Here’s the latest rundown of rules for every COVID-19 vaccine site
Florida’s rollout of the COVID-19 vaccine has made it nearly impossible for the public to keep track of the rules.
The vaccinations have been expanded to now include Florida residents age 60 and older, which is a move this week that makes about 2 million more people in the state eligible. The eligibility change applies at state-supported walk-up and drive-thru sites as well as pharmacies and pop-up locations.
Some vaccination sites require doctor’s notes; others don’t. Some take younger people; some take teachers and law enforcement.
And homebound residents can now request a vaccination at their residence by sending an email to homeboundvaccine@em.myflorida.com.
Here are the rules:
Jackson Health
Who may get the vaccine?
Any Florida residents ages 16 and older who say that their physician recommended they “receive the vaccine due to your high-risk condition” can be vaccinated. No documentation or proof will be needed. Those ages 16 and 17 must have a parent or legal guardian present at the appointment to provide consent. “We’re assuming everyone is going to go forward in good faith, but we don’t know that,” said Lidia Amoretti, hospital spokeswoman. Any condition considered high-risk by a doctor is acceptable, she said.
People age 60 and older.
Police, firefighters, and teachers for grades K-12.
What do you need to prove eligibility? A Florida’s driver’s license or ID card. Sworn law enforcement officers must have a valid badge. Firefighters and school personnel must have a valid work ID.

Broward Health
Who may get the vaccine?
People age 60 and older.
All health care providers.
People ages 18 and older with high-risk conditions with one of 20 conditions, including diabetes, liver disease, cancer and smoking.
Sworn officers, firefighters and teachers 50 and older.
What do you need to prove eligibility? A Florida’s driver’s license or ID card.
Do you need an appointment? Yes. Appointments can be requested at BrowardHealth.org.
Where is the vaccine offered? Inter Miami CF Stadium at Lockhart Park, at 1350 NW 55th St. in Fort Lauderdale.
Cleveland Clinic
Who may get the vaccine?
People age 60 and older.
People ages 18 and older with high-risk conditions.
What do you need to prove eligibility? Established Cleveland Clinic patients with a Florida driver’s license or ID card.
Do you need an appointment? Yes. Established patients will be contacted through MyChart, or will receive a phone call to schedule a vaccine appointment.
Where is the vaccine offered? In Broward, the locations are:
The Cleveland Clinic Weston Hospital, Jagelman Conference Center, 2950 Cleveland Clinic Blvd. in Weston.
In Palm Beach County, the location is the Tomsich Health and Medical Center, 525 Okeechobee Blvd., Rosemary Square Offices, first floor in West Palm Beach.
Holy Cross Health
Who may get the vaccine?
People any age with one of 13 high-risk conditions, which include asthma, current cancer, chronic kidney disease, sickle cell, and heart failure.
People age 60 and older.
What do you need to prove eligibility? A photo ID and, if under 60, a doctor’s note or the state form proving vulnerability.
Do you need an appointment? Yes. To request an appointment, drop off the registration form/consent along with a doctor’s note to the COVID Vaccine Center. The documentation will be reviewed and someone from the Vaccine Center will get back to you about an appointment.
Where is the vaccine offered? Holy Cross Health COVID Vaccine Center, 4701 N. Federal Highway, Bldg. C. in Fort Lauderdale.
Memorial Healthcare System
Who may get the vaccine?
Current Joe DiMaggio Children’s Hospital patients 16 or 17 years old with one of 16 high-risk medical conditions, including asthma, HIV and diabetes, are eligible for vaccination.
All people ages 60 and older.
People ages 18 and older who have seen a Memorial physician in the last year for one of 14 high-risk conditions including asthma, HIV and obesity.
What do you need to prove eligibility? A Florida driver’s license or ID card. A seasonal resident can show a deed or monthly mortgage statement.
Do you need an appointment? Yes. Appointments can be requested through the patient’s MyChart account or call 954-276-4340.
Where is the vaccine offered? Memorial Regional Hospital, 3501 Johnson St. in Hollywood, and Memorial Vaccine Village, 9581 Premier Parkway in Miramar.
CVS
Who may get the vaccine?
People age 16 and older with a state form that’s signed by their physician, in which the person is deemed extremely vulnerable to the virus. What do you need to prove eligibility? A photo ID and, if applicable, the state form proving vulnerability.
Do you need an appointment? Yes, at www.cvs.com/immunizations/covid-19-vaccine. Patients also may call 800 746-7287. A spokeswoman said appointments are open as they receive vaccine, and there are no specific days or time.
Where is the vaccine offered? It’s being offered at 157 CVS Pharmacy, CVS Pharmacy y más and Navarro Discount Pharmacy locations. across Florida.
Publix
Who may get the vaccine?
All people age 60 and older.
Adults under the age of 65 who are determined to be extremely vulnerable to COVID-19 by a physician. The state’s completed form, deeming someone extremely vulnerable to COVID, is required.
All K 1/4 u201112 and child care teachers and personnel, who receive priority among the eligible groups.
Health care professionals with direct patient contact.
Firefighters and police age 50 and older.
What do you need to prove eligibility? Adults under the age of 60 who meet the state’s criteria of being extremely vulnerable to COVID-19 are required to provide the completed state form. Other groups do not need identification.
Do you need an appointment? Yes. Vaccinations are provided by appointment only at publix.com/covidvaccine at 7 a.m. Mondays, Wednesdays and Fridays.
Where is the vaccine offered? All Publix pharmacies.
Walmart
Who may get the vaccine?
People 60 and older.
Health care workers.
Emergency Medical Services personnel.
Sworn law enforcement, ages 50 and older.
Firefighters, ages 50 and older.
People age 16 and older with at least one high-risk medical condition as determined by a physician.
Educators.

Winn-Dixie
Who may get the vaccine?
People age 60 and older.
Sworn law enforcement and firefighters age 50 and older.
Grades K-12 school personnel of all ages.
Medically vulnerable adults.
What do you need to prove eligibility? You must show Florida residency and the medically vulnerable must bring the state-required form to the appointment.
Do you need an appointment? Yes. Sign-ups are at www.frescoymas.com/pharmacy/covid-vaccine and www.winndixie.com/pharmacy/covid-vaccine.
Where is the vaccine offered? In Broward, the Winn-Dixie location is at 5600 W. Sample Road in Margate. In Palm Beach County, the Winn-Dixie is at 6600 Hypoluxo Road in Lake Worth. The Miami-Dade locations are Fresco y Más Store, 5850 NW 183rd St. in Hialeah, and Fresco y Más,18300 SW 137th Ave. in Miami. The three Miami Winn-Dixie store locations are 1150 NW 54th St., 1155 NW 11th St. and 11241 SW 40th St.
Walgreens
Who may get the vaccine? A company spokeswoman did not respond, and eligibility information for Florida was not on its website.
What do you need to prove eligibility? Patients are required to attest to their eligibility before the appointment is made, according to a company spokeswoman. “They also must sign an affidavit confirming they meet their state’s eligibility requirements for the vaccine upon arriving at their vaccine appointment and show a valid government ID to confirm their identity. Unless specifically required by a jurisdiction, we will not turn way individuals who do not produce an ID.”
Do you need an appointment? Yes. The sign-up is at Walgreens.com/ScheduleVaccine or by phone at 1-800-WALGREENS.
Where is the vaccine offered? There are now 100 stores in 19 Florida counties. In South Florida, that includes Palm Beach County with two stores in Loxahatchee, one in Belle Glade and one in Wellington.
Florida Department of Health in Broward
Who may get the vaccine?
Vaccinations for sworn law enforcement and firefighters age 50 and older.
Vaccinations for K-12 school personnel age 50 and older.
Medically vulnerable.
People age 60 and older.
What do you need to prove eligibility? Driver’s license or other proof of age. Health care workers need proof of employment such as an identification badge. School staff also need a school identification. The medically vulnerable must bring the state-required form to the appointment.
Everyone being vaccinated needs proof of Florida residency.
Do you need an appointment? Yes. Register online at myvaccine.fl.gov or call 866-201-6313 (TTY 833-476-1526). You will be contacted when an appointment is available.

What do you need to prove eligibility? Those medically vulnerable must have the state form signed by a medical professional.

Federally supported vaccine sites in Miami-Dade
Who may get the vaccine?
People age 60 and older.
Health care personnel with direct patients.
K-12 school employees of any age.
Sworn law enforcement officers and firefighters age 50 and older.
Medically vulnerable who have the state-required form.
What do you need to prove eligibility? Those under age 60 must have the state form for the medically vulnerable, as well as proof of age, and proof of Florida residency. On the ground, some people who’ve received vaccines at Miami sites have reported little to no restrictions.
Do you need an appointment? Appointments are not required at the federally supported COVID-19 vaccination sites, but pre-registration is permitted at https://myvaccine.fl.gov/.
If a person arrives to the site and all time slots are filled for that day, staff will work with them to schedule an appointment. Individuals also may schedule appointments on site in a separate registration lane if they do not want to wait for walk-up availability.
Where is the vaccine offered? Miami Dade College North Campus, 11380 NW 27 Ave. in Miami. It’s one of four sites in Florida that are supported by the Federal Emergency Management Agency.
Two satellite locations are also open through Wednesday, March 17:

Who may get the vaccine?
People age 60 and older.

Health care workers and EMS personnel.
People age 16 and older with high-risk conditions, with the signed state form.
Sworn law enforcement officers and firefighters age 50 and older.
What do you need to prove eligibility? The state form is required for the medically vulnerable, and teachers, police and firefighters must show proof of employment, such as a badge, when they arrive at the site to receive their vaccine.
Do you need an appointment? To get an appointment you must call 1-888-499-0840 (TTY#: 1-888-256-8918 ). You will be contacted when first-dose appointments become available.
Second-dose appointments are scheduled at the site immediately after individuals receive their first dose.
