Fuzzy black bananas. Lots of roaches. The Midlands restaurants with the worst April inspections

South Carolina health inspectors found multiple violations at restaurants in the Midlands in April, from heavy roach activity in kitchens and storage areas to rotting and moldy fruit stored with other food. The state each month releases grades and inspection reports for food establishments across Lexington and Richland counties.

Here are the restaurants that received some of the lowest ratings in April.

K Seafood Market at 1929 Broad River Road in Richland

K Seafood Market had an inspection on April 18 and got a C grade.

An inspector observed employees washing dishes directly above raw fish in a compartment sink and the soapy water flowing directly from the dishes into the sink with the fish. Also observed dishes being stored as clean without sanitizing.

Hush puppies were seen inadequately cooling in a large fryer basket left at room temperature. A large accumulation of debris was seen on the floor of the walk-in cooler beneath shelving.

A required followup inspection was held on April 24 and the market got an A grade.

Miyo’s on Forest at 3250 Forest Drive in Richland

Miyo’s on Forest had an inspection on April 26 and got a C grade.

An inspector observed heavy roach activity around the to-go bags and in the dish area.

Open raw fish was seen stored with ready-to-eat food in reach in the freezer. Also seen were dishes with label residue and food matter stored as clean, biofilm on dish racks and food splash, food debris and heavy encrusted buildup on all equipment throughout kitchen areas.

Buildup on flooring under equipment in the kitchen areas was also observed.

A required followup inspection has not yet been held.

Taqueria Guadalajara at 1807 Decker Blvd. in Richland

Taqueria Guadalajara had an inspection on April 6 and got a C grade.

An inspector observed water waste from the freezer being rerouted to the hand sink in the kitchen. Also observed were cooked beans being inadequately cooled in a large, covered tub on the bottom shelf of the cooler. A bag of cooked rice was also seen inadequately cooling.

Also, a gap was seen at the bottom of the back door beside reach in refrigeration.

A required followup inspection was held on April 13 and the restaurant got an A grade.

Applebee’s at 4505 Devine Street in Richland

Applebee’s #1133 had an inspection on April 21 and got a C grade.

An inspector observed dead roaches on the floor under the dish washing machine. Also observed was a gap in the bottom of the walk-in cooler door and standing water on the floor of the cooler.

Grease was seen on the ground surrounding the grease bin, along with damaged floor tiles and ceiling tiles throughout the building.

The restaurant had a required followup inspection on April 27 and got an A rating.

Bistro on the Boulevard at 1085 Lake Murray Blvd. in Lexington

Bistro on the Boulevard had an inspection on April 20 and got a C grade.

An inspector observed roach activity in the kitchen, fly activity throughout the facility and that the back door was propped open multiple times without an adequate secondary method of preventing entry by pests.

Certain food containers were seen showing signs of fermentation such as bulging. Also seen were covered pans of meatballs stored in the middle of the walk-in cooler inadequately cooling.

Food buildup and old adhesive were seen on dishes stored as clean.

The restaurant had a required followup inspection on April 25 and earned an A grade.

Cautros Caminos at 2920 Fairview Road in Lexington

Cautros Caminos had an inspection on April 17 and got a C grade.

An inspector observed flying insects on and near fruits stored in the storage area. Also observed were black bananas with fuzzy white substance on them, kept next to other fresh fruits.

Also seen were eggs that were not clean or from an approved source and bagged and boxed single serve items stored directly on the floor in the kitchen and storage area. Soiled puddling water near the back door was seen and was said to be discarded mop water.

The restaurant had a required followup inspection on April 27 and got another C grade. It then had a second followup inspection on April 28 and earned an A grade.

El Jimador at 1072 Lake Murray Blvd. in Lexington

El Jimador had an inspection on April 20 and got a C grade.

An inspector observed small flying insects in the dish washing area and fuzzy growth on peppers and tomatoes stored with in use inventory. Also observed was packaged single serving pizzas stored in direct contact with repackaged raw hamburger patties in a chest freezer inside the walk-in cooler.

Buildup of food debris was seen on dishes and equipment stored as clean.

The restaurant had a required followup inspection on April 25 and earned another C grade. The restaurant had a second followup inspection on April 27 and got an A grade.
