Portage County inspections: One restaurant has 24 violations and four others have at least 9

The following are the latest results of Portage County Health Department inspections on licensed food establishments and other types of businesses, which the Stevens Point Journal reports monthly.

Amherst Junction

  • Junction Bar, 3986 N. Second St.: May 24: Three violations: Handwashing sinks not hands free; window trim around kitchen window bare wood; no self-closing door on back employee restroom.

  • Smokey Spur, 4180 Smokey Road: May 10: Six violations: Flat of raw shell eggs stored on beer keg in walk-in cooler; consumer advisory missing disclosure statement; two-door refrigerator in kitchen has broken gasket, broken door handle and missing panel on bottom of unit; canopy hood dirty; cooling unit fan in four-door reach-in refrigerator dusty; flooring in kitchen by fryer and dish machine pitted and damaged.

  • Spun N Fun, 5576 Morgan Road: April 28: No violations.

Custer

  • The Backyard Grille, 6805 State 66: May 18: Six violations: No procedures in place that address responding to vomiting or diarrheal events; freezers have excessive amounts of frost build up; bar pizza oven drip tray dirty; flooring in kitchen damaged and pitted; no handwashing signs at kitchen, kitchen bathroom, men's bathroom and women's bathroom handwashing sinks; mops stored in mop sink.

  • Becky's Downtown Polonia Cafe, 7419 County Z: May 3: No violations.

Junction City

  • Brian and Wendy's Hiawatha, 817 Main St: May 25: No violations.

Plover

  • Chili's Grill & Bar, 165 Crossroads: April 12: Six violations: Pico, diced tomatoes and corn bean salsa not held at proper temperature in three-door cooler; food dicers stored in food prep area dirty (corrected during inspection); no permanently attached thermometer in cooler near flat-top grill; salad cooler not at proper temperature; plastic bins used to stored food stacked without being air dried; coffee spoons stored upright near beverage station (corrected during inspection). Re-inspected May 1: One violation: Plastic bins used to store food stacked without being air dried.

  • Taco Bell No. 36274, 1550 Plover Road: May 22: Two violations: Clean metal pans stored stacked without being air dried; food prep sink leaking and not in good repair.

Stevens Point

  • Backwoods Bar and Grill, 1511 N. Second Drive: May 3: Six violations: Raw shell eggs stored above cheese and condiments (corrected during inspection); dish machine sanitizer not used according to label directions; potato french fry press soiled; freezers have excessive frost buildup; canopy filters visibly soiled; men's bathroom does not have hand soap.

  • China Wok, 135 N. Division St.: May 9: Nine violations: Frozen foods stored in plastic grocery bags not approved for food storage; wiping cloth sanitizer not used according to directions (corrected during inspection); bulk storage bins and bulk food storage bags of rice and flour unwrapped or uncovered (corrected during inspection); cardboard used on floor of walk-in freezer; surfaces of back splash and drain boards at ware-washing sink not smooth and easily cleanable; walk-in cooler shelves soiled; walk-in cooler walls soiled; package of take-out boxes stored on kitchen floor (corrected during inspection); a few floor tiles in kitchen cracked and no longer smooth and easily cleanable; light fixture in storage area missing light shield; light in storage area not working properly.

  • CPS Cafeteria, 1901 Fourth Ave.: May 2: One violation: Light fixture in dish machine room missing light shield.

  • Dilly Dahl Deli, 1025 N. Second St.: May 10: Four violations: Piercing tip of table mounted can opener dirty; frozen food items stored in upright freezer not frozen; no handwashing sign at employee handwashing sink; flooring below broaster, fryer and cooking equipment dirty.

  • Gifted Hands Kitchen, 324 Frontenac Ave.: May 17: No violations.

  • Hurd's, 3279 Orchid Lane: May 17: No violations.

  • Jimmy Johns, 108 N. Division St. Suite A: May 4: One violation: Mop stored in basin of mop sink.

  • Little White Inn, 441 Second St. N.: May 4: No. violations.

  • Moose Lodge, 1025 Second St.: May 10: Eleven violations: Sliced ham stored in walk-in cooler exceeded date mark (corrected during inspection); dish machine at bar not providing sanitizer in final sanitizer rinse; table mounted can opener piercing tip dirty; employee working in food preparation and cooking area without hair restraint (corrected during inspection); clothes used for wiping counters and equipment not stored in sanitizer solution; frozen foods in upright freezer not maintained frozen; baked potatoes in walk-in cooler improperly cooled tightly wrapped in foil and in covered container; walls, ceiling and refrigeration condenser grate of walk-in cooler dirty; popcorn machine dirty; no handwashing sign at employee handwashing sink in kitchen; flooring below broaster, fryer and cooking equipment dirty.

  • Muse At Sentry, 1800 North Point Drive: May 4: Four violations: Several foods held in upper prep line cooler not at proper temperature; reduced oxygen packing for raw meats not labeled with date; interior surfaces of ice machine dirty; wiping cloth sanitizer solution stored on floor (corrected during inspection).

  • Pizza Hut, 417 Division St.: May 8: Nine violations: Thermometer monitoring temperatures in foods does not work; thermometers missing from wing station freezer and cooler; upright cooler not at proper temperature (no food stored in cooler); salad cooler not in good repair and at proper temperature; top of dish machine dirty; wall coving below dish machine pulled away from wall; lights above wing station and mop sink not working; flooring under pizza oven and shelving unit in back storage areas dirty; ceiling air ducts dirty; filters in canopy hood system dirty; wet mop stored in mop ringer.

  • Sentry Division Street Cafe, 1501 North Point Drive: May 18: Nine violations: Containers of dried cranberries, croutons, fried onions, lemon wedges and pecans in self-service salad bar not protected under sneeze guard; garlic green beans and falafel balls not at proper temperature; cutting surface of vegetable dicer dirty; piercing tip of table-mounted can opener dirty; plastic food pan for dispensing panko bread crumbs from bulk storage container stored in food; open case of smart snacks stored on floor in walk-in beverage cooler; employee towel drying clean equipment after it was washed in dish machine; clean food pans stacked without being air dried; no handwashing sign at employee handwashing sink near main entrée line.

  • Silver Coach, 38 Park Ridge: May 17: Five violations: Dish machine sanitizer not used according to label instructions (corrected during inspection); consumer advisory missing from disclosure statement; no test kit for monitoring sanitizer concentrations; ice machine splash guard dirty; no food protection manager's certificate posted in establishment.

  • South Point, 3057 Church St.: April 12: One violation: No certified food manager for establishment.

  • Subway No. 103 CS, 3296 Church St.: May 9: No violations.

  • Subway No. 112, 1201 Badger Ave.: May 4: One violation: No handwashing sign at the two employee handwashing sinks near sandwich making tables.

  • Taco Bell No. 36270, 433 N. Division St.: May 8: Three violations: Open cases of frozen potato bites and french fries stored on floor in walk-in freezer; filters in canopy hood system dirty; flooring in kitchen under prep line, equipment and soda machine at drive-thru dirty.

  • Taco Bell No. 36271, 5400 State 10 E.: May 17: Four violations: No procedures to address responding to vomiting or diarrheal events; clean food pans stacked together without being air dried (corrected during inspection); kitchen floors dirty; food establishment permit not posted.

Wisconsin Rapids

  • Yia Xiong, 3099 State 73: May 17: No violations.
