ABOUT RESTAURANT INSPECTIONS

This is a snapshot of the inspections conducted by the Marion County health department.

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Grilled mushrooms in the steam unit measured 109-131 degrees Fahrenheit (water underneath measured 154 degrees). Handwashing sinks are not conveniently located in the food preparation area or provided in or adjacent to toilet rooms, specifically: Designated hand washing sink removed from the kitchen when the four-compartment sink was swapped with a three compartment sink. Point deduction: 3.

A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: The hand washing sink in the prep area is "out of order" and not currently in operation. Point deduction: 3.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Many foods inside the walk-in refrigerator are warmer than 41 degrees Fahrenheit. (Chicken 48 degrees, teriyaki chicken 48 degrees, eggroll 48 degrees, chicken breast 47 degrees, chicken strips 49 degrees). Point deduction: 5.

Panezanellie Breadstick Shoppe

Location: 111 NE Starr Street, Sublimity

Date: Oct. 25

Score: 86

Priority violations:

A handwashing sink does not provide the minimum water temperature or is not equipped with a mixing valve or combination faucet, specifically: Hot water not provided at restroom handwashing sink. After running water for a few minutes, water only reaches 62 degrees Fahrenheit. Point deduction: 3.

Food-contact surfaces are not clean, specifically: Stored food probe thermometers soiled with food debris. Point deduction: 3.

Ready-to-eat food is not properly date-marked, specifically: Opened milk and half and half opened more than one day prior to inspection are not dated. Point deduction: 3.

Chemical sanitizers are not approved, specifically: Quat solution in wiping cloth buckets used to sanitize food contact surfaces measured over 400 parts per million quat. Point deduction: 5.
