How To Get A Georgia COVID-19 Vaccination Card Online

GEORGIA — As the omicron variant of COVID-19 continues to drive up cases and the pandemic wears on, vaccines continue to be touted as the number one tool to fight back. And the longer the pandemic goes on, and the more widespread vaccine requirements become, the more Georgia residents might have need to show a vaccine card.

In fact, last year, Decatur and Brookhaven became the first metro Atlanta cities to require their employees, including first responders, to get vaccinated against COVID-19 or undergo weekly coronavirus tests to remain employed, the AJC reported.

Georgia residents receive a Centers for Disease Control card when they get a shot. For those who lose their card, an official proof of vaccination can be requested online through the Georgia Department of Public Health. You can request a copy of your immunization records at no cost.

When you start the process online, you won't necessarily receive another COVID-19 vaccine card, however, your immunization records will confirm any vaccinations you may have received for COVID-19.

If you've lost your COVID-19 vaccination card, the Georgia Department of Public Health offers these steps you should take to request proof of immunization:

Gather What You'll Need

  • The full name, date of birth, and mother’s full name of the person whose immunization records you are requesting.

  • Valid, government-issued identification of the person requesting the immunization records. (Acceptable forms of identification include a state-issued photo driver’s license with address, a state-issued photo identification card with address, or a U.S. passport or passport card with photo.)

Request Immunization Records

  • Download, fill out, and sign the Immunization Record Request Form.

  • Submit the request form and a copy of your government-issued ID to the Department of Public Health’s Immunization Registry by mail, email, or fax using the instructions on the form.

Next Steps

  • Allow at least 3-5 business days for processing.

  • You will receive an encrypted message with your complete immunization record from the DPH via email.

  • If you believe a vaccine dose is missing from your records, please contact the healthcare provider that administered the vaccine.
