Aug. 28

Hometown Eatz-n-Sweetz, 34 Potters Ave., Roseville, Potato residue on fry press. Temperature controlled, ready to eat foods not properly date marked.

GP Mart 2, 4061 West Pike, Handwashing sink not accessible. Food not properly protected from contamination by separation, packaging, and segregation.

Aug. 30

Maysville BP, 1311 Maysville Ave., Handwashing sink used to store personal items. Raw steak for personal use stored above ready to eat foods. Mold on ice machine and ice dispenser of soda machine.

Kroger 3387 Maple Ave., Raw fish stored above ready to eat foods. Items stored in several handwashing sinks.

Aug. 31

Maxwell's Drive-Thru, 2759 Maysville Pike, Mold on ice machine.

EagleSticks Inn, 2655 Maysville Pike, Raw bacon stored above ready to eat foods. Mold on soda gun.

Sept. 5

Norwich BP, 8490 East Pike, Norwich, Mop bucket stored in handwashing sink. Sandwiches and milk stored in deli case had temperatures of 68 degrees; item discarded.

Mizu Sushi and Grill, Maple Ave., Raw chicken storied above raw fish.

Chapman's Coffee House, 68 1/2 W. Main St., New Concord, Raw eggs stored above ready to eat foods.

Monterrey Mexican Restaurant, 3523 Maple Ave., Paper towels stored in handwashing sink. Mold on slushy machine. Rice stored in contact with rusty bucket being used as a scoop; rice discarded. Degreaser and soap dish stored above clean dishes on drying rack.

Oaks at Northpointe, 3291 Northpoint Drive, Mold on ice dispenser.

Sept. 6

Dairy Queen, 288 Main St., Duncan Falls, Manager unable to demonstrate knowledge of proper pH in a sanitizer bucket.

Sept. 7

Amaan, 2550 East Pike, Raw eggs stored above milk.

Best Western, 4929 East Pike, Towels and spray bottles stored in handwashing sink.

Sept. 8

Texas Roadhouse, 635 Zane St., Items stored in handwashing sink. Raw eggs stored above ready to eat foods.

Tapas Caribbean Kitchen, 57 S. Sixth St., Food not properly protected from contamination by separation, packaging, and segregation. Refrigerated, ready to eat foods not properly date marked.

Lazy River Lounge, 2290 Old River Road, Refrigerated, ready to eat foods not discarded when required.

Jud's Pub-Lucky Bar, 225 E. Main St., Mold on soda guns.

Sept. 11

Pizza Hut, 3355 N. Maple Ave., Buildup of hard, black material on ice machine.

Sept. 12

Putnam Tavern, 721 Putnam Ave., Raw meats stored above ready to eat foods. Upright beverage cooler at 48 degrees; food discarded. Toxic chemicals stored above beverages. Large number of fruit and house flies observed.

Sept. 13

VFW, 1318 Putnam Ave., Mold on ice machine. Lighter fluid stored above food containers.

Subway, 93 W. Dave Longaberger Ave., Dresden, Mold on ice machine.

Sept. 14

Little Tijuana, 1347 Ridge Road, Mold on ice machine and soda gun.

Zanesville Country Club, 1300 Country Club Drive, Raw chicken stored above ready to eat foods. Temperature controlled ready to eat foods not discarded after 7 days.

Sept. 22

Quad Student Center, 163 Stormont St., New Concord, Raw eggs stored above ready to eat foods.

Burger King 150 Maysville Pike, Mold on slushy machine.

Sept. 25

Zema Bros. Petroleum, 2497 East Pike, Raw eggs stored above ready to eat foods. Mold on slushy machine. Meatballs and marinara sauce not discarded after 7 days. Chemicals in spray bottles not marked with common names. Window cleaner stored on top of food. Muffins had no label or allergen information.

Giacomo's Bread and More, 2236 Maple Ave., Bean and corn salsa not discarded after 7 days. Significant amount of gnats.

Sept. 26

Taco Bell, 2145 June Parkway, Mold on freeze machine.

Sept. 28

Bryan Place, 49 N. Sixth St., Mold on ice machine. Beer cooler at 50 degrees; food discarded. Several live roaches and numerous droppings on dish tank.
