The following are restaurant inspections with critical violations, as conducted by the Zanesville-Muskingum County Health Department between June 26 and July 28.

June 26

Fondales Tavern, 203 Water St., Duncan Falls, Raw bacon and sausage stored above ready to eat foods. Sliced turkey held past 7 days.

Dairy Queen, 288 Main St., Duncan Falls, Raw ground beef stored above ready to eat foods. Mold buildup on ice bin.

June 27

Papa John's Pizza, 1434 Maple Ave., Cooler had temperature of 55 degrees.

Donatos, 3390 N. Maple Ave., Mold buildup on ice shoot of soda machine.

June 28

Rake's Steaks, 2207 Linden Ave., Bar cooler was blocking hand sink.

The Barn, 1947 Linden Ave., Raw beef stored above macaroni noodles in cooler.

June 30

Horse Shoe Bar & Grill, 213 Main St., Duncan Falls, Raw eggs stored above ready to eat foods. Chemicals stored above food.

July 7

The Olde Falls Inn, 3452 Newark Road, Mop blocking access to handwashing sink. Mold buildup on ice machine and soda gun. Multiple dented cans in storage room.

July 10

Tumbleweed Bar & Grill, 746 Monroe St., Towels stored in bar handwashing sink. Ants, gnats and flies in the kitchen.

July 13

WalMart, 2850 Maple Ave., Significate amount of dented cans.

July 21

Just 1 More Bar & Grill, 1317 Linden Ave., Person in charge unable to demonstrate knowledge; multiple critical violations observed. Cutting boards stored on top of handwashing sink. Raw chicken wings stored on top of drinks, raw beef stored above French fries, and raw egg stored above salad dressings and wing sauces. Significant amount of flies and gnats in the facility.

July 26

Fazoli's, 2580 Maple Ave., Ice built up on food from leaking unit in freezer.

Cracker Barrel, 225 Scenic Crest, Ice scoop in well without continuous water flow. Timer for water to be changed not on.

July 28

Russo's Wood Fired, 2526 Maple Ave., Raw sausage stuffed mushrooms stored above mac and cheese.
