Marion County restaurant inspections: December critical violations

The Louisiana Department of Health performs regular inspections of restaurants and stores that sell food. These are the local results of those inspections.

The following restaurant inspections with critical violations were conducted by Richland Public Health in December:

McDonalds #5511, 483 N. Main St., Marion, Dec. 6. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed ham in the cooler to be held past its disposition date. Person in charge (PIC) disposed of item. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed bacon in the cooler to be incorrectly date marked.

Lee's Famous Recipe Chicken, 571 E. Center St., Marion, Dec. 12. Food contact surfaces not easily cleanable (critical, corrected). Observed in-use cooking equipment to be stored on rags.

Little Caesars #443, 1275 Mt. Vernon Ave, Marion, Dec. 15. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected during inspection). Observed handwashing sink being used as a prep sink. Improper use of time as a public health control (critical, corrected). Observed pizza sauces near the pick-up window to not have the proper time stamps on them. Food contact surfaces not easily cleanable (critical repeat, corrected). Observed in-use food utensils to be set on single-use items when not in use. PIC removed items and placed them in the correct spots.

Dino's West, 810 Davids St., Marion, Dec. 15. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed several foods in the walk-in cooler to be held past their disposition date. PIC disposed of foods.

Perkins Family Restaurant, 1197 Mt. Vernon Ave, Marion, Dec. 16. Food package(s) received in poor condition (critical, corrected during inspection). Observed there to be a dented can in storage and not placed in a designated area for damaged goods. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed eggs in cooking line cooler to be stored above hamburger. TCS foods not being cold held at the proper temperature (critical, corrected). Observed pancake mix to be sitting at 61 degrees. Chef disposed of mix.

Baires Deli LLC, 131 S. Main St., Marion, Dec. 16. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Did not observe date marks on food items that had been held for longer than 24 hours. PIC recalled when foods were placed in coolers and placed the appropriate date marks on them.

Donato's Pizza Inc. - Marion #35, 1181 Mt. Vernon Ave., Marion, Dec. 22. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed several items in reach-in coolers and the walk-in cooler to be held past their disposition date. PIC disposed of foods.

Pizza Hut Restaurant - Delco, 665 Delaware Ave., Marion, Dec. 22. Handwashing sink not accessible (critical, corrected during inspection). Observed back handwashing sink to have cardboard in it during the time of inspection.

OK Café, 734 E. Center St., Marion, Dec. 23. Food package(s) received in poor condition (critical, corrected during inspection). Observed dented can on shelf. PIC separated can to be returned. Food in contact with an unclean surface or linens (critical, corrected). Observed lettuce scoop handle to be sitting in lettuce.

North Marion Fuel Inc., 847 N. Main St., Marion, Dec. 24. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Did not observe signed sick policies in the facility. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). PIC stated that hot holding units, utensils and coffee pots were not cleaned at required frequency. Spoke with PIC about needing to clean these items once a day since utensils are stored inside hot holding unit.

Carlyle's Restaurant, 1116 Mt. Vernon Ave., Marion, Dec. 24. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed storage in standup cooler next to grill to be done incorrectly.

Papa Johns #626, 254 E. Center St., Marion, Dec. 24. Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed food in make table cooler to be held past its disposition date. PIC disposed of food. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Did not observe labels on chemical bottles.

Speedway #9733, 1351 Delaware Ave., Marion, Dec. 24. Missing and/or incomplete allergen declaration(s) (critical). Did not observe complete allergen declarations for each food item in the bulk food container.

Shell, 587 Delaware Ave., Marion, Dec. 28. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Did not observe an employee sick policy on site. The employee working at the time was handed a sick policy and signed it on site after reading. Handwashing sink is being used for purposes other than handwashing (critical, corrected). Observed a hose hooked up to the handwashing sink.

Circle K #4705332, 588 N. Main St., Marion, Dec. 28. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed the ice shoots on the self-service machine to have build up. PIC had employee clean ice shoots on site. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed the breakfast pretzel stacker and the turkey sausage flatbread out of date. PIC discarded items.

Duchess Shoppe #1216, 1881 Marion-Mt. Gilead Road, Marion, Dec. 29. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed the pop nozzles and the ice shoot to have build up and debris.

Marion Motomart South, 1297 S. Prospect St., Marion, Dec. 30. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed the ice shoot on the self service pop machine to have build up.

Marion Manor Nursing Home, 195 Executive Drive, Marion, Dec. 30. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed eggs to be stored above RTE foods in the cooler.

Fairpark Duchess Shoppe #1221, 440 E. Fairground St., Marion, Dec. 30. Handwashing sink is being used for purposes other than handwashing (critical repeat, corrected during inspection). Observed the handwashing sink in the front area to have containers inside. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed milk being stored on the floor in the walk-in fridge. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed multiple items (cold meat, cheeses, meatballs, and sliced cheese) in the small fridge to not have a date. PIC had employee date all items during the inspection.

DeWolfe Place, 1140 Wilson Ave., Marion, Dec. 30. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed leftover spaghetti being held past its disposition date. PIC disposed of food during the time of inspection.
