Greene Co. food inspection reveals moldy chicken, restaurant's risk control violations

A Springfield restaurant was put on a risk control plan after violating a previous risk control plan, a plan outlining how a restaurant aims to control the risk of foodborne illness.

Among buckets of dried out and moldy chicken without a use-by date, Springfield-Greene County Health Department inspectors found that Creasian, 501 W. Chestnut Expressway, had deviated from their current risk control plan. A requirement of the plan was to maintain written documentation of how long a ready-to-eat potentially hazardous food was not being kept hot or cold, but the last time the restaurant documented that was in mid-January. All potentially hazardous food will now have to be held above 135 degrees or below 41 degrees.

The inspector also found rat poison on the floor in the storage room and under an employee hand washing station, as well a dead and decaying mouse in the storage room.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

Also found that area of sink under potato dicer has moderate build-up. No priority violations. One non-priority violation: Observed no paper towels in men's restroom. Promptly corrected by placing roll of paper towels in bathroom.

Galloway Station, 1281 E. Republic Road, Springfield. March 20 routine inspection – Result: permitted with re-inspection. One priority violation: Observed several potentially hazardous foods held at 46 degrees or above, including ham at 50 degrees and cream cheese at 48 degrees, in the make table next to the grill. Items discarded and re-inspection will be performed. No non-priority violations.

Golden Palace, 1313 W. Kearney St., Springfield. March 21 routine inspection – Result: active. Two priority violations: 1. Observed rangoon stored in a non-food-grade container. Observed tongs stored on table next to oven with grease build-up; sheet pan of fries stacked on top of another sheet pan of fries with no barrier in between bottom of pan and fries. IHOP No. 5436, 2647 N. Kansas Expressway, Springfield. March 20 routine inspection – Result: active. Plumber called and front two floor drains draining properly within 45 minutes of arrival. Facility uses lactic acid sanitizer, which was at proper concentration. No priority violations. Four non-priority violations: 1. Observed front two floor drains full of wastewater upon inspector's arrival. Establishment had already called a plumber but called another when inspector emphasized importance of getting situation corrected. Floor drains were draining properly within 45 minutes of plumber being contacted. 2. Observed tube for milk dispenser sticking out about 1-and-a-half inches, and was cut off straight. 3. Observed the following areas in poor repair – grout in kitchen floor, floor tiles and some coving is broken or missing. 3. Observed some areas of floor in kitchen and walk-in refrigerator and some equipment surfaces were not clean.

Observed chicken placed in Cambro container measure at 125 degrees. Education provided – do not store foods in Cambro container if they cannot maintain 135 degrees or above. 7. Observed container of fried chicken sitting out next to fryer. Chicken tested at 50 degrees. Corrective action – Chicken was placed out 30 minutes prior so was moved back to walk-in cooler. Corrected on-site. 8. Observed multiple food items, (chicken, noodles, rice, beef, etc.) were not date marked. Corrected on-site. 9. Observed food items on display not marked for time as control. Chicken at 101 degrees and bourbon at 98 degrees in steam table. Several other items out of temperature and foods were not marked for time as control upon arrival. Corrected on-site by person in charge marking appropriate start timing. Foods will be discarded at the end of 4 hours. Foods cannot be saved or reused. When bringing new foods out to be put on time as a control, they cannot be marked for new time and be combined with old foods. 10. Observed employee wash hands in three-compartment sink by spraying gloved hands with mop hose, then proceeded to put cleaned dishes away. Had employee rewash dishware, remove gloves and wash hands in handwashing sink before returning to work. Two non-priority violations: Observed bowl of raw chicken stored on top of cucumbers in walk-in cooler, which was corrected on-site; observed opened plastic bag of chicken being stored in handwashing sink. Person in charge voluntarily discarded chicken and education was provided.

The Roost, 2025 W. Sunshine St., Springfield. March 20 routine inspection – Result: permitted with re-inspection. Discussed proper storage of par-cooked chicken wings. Two priority violations: Observed no sanitizer residual in dish machine, which was corrected at time of inspection; observed foods held at temperatures above 41 degrees in small under-counter cooler. Three non-priority violations: Observed hot water at 87 degrees in men's and women's restrooms; observed no handwashing sign in men's restroom or combined restroom; observed behind and under equipment, floors and walls are dirty.

The Sub Shop, 456 W. McDaniel St., Springfield. March 20 routine inspection – Result: active. One priority violation: Observed small silver fridge tested at 53 degrees for ambient air and potato salad tested at 53 degrees. Disposed of five containers of potato salad, and fridge will not store potentially hazardous food any more. No non-priority violations.

Tropical Smoothie Cafe, 3080 N. Kentwood Ave., Springfield. March 22 routine inspection – Result: active. Two priority violations: Observed can opener blade and can opener holder was dirty, education given and corrected during inspection; observed vegetable dicer not clean, education given and corrected on-site. Four non-priority violations: Observed daylight along bottom of back exterior door, which is repeat violation; observed old date labeling sticker residue on outside of food containers; observed employee personal items, food and drinks not stored in designated areas; observed walk-in cooler door has latch that sometimes prevents door from closing properly.

USA Investments MO123, 2953 N. National Ave., Springfield. March 17 routine inspection – Result: active. One priority violation: Observed dented cans on shelves. Education given and items voluntarily removed from sales floor. Corrected on-site. Two non-priority violations: Observed creamer dispensing tube found with greater than one inch protruding from chilled dispensing head, which was corrected on-site; observed vents in sandwich cooler found not clean.
