OHIZ Community Feedback Survey responses requested

Mar. 4—NORWALK — In July 2022, the Ohio Department of Health and its Office of Health Opportunity awarded $6.89 million to 26 organizations, including Huron County Public Health, to fund projects designed to address community conditions that impact health and improve the lives of residents in Ohio Health Improvement Zones (OHIZ), and thus the OHIZ pilot project was established.

What is OHIZ?

The OHIZs are neighborhoods with socioeconomic and demographic factors that affect the resilience of individuals and communities. Organizations who were awarded funding will either establish or expand initiatives that will build local capacity to help remove barriers to health in some of Ohio's communities facing the greatest risk of poor health outcomes.

HCPH has been awarded $100,000 from the ODH to focus efforts on Census Tracts #9162 (see Figure 1) and #9163 (see Figure 2) in Huron County which encompass census designated Celeryville, New Haven and Richmond Townships (partial), the village of Plymouth (partial), and the city of Willard (partial).

HCPH Seeks Community Feedback to Effectively Allocate the OHIZ Award

To learn more about residents in the OHIZ and the barriers they face to achieving optimal health, HCPH has created a brief survey to gain knowledge directly from residents about:

—What the community needs;

—What issues matter most to the community;

—What assets exist in the community, as well as what gaps exist in the community.

The OHIZ Community Feedback Survey can be accessed by using the QR code and/or link below, and is also linked on HCPH's OHIZ webpage at www.huroncohealth.com/ohiz. All responses will remain confidential, and HCPH asks all participants to please answer candidly and honestly. Participants will have an option to provide their contact information to be entered into a drawing to win 1 of 3-$100 Amazon gift cards. A postcard containing this survey information is also being sent via direct mail to the OHIZs. Note that in order to be entered in the drawing, participants must provide their contact information on the survey. Please submit all surveys by Friday, March 10.
