How to look up your doctor's disciplinary history in Ohio

1. In your internet browser, go to med.ohio.gov/License-Lookup.


2. Click the circle that says “eLicense” on the left side of the page. You will be taken to a query page.


3. Enter a doctor’s name and information in the fields, and hit the blue search button at the bottom of the page. You can enter as little as one field. (For example, if you only know a doctor’s last name, you can start with that.)


4. Search results will appear below the query fields. If your search yields multiple names, scroll through to find your doctor. City and county fields may help you narrow your search.


5. When you find your doctor, look under the ‘Board Action?’ column. The field will say ‘Yes’ if the medical board has taken formal action against him or her. Click the ‘Yes’ to call up additional details about the disciplinary action the board took against the licensee.


6. In the top right of the disciplinary detail window, click “View” to see publicly available documents about the board’s discipline of the doctor, including hearing notices, board votes, hearing reports and recommendations and appeals. The documents typically are ordered from newest to oldest.


Other tips:

  • Within the search results, click the drop-down arrow on the far right of a doctor’s listing to see license issue and expiration dates.

  • If you are unsure of spelling, use % as a wildcard in place of unknown letters.
