News of public record: Guernsey County restaurant inspections

The following are restaurant inspections with critical violations, as conducted by the Guernsey County Health Department from Feb. 5 to 9.

Feb. 6

St. Benedict Church and School, 220 N. Seventh St., Cambridge. The dishwashing machine is achieving a utensil surface temperature of 152.6 F.

Feb. 7

Subway of Cambridge, 1710 Southgate Parkway, Cambridge. Food residue and debris in the handwashing sink. Pre-cooked meatballs and soups are being reheated from a cold state in hot-holding unit. Meatballs in a hot holding unit with an internal temperature of 127 F. Milk in cooler had a temperature of 48 F. Time/temperature controlled for safety (TCS) foods must be cold held at 41 F or below to prevent pathogen growth. A leaking container of liquid ant bait in the cabinet where single-use containers are being stored. The water supply hose for the mop sink was coiled and resting directly in the sink with no air gap or backflow prevention device provided.

Cambridge Health and Rehabilitation Center, 1471 Wills Creek Valley Drive, Cambridge. The person in charge (PIC) did not ensure employees were properly maintaining the temperatures of TCS foods during hot and cold storage. The PIC did not ensure their employees were properly trained in food safety. TCS food was being reheated on a steam table and food registered 97-144 F. According to an employee, the steamer unit that is used to reheat/cook food is out of order and there is no other way to heat food. Mashed potatoes registered 112 F. TCS food must be held at 135 F or above. Observed 3-compartment sink without proper backflow prevention being used to thaw food.

Feb. 8

Burger King, 61688 Southgate Road, Cambridge. The handwashing sink is stained with coffee residue.

Shogun Cambridge, Inc., 61600 Southgate Road, Cambridge. Raw shell eggs stored above ready-to-eat, cut vegetable. Food containers in cold-holding units without lids or otherwise covered to prevent contamination. Used single-use glove stored on container of scallops. Employees indicate they reuse said glove throughout the day when dispensing portions of scallops. The consumer is not properly advised of the risk of consuming raw or undercooked animal foods.

Chipotle Mexican Grill #3560, 61690 Southgate Road 4, Cambridge. Quaternary ammonium solutions in sanitizer buckets throughout had a concentration of 0 parts per million (ppm), should have a minimum concentration of 200 ppm and a minimum temperature of 75 F.

Theo Brothers Inc, The Forum, 2205 Southgate Parkway, Cambridge. The PIC did not ensure employees were effectively cleaning their hands as required. The PIC was not ensuring proper cooling. The PIC did not ensure employees were properly maintaining the temperatures of TCS foods during hot and cold storage. Employees throughout the facility were not washing their hands when required. Two sinks in the kitchen area were dirty and had pieces of food in the bottoms. Packaged bread is not labeled with ingredients. Handle for spatula sitting in food in prep cooler and bowl sitting in rice in walk-in cooler. Raw meat stored above cooked/ready-to-eat food. Raw turkey was being thawed in a pan above salad dressing, and raw beef above pepperoni and salami. Lasagna on the steam table registered 117-123 F. TCS foods must be held at 135 F or above to prevent the growth of harmful organisms. Bacon in the back prep unit registered 48 F. Cole slaw in the prep unit registered 46 F. Cheeses in the prep unit next to the grill registered 46 F. TCS food must be held at 41 F or below to prevent the growth of harmful organisms. Several TCS foods in the walk-in cooler were not dat marked including lettuce, meatballs, potato skins, cole slaw, brown rice, tomatoes, prime rib, kopanisti mixtva. The chemical squirt bottles throughout were not labeled with the common name. Wooden platters used for fajita pans and wooden cutting boards were cracked, warped and damaged.

Astoria Place, 8420 Georgetown Road, Cambridge. Employees were not washing their hands in between tasks. Baked potatoes observed uncovered in 2" of water in the bottom of the pan. Baked potatoes and chicken noodle soup were not date marked. Chemical squirt bottles throughout the facility were not labeled. The detergent canister was not connected. Once connected, it was discovered that the water to the detergent was not working and the detergent was not getting to the machine.

McDonalds, 2022 Southgate Parkway, Cambridge. The detergent container was empty in the dishwasher. The container was replaced, but detergent was still not going into the machine. After being evaluated, it was discovered that the detergent line was clogged with hardened detergent and the feeder control box was not working. Until the machine is fixed, all dishes must be washed, rinsed and sanitized in a 3-compartment sink.

Georgetown Vineyards, 62920 Georgetown Road, Cambridge. The bread in the freezer did not have a list of ingredients on the packaging.

Feb. 9

Meadowbrook Middle School, 58607 Marietta Road, Byesville. Microwave ovens, hot dog rollers, and food storage containers had accumulations of food residues, spills, splatters, and generally not clean to sight or touch.
