The Iowa Board of Mortuary Science has fined a funeral director $350 for failing to report multiple criminal convictions to the board.

State records indicate David Dolezal of Toledo was issued an Iowa funeral director’s license in 1989 and that he subsequently practiced mortuary science in Toledo.

In October 2023, he applied for renewal of his license and, according to the board, failed to disclose “multiple criminal convictions.” The board subsequently approved Dolezal’s  application for renewal, but in June charged him with failing to report all criminal convictions to the board within 30 days.

The parties recently agreed to a settlement that requires Dolezal to pay a $350 civil penalty.

The board has not publicly disclosed the nature of the criminal convictions. However, court records indicate that in 2022, Dolezal was convicted of domestic abuse assault. In 2023, he was charged with two counts of second-offense domestic abuse assault and later pleaded guilty to one count with the remaining count dismissed by the court.

Also in 2023, Dolezal was arrested and charged with felony burglary and felony stalking in violation of a protective order. He was convicted of both charges and received a deferred judgment.

Dolezal declined to comment on the matter when contacted by the Iowa Capital Dispatch, except to confirm the nature of his criminal convictions.
