Marion County restaurant inspections: Oct. 3-13 critical violations

The Louisiana Department of Health performs regular inspections of restaurants and stores that sell food. These are the local results of those inspections.

The following restaurant inspections with critical violations were conducted by Marion Public Health between Oct. 3 and Oct. 13:

Kroger Company N 808, 1428 Marion-Waldo Road, Marion, Oct. 3. Food package(s) received in poor condition (critical repeat, corrected during inspection). Observed dented cans of tomato juice on store shelves. Person in charge (PIC) removed the cans to discard. TCS foods not being cold held at the proper temperature (critical, corrected). Observed packages of cut lettuce in the produce case as well as beef patties in the meat room and unprepared chicken wings in the deli to be above 41 degrees. PIC had food employees discard all packages that were above 41 degrees. Food in an RFE must be embargoed due to adulteration, misbranding or baby food or formula is expired (critical, corrected). Observed eight expired containers of Gerber Harvest Bowl baby food, one expired packet of Gerber Organic Mango Sweet Potato baby and one expired packet of Gerber Organic Banana Mango baby food. The expired baby food was removed from the shelves to be discarded.

Hong Kong Buffet 88 Inc., 1561 Marion-Mt. Gilead Road, Marion, Oct. 4. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed two coolers with food not separated accordingly. Beef was stacked under chicken, eggs were stacked over onions, and raw shrimp was stacked over ready-to-eat food. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed a build-up of debris on the soda machine ice chute, inside the ice maker and on the ice scoop.

Family Dollar Store #24129, 150 Barks Road E., Marion, Oct. 10. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical repeat, corrected during inspection). Observed dented cans at the time of inspection. PIC was notified and cans were removed from the shelf.

Subway of Marion, 571 Delaware Ave., Marion, Oct. 11. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Did not observe an employee health policy. PIC was provided with a copy of Marion Public Health's employee illness policy for employees to review and sign. Equipment food-contact surfaces or utensils are unclean (critical). Observed build-up of debris in soda machine ice chute. Correct by Oct. 18. Improper backflow prevention device installed on a water supply system (critical). Observed a broken backflow prevention device on the mop sink faucet. Correct by Oct. 31.

Van Java Hibachi, 526 W. Center St., Marion, Oct. 11. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed build-up of debris in ice maker. TCS foods not being hot held at the proper temperature (critical, corrected). Observed cooked cabbage below 135 degrees. PIC discarded the cabbage. Raw shell eggs not maintained at 45 degrees or below (critical, corrected). Observed raw shell eggs sitting out at room temperature. PIC discarded the eggs. TCS foods not being cold held at the proper temperature (critical, corrected).Observed sauces in the sauce cooler to be above 41 degrees. PIC discarded all affected sauces and was advised not to use the cooler until it has been repaired. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Did not observe a date mark on the large container of yum yum sauce. No air gap or approved backflow prevention device on the plumbing system (critical). Did not observe a backflow prevention device on the mop sink faucet. Correct by Oct. 25.

Dunkin' #560, 1136 Mt. Vernon Ave., Marion, Oct. 12. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed two jugs of skim milk not date marked within 24 hours after opening. PIC discarded the milk. Unapproved chemical sanitizer used on food contact surfaces (critical, corrected). Observed sanitizer dispenser dispensing quat sanitizer below the approved concentration. PIC hand-mixed the sanitizer during the inspection and will have food employees continue doing so until the dispenser has been repaired.

Panda Express #3213, 1692 Marion Mt. Gilead Road, Marion, Oct. 12. Unapproved chemical sanitizer used on food contact surfaces (critical, corrected during inspection). Observed sanitizer buckets with quat sanitizer below the approved concentration.

Kentucky Fried Chicken, 1917 Marion-Mt. Gilead Road, Marion, Oct. 12. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed a tray of chicken in the hot-holding unit below 135 degrees. PIC discarded the chicken.

Bob Evans Restaurants LLC #130, 188 American Blvd., Marion, Oct. 12. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed a build-up of debris on container lids located in the storage area under the prep table. Unapproved chemical sanitizer used on food contact surfaces (critical, corrected). Observed the sanitizer dispenser dispensing quat sanitizer above the approved concentration. PIC was advised to dilute the sanitizer with water until the dispenser has been repaired.

Rally's #4054, 412 N. Main St., Marion, Oct. 12. PIC unable to demonstrate knowledge of temperatures and times for safe cooling of TCS food (critical, corrected during inspection). PIC was unable to explain proper cooling times and temperatures when asked. PIC was retrained on the requirements and provided with an informational handout. Improper bare-hand contact with RTE foods (critical, corrected). Observed a food employee handle onions with bare hands.

Big P's Pit Stop, 6551 Marion-Agosta Road, Marion, Oct. 13. Food package(s) received in poor condition (critical, corrected during inspection). Observed dented can of pizza sauce on store shelves. The can was discarded. Presence of live insects, rodents and other pests (critical, corrected). Observed live flies and gnats around the three-bay sink and prep table. Facility was advised to contact their pest control company for persisting issues.
