Marion County restaurant inspections: North Third, Crazy Fruits, Axel’s Taco Shop

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

ABOUT RESTAURANT INSPECTIONS

This is a snapshot of the inspections conducted by the Marion County health department.

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

North Third

Location: 282 N Third St., Stayton

A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: Observed operator begin to rinse dirty containers in handwashing sink. Adjacent sinks were full of dirty dishes. Point deduction: 3.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Measured container of clam chowder in soup warmer at 77 degrees Fahrenheit. Measured temperature of tomato sauce and two containers of béchamel sauce between 67-100 degrees in steam table. Point deduction: 5.

Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: Observed growth on chili peppers dated Feb. 1 and olives (Feb. 18) past seven day disposition. Point deduction: 5.

A consumer advisory is not provided, is not presented in the correct format, or does not contain the required information, specifically: Various foods served raw or undercooked missing consumer advisory asterisk or disclosure on menu including: lemongrass ahi taco, jalapeno pepper jack burger, burger and a brew and happy burger. Point deduction: 3.

Axel's Taco Shop

Location: 340 Lancaster Drive NE, Salem

Date: March 1

Priority violations:

Raw or ready-to eat food is not properly protected from cross contamination, specifically: Container with raw cut chicken stored above sealed packages of raw beef inside the walk-in refrigerator. Raw ground beef stored above horchata, rice and salsas inside the walk-in refrigerator. Raw chorizo and Milanese stored above cooked meats inside the cookline refrigerator. Raw blended eggs stored above pot of cooked beans inside the cookline upright refrigerator. Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Container of cooked carnitas measured 51-54 degrees Fahrenheit inside the walk-in refrigerator. Condensation observed along the inner lid. Large pot of uncovered beans measured 48-49 degrees in the center. Carne asada stored above the hot water level of the steam table measured 111-124 degrees in different areas. Point deduction: 5.

Crazy Fruits

Location: 401 Center St. NE, #2252, Salem

Date: March 1

Priority violations:

Unauthorized personnel are on the premises or employees are not properly trained in food safety or food allergy awareness, specifically: Employee present at the beginning of the inspection was not aware of the requirement to wash, rinse and sanitize multi-use food contact surfaces, or trained how to do so. A food handler card for this employee was not available. Point deduction: 3.

Hand towels or a hand drying device is not provided at the handwashing sink, specifically: A sanitary method for drying hands was not available at the start of the inspection. Point deduction: 3.

Food-contact surfaces are not cleaned between different raw animal foods, between raw to ready-to-eat foods, or as frequently as necessary, specifically: Employees were not sanitizing multi-use food contact surfaces of utensils and equipment due to missing drain plugs. Discussion with an employee on-site revealed the potential last time that food contact surfaces were washed, rinsed and sanitized was prior to 10 a.m. (more than four hours prior to inspection). Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: All items in the steam wells were observed improperly hot-held. Specifically: cheese sauce (117 degrees Fahrenheit), middle compartment of corn off the cob (116 degrees) and front compartment of corn off the cob (117 degrees). Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: The following items are kept out of refrigeration for the lunch rush and put back into refrigeration afterward: cooked cabbage (48 degrees Fahrenheit), cooked broccoli (49 degrees), cooked noodles (51 degrees), cooked breaded chicken (49 degrees) and salad mix (45 degrees).
