Marion County restaurant inspections: Critical violations Aug. 1-15

The Louisiana Department of Health performs regular inspections of restaurants and stores that sell food. These are the local results of those inspections.

The following restaurant inspections with critical violations were conducted by Marion Public Health between Aug. 1 and Aug. 15:

Taco Bell #38893, 1270 Delaware Ave., Marion, Aug. 1. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed nacho chips stored uncovered in the kitchen. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed build-up of debris on food scissors. Unapproved chemical sanitizer used on food contact surfaces (critical, corrected). Observed quaternary ammonia sanitizer at incorrect concentration. Food contact surfaces not easily cleanable (critical, corrected).

Observed cracked water pitchers. Person in charge (PIC) discarded the pitchers. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment or utensils are placed (critical). Observed drain line from walk-in cooler not having a large enough air gap at the floor drain.

Buffalo Wild Wings #576, 1573 Marion Mt. Gilead Road, Marion, Aug. 3. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed bar dishwasher dispensing less than the required sanitizer concentration. Employees are using the three-bay sink for sanitizing until the dishwasher is repaired.

Marion Motomart South, 1297 S. Prospect St., Marion, Aug. 3. Presence of live insects, rodents and other pests (critical, corrected during inspection). Observed some live flies. PIC was advised to contact pest control company for persisting issues.

Meijer #111 (gas station), 2153 Marion-Mt. Gilead Road, Marion, Aug. 3. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed TCS food not cold held at the proper temperature in the Arctic Air unit and sandwich case. PIC discarded all affected foods and will put a work ticket in for repairs.

Tin Star Saloon, 104 E. Main St., Waldo, Aug. 4. Equipment food-contact surfaces or utensils are unclean (critical). Observed build-up of debris in ice maker. Presence of live insects, rodents and other pests (critical). Observed mouse droppings on kitchen floor and in office.

American Legion Post #605, 367 W. Main St., Waldo, Aug. 4. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed build-up of debris in ice maker. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed boiled eggs and opened package of cut lettuce not date marked.

Clase All Occasions Catering LLC, 6989 Waldo-Delaware Road, Waldo, Aug. 4. PIC unable to demonstrate knowledge of temperatures and times for cooling and reheating (critical, corrected during inspection). PIC was re-trained on the process. Food contact surfaces not easily cleanable (critical, corrected). Observed deeply scored cutting boards. PIC discarded all affected cutting boards.

Cruiser's Pizza Subs & Suds, 2827 Marion-Marysville Road, Marion, Aug. 8. Presence of live insects, rodents and other pests (critical, corrected during inspection). Observed live flies. Advised PIC to contact a pest control company. Food contact surfaces not easily cleanable (critical, corrected). Observed deeply scored cutting board. PIC discarded the cutting board.

Baires Deli LLC, 131 S. Main St., Marion, Aug. 8. Wild mushrooms from an unapproved source (critical, corrected during inspection). Observed mushrooms that were purchased from an unregulated operator. PIC discarded mushrooms. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed build-up of debris in ice maker. Raw shell eggs not maintained at 45 degrees or below (critical, corrected). Observed raw eggs sitting out in kitchen. PIC placed the eggs in ice and in the cooler.

Little Caesars #443, 1275 Mt. Vernon Ave., Marion, Aug. 9. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Did not observe an employee illness policy at the facility. PIC was provided copy of the Marion Public Health policy for employees to review and sign. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed open oil containers not covered in the kitchen. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed cleaned dishes to have build up of debris.

Hissho Sushi @ Meijer #111, 2153 Marion-Mt. Gilead Road, Marion, Aug. 10. Single-use gloves used improperly (critical, corrected during inspection). Observed food employee not changing gloves between calibrating the pH meter with calibration chemicals and taking portions of rice out of the rice container with gloved hands.

SunSwim Pool, 1 Sugar St., Prospect, Aug. 10. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Did not observed the correct employee illness policy on site at the time of inspection. Gave PIC copies of MPH employee health policy.

Waldo Swim & Recreational Center, 275 W. Main St., Waldo, Aug. 10. Public water system does not meet required drinking water regulations (critical, corrected during inspection). Most recent water sample did not pass. The sample tested positive for total coliform. The operator stated they would be using bottled water for handwashing and dishwashing until the end of the season.

Meijer Store #111, 2153 Marion-Mt. Gilead Road, Marion, Aug. 10. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected during inspection). Observed several dented cans. PIC discarded the cans. Food contact surfaces not easily cleanable (critical, corrected). Observed chips and deep score marks on the meat room cutting boards. PIC is ordering more cutting boards to replace the damaged ones. Food in an RFE must be embargoed due to baby food or formula is expired (critical, corrected). Observed 22 expired packages of various brands of baby food and formula. PIC discarded all listed items.

G & R Tavern, 103 S. Marion St., Waldo, Aug. 11. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed build-up of debris in ice maker and on can opener blade. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed facility-made blue cheese dressing past 7 days. PIC discarded the dressing.

Duchess Shoppe Waldo #1235, 262 N. Marion St., Waldo, Aug. 11. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed build-up of debris in strawberry slushi dispenser. Food employee placed the machine out of order for cleaning. TCS foods not being hot held at the proper temperature (critical, corrected). Observed some foods not hot held at 135 degrees in the warming unit. Food employee discarded affected food and adjusted the temperature on the warming unit. Food contact surfaces not easily cleanable (critical, corrected). Observed deep score marks on cutting boards. Cutting boards were discarded.

740 Restaurant Ventures LLC, 143 W. Center St., Marion, Aug. 14. PIC unable to demonstrate knowledge of temperatures and times for cooling and reheating (critical, corrected during inspection). PIC was re-trained on the requirements. TCS foods not being cold held at the proper temperature (critical, corrected). Observed some TCS foods above 41 degrees in the pasta prep table. PIC discarded all affected food, placed unaffected food in the walk-in cooler, and adjusted the temperature settings on the prep table. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed beef not date marked within 24 hours of preparation. Food contact surfaces not easily cleanable (critical, corrected). Observed cutting boards with deep score marks. PIC discarded all affected cutting boards.

Red Lobster #560, 1318 Mt. Vernon Ave., Marion, Aug. 14. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed clean plates to be dirty. Food contact surfaces not easily cleanable (critical, corrected). Observed deep score marks in cutting boards. PIC discarded cutting boards.

Whirlpool Corporation-Satellite, 1300 Marion-Agosta Road, Marion, Aug. 14. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed TCS foods above 41 degrees in the Satellite cooler. PIC discarded affected food items and is getting the cooler serviced.

Attaboys, 289 W. Center St., Marion, Aug. 15. Consumer is not properly advised of the risk of consuming raw or undercooked animal foods (critical, corrected during inspection). Did not observe a consumer advisory on the menus. PIC was provided a consumer advisory document to post until menus are updated. Presence of live insects, rodents and other pests (critical, corrected). Observed live flies. PIC was advised to contact their pest control company.
