Ohio woman killed after minivan struck twice on I-65 near Madison

An Ohio woman died after the vehicle she was a passenger in was struck twice on Interstate 65 near Madison.

Metro Nashville police said Phyllas Louise Fone, 76, of West Salem, Ohio, died shortly after arriving to a hospital Friday.

The wreck happened about 1:20 p.m. when the Dodge Journey minivan Fone was a passenger in, was sideswiped by a white or silver vehicle. The minivan's right front tire blew out disabling the vehicle on the interstate.

The light colored car that struck the minivan didn't stop, police said.

A Jeep Wrangler came upon the crash scene but was unable to avoid a collision, striking the minivan from behind, police said.

The driver of the minivan, a front passenger and the driver of the Jeep were all taken to Skyline Medical Center with non-life threatening injuries.
