  • Observed knife stored in the cervices of prep top cooler. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a); COS: Knife was moved to sink for cleaning.

  • Observed chlorine dishwasher incapable of rinsing utensils. Washed utensils and equipment shall be rinsed so that abrasives are removed and cleaning chemicals are removed or diluted through the use of water or a detergent-sanitizer solution. C/A: Advised PIC to hand wash dishes until dishwasher is fixed.

  • Observed floor tiles in poor repair in dishwashing area. All physical facilities shall be maintained in good repair.

  • Observed heavy presence of fruit flies throughout facility. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by eliminating harborage conditions. C/A: PIC was unable to provide pest control invoices. PIC stated that there was a clogged pipe in facility.

  • Observed raw shelled eggs stored above vegetables. Also, observed cooked chicken stored next to raw shrimp. Advised PIC that all raw animal foods must be stored according to final cook temperature. Raw shelled eggs and cooked chicken was relocated.

  • Observed rice on countertop and in walk in cooler not held at proper cold holding temperatures. Advised PIC the all TCS foods cold holding must be held at 41F or below. Rice discarded.

  • Observed spray bottle filled with purple chemical unlabeled. Advised PIC that chemical must be labeled with common names. COS- chemical bottle was labeled.

  • A follow-up inspection will be held Jan. 17.

  • Observed failure to have any sanitizer buckets set up for cleaning food contact surfaces and utensils. CA: Chlorine sanitizer must be set up in buckets and wet wiping cloths must be stored in sanitizing solution when not in use.

  • Observed items stored at front food service prep top cooler failing to maintain temperatures of 41 degrees F or lower (see temperature observation). CA: All items out of time were discarded onsite. Advised that maintenance be performed on the unit. Follow up scheduled to check on repair status.

  • Observed potentially hazardous foods not held at 135 degrees Fahrenheit of above at steam table and stove top near grill. CA: All items found out of temp were discarded onsite. Hot held items must maintain temperatures of 135 degrees F or higher.

  • Remarks: No sanitizer present during time of inspection.

  • A follow-up inspection is scheduled for Jan. 18.
