • Observed heavily scratched and scored cutting board on the drainboard at the 3 compartment sink and beverage prep cooler.

      • The Quaternary Ammonium testing kit in the facility expired November 2021. Instructed the person in charge to purchase a new test kit.

      • Observed significant accumulation of grime and debris on all nonfood contact surfaces of equipment including inside refrigeration equipment, vents of reach in refrigeration equipment, exterior surfaces of blenders, panini press and shelving throughout the facility.

      • Observed accumulation of trash and debris around the trash enclosure.

      • Observed both dumpster doors open. Discussed closing them to prevent attraction of pests.

      • The facility is in need of a deep and thorough cleaning of the facilities including the floors, walls, ceiling tiles and vents, including under the 3 compartment sink and inside walk in refrigeration units.

      • Observed 3.5 foot candles of light inside the walk in cooler.

  • Bangkok Cafe, 825 Caroline Street Fredericksburg, VA 22401


    • Date inspected: Feb. 10

    • Total violations: 14

      • Observed a food employee touch their mask and another food employee handle soiled dishes then proceeded to engage in food preparation without first washing their hands and donning gloves. Employees were asked to wash their hands.

      • Observed tongs and serving spoons that were soiled to the sight and touch. The items were placed by the dishmachine to be washed.

      • Observed 2 boxes of shelled eggs (65-69'F) stored on the floor in the servers area. The eggs were delivered overnight and left on the floor. The eggs were discarded.

      • Observed 2 bottles of bug spray and a bottle of Lysol stored next to food and single serve articles. The person in charge relocated the bottles.

      • The exterior door which is located in the kitchen is not tight-fitting. This may allow the entry of insects or rodents.

      • Observed several cockroaches on the wall by the handwashing sink in the kitchen. Pest control company was contacted and a technician arrived during the inspection.

      • Observed several bags of Jasmine rice stored directly on the floor.

      • Observed in-use utensils stored in water at a temperature of 72'F. Utensils were moved to be cleaned.

      • single service manufacturer containers were saved and re-used to store other food items.

      • Observed several containers of single service articles and equipment stored on the floor in the storage area by the stairway.

      • The following were observed in need of repair. 1) water collecting on the bottom shelf of refrigeration unit by the counter, 2) gasket is broken in the reach in cooler.

      • Accumulations of debris were noted on the following nonfood contact surfaces: 1- vents and interior of reach in refrigeration units, 2- interior of reach and chest freezers, 3- shelving units by the dishmachine, 4- cart by the frying station.

      • Observed the following areas in need of cleaning: 1) the wall behind the fryer station, 2) ceiling tiles and vents. 3) kitchen floor. 4) kitchen walls.

      • Observed the following in need of repair:1) three compartment sink needs re-caulking, 2) Missing tiles under the ice machine, 3) several ceiling tiles throughout the facility need to be re-adjusted.

  • Metro Diner, 2068 Plank Road Fredericksburg, VA 22401


    • Date inspected: Feb. 10

    • Total violations: 9

      • Observed a container of butter sitting out uncovered and unprotected from the environment by the front service line. During the inspection the person in charge covered the container of butter.

      • Observed the interior surfaces of the ice machine were soiled.

      • Observed the cutting board at the cooks line prep area are heavily stained. Person in charge stated they have been ordered to be replaced.

      • Observed plastic food containers stored stacked while wet.

      • Observed numerous containers of clean silverware stored with the food contact surface inverted upwards.

      • Observed the dish room hand sink, 3 compartment sink and sink surrounding dish machine in need of caulking.

      • Observed the following nonfood-contact surfaces soiled: inside the fryer cabinets, inside surfaces and gaskets of cooling/freezing units, a cart used to transport dirty and clean dishes between hot line and dish room.

      • Floors, walls, ceiling/hood vents and floor drains throughout the kitchen are in need of cleaning.

      • Observed the following in poor repair: Two lights out at the hood vent of the prep room, floors at the cooks hot line is damaged.

  • China Garden, 17010 Dahlgren Rd #3 King George, VA 22485


    • Date inspected: Feb. 11

    • Total violations: 8

      • The cavity of the microwave oven and the toaster oven were observed soiled. Both units were cleaned during inspection

      • The food contact surfaces of 4 meat cleavers hanging above the 3 compartment sink and the cutting boards along the prep units were observed soiled. All items were taken to the dish machine to be cleaned.

      • The prepared ready-to-eat (RTE) pork egg rolls (3 baskets) in the walk in cooler are not properly dated for disposition. Rolls were date marked during inspection.

      • Observed a large gap along the bottom of the outer screen door in the kitchen.

      • he following nonfood contact surfaces have accumulations of grime and debris: 1-the inner and outer surfaces of the deep fryer cabinets, including the sides of the cabinets, 2-the outer surfaces of the oven and cooktop, 3-top shelf of the expo station, 4-the wok station cart, 5-the wire shelving throughout the kitchen, 6-the spice shelf, 7-the shelf under the hot holding unit, 8-the wok station gas handles and surrounding areas, 9-the bins holding the single service condiments at the expo station, 10-the outer surfaces of the rice storage bin. IN GENERAL, ALL SURFACES IN THE KITCHEN ARE IN NEED OF A THOROUGH CLEANING.

      • The following physical facility items in the kitchen were observed soiled: 1-the caulking along the 3 compartment sink (mold), 2-the ceiling throughout the kitchen (dust), 3-the covers of the light fixtures (dusty).

      • Observed several cracked light covers in the kitchen.

      • The filters of the hood ventilation system over the wok station are heavily soiled.

  • Stafford Hospital, 101 Hospital Center Blvd. Stafford, VA 22554


    • Date inspected: Feb. 11

    • Total violations: 9

      • Observed a food employee handle soiled pans then proceeded to engage in food preparation without first washing their hands and donning gloves. Employee was asked to wash their hands.

      • Observed the following: 1- a box of raw bacon stored over ready to eat food in the walk-in cooler, 2- a container of raw chicken stored over and next containers of ready to eat food. The person in charge relocated the items.

      • Observed a food employee handle pans and containers then handle ready to eat muffin and bread without first washing their hands and donning new gloves. The items were discarded.

      • The following food contact surfaces were observed soiled to the sight and touch: several pairs of tongs, lids, and other utensils. The person in charge moved the items to be washed, rinsed, and sanitized.

      • Observed six individually packaged pizzas (125-130F) hot holding in the hot holding station at improper temperatures. The pizza boxes were stacked on top of metal racks. The food employee reported the pizzas were sitting in the unit for less than an hour. The person in charge removed the metal racks and instructed the food employee to reheat the pizzas to 165'F.

      • Observed several cartons of whole milk cold holding at improper temperature (45-46'F) in the reach in refrigerator- the cartons have been in the unit for three hours- the person in charge placed the items in the walk-in cooler. EHS checked temperature an hour later and it was 40'F. The person in charge in the reach in cooler.

      • Observed an insect control device located over drinks by the dry storage area. The person in charge relocated the drinks.

      • Observed several dead flies glued to the fly traps by the warewashing area.

      • Observed a broken container and several broken lids. The person in charge discarded the items.

  • Burger King, 5320 James Madison Parkway King George, VA 22448


    • Date inspected: Feb. 11

    • Total violations: 2

      • The regular sized hamburgers was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria. They were only cooked to an internal temperature of between 145-148'F. Person in charge place the burgers back in the broiler for a short time. After this, the interior temperature of the burgers were 162'F and above. The belt speed on the broiles was slowed down.

      • The surfaces of the wall in the kitchen and the flkoors in the dry storagfe room and near the bag in box shelves were observed soiled.

  • Papa Johns Pizza, 3940 Plank Rd Suite W Fredericksburg, VA 22407


    • Date inspected: Feb. 11

    • Total violations: 2

      • Observed food employee scrub hands under running water and in less than 10 seconds. During inspection, EHS demonstrated proper handwashing procedure. Food employee rewashed before returning to work

      • The 3 compartment sink and handwashing sink next to the 3 compartment need to be sealed to adjoining wall.

  • Pizza Bono, 10426 James Madison Parkway King George, VA 22485


    • Date inspected: Feb. 11

    • Total violations: 5

      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.

      • The ready-to-eat (RTE) commercially processed sliced bologna, sliced turkey, sliced ham and sliced pastrami. in the walk in cooler was not properly dated for disposition after opening. The items were date marked during the inspection.

      • Observed gaps along the edges of the rear screen door in the kitchen. Also, the screen was noted to be torn on the door.

      • The nonfood contact surfaces of the following items are in need of cleaning: 1-the spice shelf, 2-outer surfaces of the reach in freezers, 3-the wire shelving in the walk in cooler.

      • The following physical facility items are not maintained in good repair: 1-a ceiling vent cover is missing over the reach in freezer, 2-the walls in th rear kitchen and rough and damaged, 3-missing floor tiles in front of the 3 com[partment sink, 4-the coving around the walk in cooler is peeling loose.

  • Royal Farms #297, 5000 Plank Rd Fredericksburg, VA 22407


    • Date inspected: Feb. 11

    • Total violations: 6

      • Observed food employee scrub hands under running water. EHS discussed proper handwashing procedure with food employee. The food employee rewashed hands before returning to work.

      • Observed a food employee breading chicken while wearing a watch. EHS informed PIC of observation. PIC had food employee remove the watch.

      • The folded take out boxes under the fried chicken hot holding unit were observed covered in breading.

      • The caulking at the 3 compartment sink and the handwashing sink next to the 3 compartment sink were observed soiled.

      • The mop sink is in need of cleaning.

      • Observed 3 personal beverages on the shelf over boxes of plastic cups. During the inspection, the PIC removed the beverages.

  • Gloria Luz Market & Restaurant, 432 Garrisonville Road Suite #9 Stafford, VA 22554


    • Date inspected: Feb. 14

    • Total violations: 15

      • The person in charge has failed to ensure employees are complying with the requirements of the regulations. Observed hot holding, cold holding, improper thawing methods, date marking, bare hands contact, and handwashing violations.

      • Person in charge was unable to provide documentation of training relating to the employee health policy for this facility. Provided employee health policy sample.

      • Observed food employee cleaning their hands at the three compartment sink. Employee was instructed to wash their hands only at the handwashing sink.

      • Observed food employee wash their hands for less than 5 seconds without applying soap and drying their hands. Observation was discussed with the person in charge.

      • Observed food employee handle soiled dishes and proceeded to engage in food preparation without first washing their hands and donning gloves. Employee was asked to wash their hands.

      • Observed food employee handle ready to eat cut cabbage with their bare hands. the cabbage was discarded.

      • Observed food employee handle raw chicken then changed tasks without removing the soiled gloves and washing their hands. The observation was discussed with the employee. Gloves were discarded and hands were washed.

      • Observed a pot of black beans (110'F) hot holding at improper temperature for less than one hour- the person in charge reheated the beans to 165'F and will start monitoring the temperature to ensure it remains above 135'F.

      • Observed a container of cut cabbage that was prepared over 48 hours prior to time of inspection cold holding at improper temperatures (65'F) in the reach in cooler. The item was discarded.

      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods. Discussed requirements with the person in charge.

      • Observed a bottle of all purpose cleaner (Lysol) stored next to single serve articles on the shelving unit. The person in charge relocated the bottle.

      • Observed a plastic container of black beans thawing on the prep table by the grill. The person in charge placed the item in the reach in cooler.

      • Observed rodents droppings in different areas in the facility.

      • Observed an uncovered bowl of cut cabbage stored inside of a trash can. Corrected on site.

      • Observed several wet wiping cloths on the counter. The person placed the wet cloths in the sanitizer bucket.

  • McDonald's Restaurant #33884, 8265 Ladysmith Road Ruther Glen, VA 22546


    • Date inspected: Feb. 14

    • Total violations: 3

      • No hand drying provisions were provided at the handwashing sink in the kitchen near the office. Paper towel was provided during the inspection.

      • The gaskets on the doors of the chicken reach in freezer are torn.

      • The faucet on the hand sink near the office in the kitrchen is leaking.

  • Mason-Dixon Cafe, 11 Hope Rd Suite #115 Stafford, VA 22554


    • Date inspected: Feb. 14

    • Total violations: 17

      • The person in charge could not provide documentation of employee health training, he reported the documents were held off site. Instructed the person in charge to email employee health documents and ensure documentation is provided during inspections. The person in charge was knowledgeable of health reporting requirements.

      • Observed a food employee handle soiled equipment wearing single use disposable gloves and then proceed to store cleaned and sanitized equipment from the dishwasher wearing the same gloves. Discussed using gloves for one purpose only and instructed the food employee to remove gloves and wash her hands.

      • Observed interior surfaces of the ice machine soiled.

      • Observed the following TCS foods cold holding at improper temperatures inside the small prep cooler in the front service area: In house prepared cardamon butter (51 degrees F) and potato salad (45 degrees F). The person in charge reported the foods were pulled from the walk in cooler at 8:45 a.m. The facility will use time as a public health control until the unit is serviced. The person in charge placed a note on the cooler to discard TCS foods by 12:00 p.m.

      • Observed shell eggs (46, 50 degrees F) cold holding at improper temperatures on the top of the small prep cooler. Observed the containers overfilled. The person in charge reported the eggs were pulled from the walk in cooler at 8:00 a.m. (temperature taken at 9:20 a.m). The eggs were relocated to the walk in cooler to cool to 45 degrees F or less by 12:00 p.m. Observed shell eggs cooled to 42 degrees F by 10:19 a.m.

      • Observed packaged bread stored on top of a cart which is less than 12 inches from the surface where raw chicken is breaded for cooking in the fryer. Discussed storage of packaged bread on lower shelves to prevent cross contamination. The bread was relocated during the inspection.

      • Observed two wiping cloth buckets without any measurable concentration of Quaternary Ammonium sanitizer. The person in charge discarded the sanitizer during the inspection.

      • Observed multiple stacks of plastic and stainless steel containers stacked while wet after chemical sanitizing on the open shelving in the warewashing area.

      • Observed the handsinks in the back of the house and 3 compartment sink are no longer sealed to the adjacent walls.

      • Observed the following food contact equipment in poor repair: Melted plastic lid inside the hot holding unit, melted spatula handle, chipped rubber spatula and flaking sauté pan on the warewashing area shelving area. The equipment was removed from service during the inspection.

      • The person in charge could not provide a Chlorine testing kit to monitor the sanitization of the low temperature dishwasher. Instructed the person in charge to purchase a test kit.

      • Observed accumulation of grime and debris on the following nonfood contact surfaces: Inside fryer doors, base of upright freezers, upsplash of the pre-wash nozzle by the dishwasher, exterior surfaces of the dishwasher.

      • Observed the oil receptacle is in poor repair, oil appears to be leaking onton the ground extending towards the dumpster enclosure The person in charge reported he has been in contact with the contractor.

      • The walls and ceilings throughout the facility and floor drain under the convection ovens, mop sink and caulking along all sinks were observed in need of cleaning.

      • Observed the following in poor repair: Missing ceiling tiles in the back of the house, ceiling tile not secured to the frame by the 3 door reach in freezer, and numerous blown out ceiling lights throughout the back of the house.

      • Observed the sealant on the concrete floor in the back of the house has eroded. The floor is no longer easily cleanable.

      • Observed 26 foot candles of light at the cook line and 15 foot candles of light at the dishwasher. Observed burned out shielded lights under the hood and numerous burned out ceiling lights in the warewashing area.

    • Total violations: 12

      • Employee failed to wash their hands after returning from the outdoor walk in cooler and continuing food prep. Employee was instructed to wash their hands after returning from outside of the kitchen.

      • The food contact surfaces of the food slicer in the kitchen was observed soiled. Person in charge said that it was used the previous day. Sliced was cleaned during inspection.

      • The interior food contact equipment surfaces of the ice machine was observed soiled. Machine was cleaned during inspection.

      • The prepared ready-to-eat (RTE) beef lasagna and cooked pasta in the refrigeration unit is not date marked. Items were dated during inspection.

      • The following commercially processed ready-to-eat (RTE) temperature controlled for safety foods in the refrigeration units were not date marked: 1-the deli sliced ham, 2-deli sliced turkey, deli sliced salami, 4-sliced cheese and other items in the walk in cooler.

      • The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control: cooked onions and cooked peppers. Items were labeled during inspection.

      • Improper methods used to thaw ROP fish. Fish was voluntarily discarded during inspection.

      • Two food employees were observed preparing food without hair restraints.

      • The clean side dish machine staging table is not sealed to the wall.

      • The following non-food contact surfaces were observed soiled with grime and debris 1-the top of the microwave oven, 2-the back of the convection oven, 3-the grease disposal container near the outdoor walk in cooler, 4-inside of the deep fryer cabinets.

      • The rear handsink is coming loose from the wall.

      • The ceiling intake air vents in the kitchen were observed soiled.

  • Casey's, 622 Kenmore Avenue Fredericksburg, VA 22401


    • Date inspected: Feb. 15

    • Total violations: 9

      • Observed ice in the kitchen handwashing sink. EHS spoke with the person in charge proper usage of handwashing sinks.

      • Observed a box of coffee filters and dry and clean sanitizing disposable towels stored near a box of toxic chemicals. During the inspection the person in charged moved the coffee filters and disposable towels away from the toxic chemicals.

      • Observed several stainless steel food containers stored stacked while wet.

      • Observed a box of roll paper towels stored on the floor of the dry storage area. During the inspection the person in charge relocated the box of paper towels.

      • The 3 compartment sink and dishwashing area need caulking.

      • Observed several cutting boards melted and heavily scratched and scored.

      • Person in charge could not provide a test kit for the chemical sanitizing dish machine or test kit for the quaternary sanitizer used in the facility

      • Observed several small holes of the kitchen ceiling tiles.

      • The following were observed in need of cleaning: floors and drains underneath equipment, walls of the dishwashing room.

    • Date inspected: Feb. 15

    • Total violations: 25

      • Observed absence of active managerial control in the food establishment during the inspection. The person in charge is not monitoring temperatures of TCS foods during cold holding and reheating for hot holding, improper cooling methods observed and the sanitization of the dishwasher is not being monitored. Ensure food employees are adequately trained on food safety risks to prevent foodborne illness.

      • The person in charge could not provide documentation of employee health training. Food employees were knowledgeable of health reporting requirements to reduce foodborne illness risks.

      • Observed absence of hand drying provision at the warewashing, cook line and bar handsinks. The person in charge replaced paper towels at the cookline and warewashing handsinks during the inspection.

      • Observed packaged raw pork stored over container of baking potatoes and packaged raw beef stored over onions in the walk in cooler and wrapped raw salmon stored over container of cut red peppers in the prep cooler. The raw animal foods were relocated to prevent cross contamination.

      • Observed the following foods uncovered without adequate overhead protection: Opened container of salsa on ice on a prep table in the kitchen, opened container of cut lemons at the beverage station, uncovered pitchers of margarita mix in the bar 3 door reach in cooler, and uncovered containers of cut onions in the walk in cooler. The person in charge placed a protective wrapping or cover on the food containers to prevent cross contamination.

      • Observed the following food contact equipment soiled: Vegetable dicer, immersion blender and cutting board. The equipment was removed to the dishwasher for cleaning.

      • Observed food contact equipment is not being adequately sanitized after cleaning in the low temperature dishwasher. Observed Chlorine sanitization at 10 ppm after 3 cycles. The temperature gauge was not readable due to build-up on the exterior of the thermometer. The person in charge contacted a technician during the inspection and agreed to set up the 3 compartment sink to wash and sanitize food contact equipment and utensils until repairs to the dishwasher are completed.

      • Observed the following TCS foods improperly reheated: Charro beans (144 degrees F), Enchilada sauce (131 degrees F) and charro beans (130 degrees F). The person in charge reported the TCS foods were reheated at 10:40 p.m., temperatures were taken at 11:31 a.m. The TCS foods were removed from the hot holding unit (charro beans) and on top of the convection oven (enchilada sauce and charro beans) and placed on the range to reheat to 165 degrees by 12:40.p.m. Observed charro beans and enchilada sauce reheated above 165 degrees within the required timeframe.

      • Improper cooling methods to rapidly cool in house prepared chicken broth observed. Observed chicken broth (43 degrees F) stored in a deep and covered plastic food storage container in the walk in cooler. The person in charge reported the broth was prepared the evening prior at 8:00 p.m. and placed in the container. The facility is not monitoring cooling temperatures of TCS foods. Discussed cooling time and temperature requirements during the inspection. The person in charge voluntarily discarded the chicken broth for safety.

      • Observed the following TCS foods cold holding at improper temperatures: Prep cooler - Cut lettuce (43, 43, 43 degrees F), shredded cheese (45, 44, 45, 44 degrees F), cooked chicken wings (43, 44 degrees F), cooked pork (44, 43 degrees F), raw salmon (44 degrees F). The person in charge reported the cut lettuce and shredded cheese in the top of the unit were pulled from the walk in cooler at 10:40 a.m. and the TCS foods on the bottom of the unit have been in the unit overnight. Observed accumulation of water on the base of the unit during the inspection. The facility will use time as a public health control (4 hours) until the unit is repaired. The raw salmon, shredded cheeses, cooked chicken wings and pork held in the unit overnight were voluntarily discarded during the inspection. Observed Garlic in oil (70 degrees F) in a container stored on top of the convection oven. The person in charge reported the mixture was prepared at 10:40 a.m. The person in charge placed a sticker on the bottle to discard at 2:40 p.m. Discussed holding the garlic in oil in refrigeration until ready for use. Observed container of salsa (43 degrees F) in on ice which was only contacting the bottom of the container. The person in charge reported the salsa was pulled from the walk in cooler at 10:40 a.m., temperature was taken at 11:22 p.m. The facility will use time as a public health control (4 hours) and will discard at 2:40 p.m. The person in charge placed a note on the container with the discard time.

      • Observed the following TCS foods which were held longer than 7 days from preparation: Rice (prepared 2/7/22), flan (prepared 2/6/22), and horchata (unknown preparation date). The TCS foods were voluntarily discarded during the inspection.

      • Observed a chemical spray bottle which was not labeled to identify the contents. The person in charge could not verify the contents of the bottle and voluntarily discarded for safety.

      • Observed chemical spray bottles stored hanging on the open wire shelving over cleaned equipment. The chemical spray bottles were relocated to prevent contamination.

      • Observed improper cooling methods to rapidly cool in house prepared chicken broth. Observed chicken broth at 43 degrees F in the walk in cooler. The person in charge reported the chicken broth was prepared the evening prior. Observed the broth in a deep and covered plastic container. The facility has an ice wand but the person in charge reported it was not used to cool the broth.

      • Observed covered containers of food are being stored under leaking water lines inside the walk in cooler. The facility has placed a large container under the leak to prevent contamination, which was observed nearly full. Instructed the person in charge to relocate the food to prevent contamination and discontinue food storage in this area until repairs are completed.

      • Observed bottles of beer stored on the floor inside the beer walk in cooler and a large container of tortilla chips stored on the floor by the cook line. The container of tortilla chips was placed on a dunnage rack during the inspection.

      • The food establishment is reusing manufacturer food containers for food storage. Observed spice containers are being reused to store other spices and for storage of horchata. Instructed the person in charge to discontinue.

      • Observed shelving inside the walk in cooler and prep cooler has become rusted and is not longer smooth and easily cleanable.

      • Absence of alarm or other visual or audio means to alert staff if detergent or sanitizer are not delivered to complete the washing and sanitization cycle requirements.

      • The facility does not have an adequate Quaternary Ammonium testing kit. The kit provided is discolored. Instructed the person in charge to purchase a new test kit.

      • Accumulation of debris observed inside reach in refrigeration units, exterior surfaces of the microwave ovens, lid of the oil storage container outside the back door, exterior of the dishwasher thermometer, and inside the left side fryer door.

      • Observed the floor under the prep sink and dishwasher are not smooth or easily cleanable. The surface appears to be unsealed concreate.

      • The floors, walls, ceiling light covers throughout the facility, toilet room sinks, mop sink and caulking at the dishwasher area are in need of cleaning.

      • Observed four blown out shielded lightbulbs in the hood vent, numerous blown out ceiling lights in the back of the house and ceiling light cover by the dry storage room is not secured to the frame and is hanging which presents a safety risk.

      • Observed 26 foot candles of light at the cook line and 1.4 foot candles of light inside the men's toilet room stall. Observed 4 blown out shielded lightbulbs in the hood vent.

      • Observed employee wash her hands with cold water. Observed the employee get soap, rinse it off, then continue to wash her hands. EHS discussed this with the Person in charge.

      • Observed that the inside of the hot holding trays have a build up of grime and debris.

      • Observed that the soda nozzles in the drive thru and the upsplash area on both soda machines are in need of detailed cleaning.

      • Observed containers of chemicals stored with the cooking oil and a spray bottle of degreaser stored by the slicer. The items were removed.

      • Observed single service food containers stored with the food contact surface facing upwards. The containers were inverted.

      • Observed that the cutting boards are heavily scored and stained.

      • Observed the following non-food contact surfaces that require cleaning: inside of the refrigeration units, metal knife holder, sides of the fryer station, cabinets under the fryers

      • Observed that the back flow prevention device is missing on the sprayer hose at the mop sink.

      • Observed that the sprayer hose is sitting below the flood rim at the 3-compartment sink.

      • Observed that the floors under the equipment, walls in the single service storage area, and ceiling to include the air vent covers are in need of detailed cleaning.

The following locations received a visit from the health department during the most recently published week of inspections, but recorded no violations.
