News of public record: Guernsey County restaurant inspections

The following are restaurant inspections with critical violations, as conducted by the Guernsey County Health Department from Jan. 8 to 12.

Jan. 8

All For Kids Inc., 1405 E. Wheeling Ave., Cambridge. Spray bottles not labeled with the common name of contents.

Jan. 9

Cirle K #4705285, 307 Clark St., Cambridge. A pizza slicer sitting in the handwashing sink; corrected during inspection.

House of Hunan, 2327 E. Wheeling Ave., Cambridge. No written procedures (time in lieu of temperature) have been submitted to our office for review including the practice of leaving shell eggs out without temperature control. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations. PIC is unable to demonstrate knowledge of cleaning and sanitizing. Employees not informed in a verifiable manner of their responsibility to report information about their health. Employee was washing hands in wok line faucet. Employee cracked shell eggs bare-handed and immediately return to the wok line. Raw chicken stored above shrimp in prep unit; corrected during inspection. Refrigerated, ready-to-eat, time/temperature controlled foods not properly date-marked.

Byesville VFW, 10355 Jackson St., Byesville. No date marking on multiple refrigerated, ready-to-eat foods including cooked ground beef, fried chicken, cheesecake, and cream pie slices. The facility offers undercooked eggs and beef products upon customer request, however, there is no consumer advisory informing of the increased risk of consuming raw or undercooked animal foods. Non-handled plastic cups stored in direct contact with foods; corrected during inspection. Cut leafy greens stored in containers lined with paper towels.

Plus 1 Pizza, 220 Clark St., Cambridge. A five pound container of butter stored on shelf at room temperature.

Jan. 10

Cambridge Dairy Queen, 2163 Southgate Parkway, Cambridge. Utensils drying on the drain board of handwashing sink. TCS foods cheese, tomatoes and lettuce registered 60-68 F. TCS foods must be held at 41 F or below to prevent the growth of harmful organisms; corrected during inspection.

Faith Community Childcare, 192 S. Second St., Byesville. The facility does not have a verifiable employee health reporting agreement on file. Cleaning supplies and chemicals stored in a box above baby foods. Mop water disposed of on ground outside.

Quality Inn, 1945 Southgate Parkway, Cambridge. The facility did not have an employee knowledgeable in food safety and proper sanitizing procedures available. Employees are not informed in a verifiable manner of their responsibility to report information about their health; corrected during inspection.

Wal-Mart #3262, 61205 Southgate Parkway, Cambridge. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. Fully cooked turkey bacon below raw bacon in retail display case. The rinse temperature of the dishwasher in the bakery area registered at 74-80 F. Rinse temperature must reach at least 180 F to effectively kill harmful organisms. Deli meats date marked for eight days. Bleach and cleaner above baby formula and food; corrected during inspection. The warewashing machine does not automatically dispense detergent sanitizer and/or does not have a visual means to ensure detergent and sanitizer is automatically dispensed. The automatic dispenser for the dishwasher was not working. Dish machine needs repaired immediately to ensure dishes are effectively cleaned to prevent foodborne illness.

Tommy's Old Fashioned Pizza, 222 Main St., Byesville. The facility does not have a verifiable employee health reporting agreement on file; corrected during inspection. Insecticide stored in closet near a restroom. Other than those for incidental (emergency) use, any pesticide or restricted-use pesticide shall only be applied by a licensed commercial pesticide applicator.

Bud's Pub, 634 Cochran Ave., Cambridge. Mop water disposed of on the ground surface outside.

We're Rolling Pretzel Company, 61205 Southgate Parkway, Cambridge. PIC was unable to demonstrate proper cleaning and sanitizing procedures. PIC was unable to demonstrate knowledge of food temperatures and times for safe cooking. PIC was unable to state the required temperatures for pretzel dogs. Wiping cloth sitting in handwashing sink; corrected during inspection. Equipment and utensils are not being sanitized in a 3-compartment sink. All 4 Steramine containers in the kitchen were full and unused. A used container was in the front part of the facility and an employee indicated that those tablets are used for sanitizing buckets for wiping cloths; corrected during inspection. Pretzel dogs in the hot holding unit registered 123 F. TCS food must be held at 135 F or above to prevent the growth of harmful organisms - corrected during inspection.

Jan. 12

Circle K #4705286, 287 Main St., Byesville. Pretzel dogs in the hot holding unit registered 123. TCS food must be held at 135 F or above to prevent the growth of harmful organisms. Raw shell eggs stored above ready-to-eat items. Commercially packaged, pre-cooked, TCS foods that are received frozen, opened, and placed under refrigeration to thaw before being reheated and hot-held are not being properly date marked. The handheld pump sprayer containing an unknown chemical solution is not labeled with the common name of contents.

Galaxy Pizza, 284 W. Main St., Byesville. Employee observed preparing food after answering the telephone and operating point-of-sale equipment without washing hands first. A red sanitizer storage bucket and cleaning equipment were stored in the handwashing sink.
