Public Records: Fairfield County restaurant inspections

The following are restaurant inspections with critical violations, as conducted by the Fairfield County Health Department in December.

Dec. 1

DP Pick, LLC, 8529-33 Refugee Road, Pickerington. There was no employee health policy on file. A spray bottle of cleaner was stored hanging over the ice bin and beer reach in case - corrected during inspection.

Happy Wok 168 LLC, 7877 Refugee Road, Pickerington. Observed container of raw chicken stored overtop of raw shrimp in walk-in - corrected during inspection.

La Petite Academy, 12968 Stonecreek Drive, NW, Pickerington. Missing date mark on a container of green beans in the refrigerator.

Dec. 4

Tim Hortons #7507, 1621 E. Main St., Lancaster. No air gap in the mop sink between the water supply inlet and the flood level rim.

Dec. 6

Taco Bell #38886, 1643 E. Main St., Lancaster. Observed no employee health agreement. Observed residue build-up around the nozzles of the slushy machine.

Burger King #5666, 1409 E. Main St., Lancaster. Employees donning gloves without washing hands. Also observed an employee handling a cell phone without washing hands and changing gloves. Tomatoes were observed using time as a public health control that was not marked or otherwise identified to indicate the time that is four hours past the point in time when the food is removed from temperature control.

Dec. 7

Penn Station, 1403 Ety Road, Lancaster. Observed a pan of butter in the handwashing sink - corrected during inspection.

Rockside Winery and Vineyard, LLC, 2363 Lancaster Newark Road NE, Lancaster. Many personal items in food storage cabinets are mixed with food service items.

Dec. 8

Cristy's Pizza - Amanda, 145 1/2 E. Main St., Amanda. Food residue on the dicer.

Dec. 11

Charleys Philly Steaks, 732 N. Memorial Drive, Lancaster. Employees donning gloves without washing hands. Observed jug being stored in handwashing sink - corrected during inspection. Observed raw chicken stored above cooked chicken - corrected during inspection. Presence of live insects, rodents, and other pests. Flies throughout the food service operation.

The Duchess Shoppe East Lancaster #1254, 561 E. Main St., Lancaster. Observed crate stored in handwashing sink - corrected during inspection. No handwashing sign in the restroom - corrected during inspection. Residue on the nozzles of the creamer machine and the ice shoot of the soda machine. No air gap between the hose of the mop sink and the flood rim.

Dec. 13

Sweet Corner, 1100 N. Memorial Drive, Lancaster. No hot water in the restroom. There appears to be an in-line water heater that may not be working.

Lancaster Save A Lot, 629 N. Memorial Drive, Lancaster. Observed Bob Evans Macaroni and Cheese and skim milk with a discard date of Dec. 10.

Dec. 14

Lakebound Pizza, 12404 Lancaster St., Millersport. No employee health agreement on file for verification.

Dec. 15

Speedway #9226, 6005 Winchester Road, Carroll. Observed stacked boxes in front of the hand sink in the kitchen - corrected during inspection.

Subway, 2687 N. Memorial Drive, Lancaster. Observed the meat slicer that had been cleaned on this date still had some food residue on the plastic guard.

Dec. 18

Rax Restaurant #104, 800 E. Main St., Lancaster. Build-up on the ice machine.

Dec. 19

Wendy's #9599, 7851 Refugee Road, Pickerington. Observed dining room soda machine dispenser and ice chute to be dirty.

Kroger #N931, 1141 E. Main St., Lancaster. No employee health policy - corrected during inspection. A package of burritos and a package of baby formula were damaged - corrected during inspection.

Dec. 20

Madison Bar and Grill Carry Out, 226 Madison Ave., Lancaster. Shell eggs in a container that was dated Dec. 3. Person in charge (PIC) stated that they were farm eggs. No hot water in the men's restroom.

American Legion Post 677, 11410 Smith Road NW, Lithopolis. No employee health agreement on file at the facility. Observed ice machine scoop with black residue present. The scoop was stored inside the ice machine - corrected during inspection. Ice machine walls have black residue on the left and right sides towards the back of the machine. Observed brats and hot dogs in refrigerator with November date on them - corrected during inspection.

Dec. 27

Lancaster ENT #2921 McDonalds, 1401 N. Memorial Drive, Lancaster. A food employee moved the mop bucket with gloves on and then went towards the food preparation area to begin working on food - corrected during inspection. Back handwashing sink blocked by mop bucket - corrected during inspection.

Wal-Mart #3792, 2687 N. Memorial Drive, Lancaster. Observed residue on produce misters. Handwashing sinks in the deli and meat room do not reach a temperature of 100 F.

Kingy's Pizza, 7470 Hill Road, Canal Winchester. Soda gun nozzle at the end of the bar closest to the front entry with build-up and soda gun holders at the bar closest to the kitchen with build-up. Observed the ice machine bin with black and pink residues on both the lip and the bin walls.
