ABOUT RESTAURANT INSPECTIONS

This is a snapshot of the inspections conducted by the Lane County health department.

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

Max's Tavern

Location: 550 E 13th Ave., Eugene

Soap is not provided at a handwashing sink, specifically: Soap is not provided at coffee bar and dishwashing room handwashing sinks. Point deduction: 3.

A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: New single door prep cooler is blocking handwashing sink in kitchen. Point deduction: 3.

Ready-to-eat food is not properly date-marked, specifically: Half and half creamer in kitchen two-door reach-in cooler, container of hummus in two-door prep cooler reach-in and gallon of milk in coffee bar reach-in cooler are held longer than 24 hours and not date marked. Point deduction: 3.

A variance has not been obtained as required, specifically: Housemade, hot-brined pickles (dated June 9) are stored in sealed jars in kitchen prep cooler and walk-in cooler. Point deduction: 3.

Location: 293 Valley River Center G-0015, Eugene

Gloves are used improperly, specifically: A food worker is wiping gloves off with a towel periodically while making sushi. Point deduction: 5.

A consumer advisory is not provided, is not presented in the correct format, or does not contain the required information, specifically: The consumer advisory for the menu board is missing the words, "meat, poultry" and "shellfish or eggs." Point deduction: 3.

Rennie's Landing

Location: 1214 Kincaid St., Eugene

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Food in the undercounter refrigeration unit in the server station is above 41 degrees Fahrenheit, specifically whipped cream aerosol cans (54 degrees) and half gallon lime juice (49 degrees). Point deduction: 5.

Poisonous or toxic materials are not properly separated or located, specifically: Two paint brushes wrapped in plastic wrap are stored above a case of beer in the walk-in cooler in the basement. Point deduction: 5.

Abby's Pizza Inn

Location: 1374 U.S. Highway 101, Florence

Food employees eat, drink or use tobacco in unapproved areas or use an inappropriate beverage container for drinking, specifically: Two cups with employee drinks were in open containers without a tight fitting lid and straw/handle. The open cups were near customer drink cups and the potential for contamination from spillage is possible. Point deduction: 3.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: The prep table cooler by the fryers is not holding food at temperature control, 41 degrees Fahrenheit. A work order had been placed to repair the cooler ahead of the inspection. However, potentially hazardous food is still being held in the cooler. Operators have been storing food in the walk-in and transferring the food over to the prep table cooler at 11:30 a.m. The food is then stored out of temperature control for the work day, then placed back into the walk-in. Various foods, including chicken, tomatoes, oil garlic mix, etc., were at around 50 degrees at 2:30 p.m. The prep table top-area cooler holds pizza sauce and garlic at 45 degrees. Point deduction: 5.

Unrefined Wellness Bar

Location: 2776 Shadow View Drive, Eugene

Food employees do not use the proper cleaning procedure for washing their hands, specifically: Observed food employee turn water off halfway through handwashing process with their bare hands and then turn it back on to complete handwashing and then turned off the faucet without using a paper towel. Point deduction: 5

Hand towels or a hand drying device is not provided at the handwashing sink, specifically: Paper towels are not available at the handwashing sink in the back kitchen area near the walk-in cooler. Point deduction: 3.

A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: Observed food handler rinse espresso portafilter in the handwashing sink in the front service area. Point deduction: 3.

Ready-to-eat food is not properly date-marked, specifically: There are two bottled items in the open display cooler that are not properly date marked. Point deduction: 3.

Unapproved or inappropriate levels of food or color additives, or sulfites are added to foods, specifically: Facility is using coconut activated charcoal powder. Facility is also using two types of spirulina powder. It is not clear if these are arthrospira platensis. Point deduction: 5.

Temperature measuring devices are not provided or readily accessible for checking food temperatures, specifically: Facility is unable to located probe thermometers during inspection. Point deduction: 3.

A handwashing sink does not provide the minimum water temperature or is not equipped with a mixing valve or combination faucet, specifically: The bar handwashing sink is not equipped with hot water. The hot water handle does not provide any water. Point deduction: 3.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: The upright cooler next to the grill area is not holding potentially hazardous foods at the proper cold holding temperature: container of ribs temperature between is 44-49 degrees Fahrenheit. Butter temperature is 48-49 degrees, shredded cheese temperature is 50 degrees. The ambient air thermometer also reads over 41 degrees. The upright warmer is also not holding potentially hazardous foods at the proper hot holding temperature. The tri-tip temperature is 124 degrees, cooked tri-tip in the steam table near the prep cooler temperature is 94 degrees. Point deduction: 5.

Ready-to-eat food is not properly date-marked, specifically: Bloody mary mix stored in the bar walk-in is not properly dated. Corn dogs stored in the reach-in cooler in the main kitchen area are not properly date marked. Point deduction: 3.

Working containers of poisonous or toxic materials are not clearly labeled, specifically: Spray bottle containing yellow sanitizer stored behind the bar is not properly labelled. Point deduction: 3.

Theo's Coffee House

Location: 199 W Eighth Ave., Eugene

Priority violations:

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Container of cream cheese and package of sliced cheese stored in single door reach-in cooler (located in back room) measure 47-48 degrees Fahrenheit. Point deduction: 5.

Ready-to-eat food is not properly date-marked, specifically: Container of open cream cheese, held past 24 hours, is not date-marked. Point deduction: 3.
