Apr. 28—Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Floor drain area under 3 bay sink has a dirt buildup around floor in corner area that must be cleaned up.

Lights are not shielded or shatter proof over the food in the deli cases in front store area. Exterior door located in the walk in cooler area of the food facility has a gap at the bottom of door and does not protect against the entry of insects, rodents, and other animals.

Carmelo's Pizza Restaurant, 116 E. Philadelphia Ave., Boyertown, April 19: eight violations. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in pizza bain marie equipment. Loose rubber door gaskets observed on the pizza bain marie cooling unit. Food facility is using tape to repair the plumbing system under the warewashing sink, which is not an approved material. A utility sink or curbed cleaning facility with a floor drain is not provided in the food facility. Flooring, in the majority of kitchen area, is made of tiles that are cracked/broken and is not durable, smooth, non-porous, non-absorbent. Also coving loose. Contract is in place to repair floors. Walls in kitchen area are plaster and is peeling cracked / roughened / and is not a smooth, easily cleanable surface. Facility scheduled for repairs. Dices Creative Cakes, 30 N. Reading Ave., Boyertown, April 19: no violations.

Fraternal Order Of Orioles, 100 N. Washington St., Boyertown, April 19: five violations. Ice scoops are stored directly on top of the ice machine that is not a clean surface. Also ice buckets are stored in a manner that prevents water from draining. Observed Onions and tomatoes stored directly on the floor in walk in cooler area, rather than 6 inches off of the floor as required. Ice machine is located in back room under sewer line. Sewer line must be shielded or machine relocated. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the walk-in cooler, is not being date marked. Sliced packs of lunch meats are not being dated. The soda gun nozzle at the bar, a food contact surface, was observed to have food residue and was not clean to sight and touch. This is a repeat violation.

Mario's Pizza, 159 S Reading Ave., Boyertown, April 19: two violations. Torn rubber door gaskets observed on the True 3 door cooling unit. scheduled for repair. Tile missing at doorway next to dough mixer.

Plaza Azteca Mexican Grill, 1013 Reading Ave., Boyertown, April 19: six violations. Vacuum packed fish observed thawing in vacuum package which is not an approved thawing method. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in basement walk in cooler equipment. Floor in walk in cooler near dish machine has old spilled food and debris under shelves. Observed clean food equipment and/or utensils in soda fountain/chip dispensing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Observed clean dish racks stored on the floor in dish washer area. Observed clean food equipment and/or utensils in kitchen area, stored uncovered or not inverted. Pan observed in the handwash sink in the bar, indicating uses other than handwashing. Sink being used as a work sink. Flooring under warewashing machine is sloped and does not allow for water to drain properly causing a ponding of dirty water under the warewashing machine and drain board area.

The Third Rail, 404 Main St., Blandon, April 18: three violations. Mechanical warewashing equipment had a grease/dirt residue buildup observed inside top sprayer arm and corner areas. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Ledge at wall around fryer/grill area has a heavy soil/grease accumulation. Observed clean food equipment and/or utensils in clean dish area, stored uncovered or not inverted. Observed clean food bowls stored wet in a manner that does not allow for draining and/or air drying (wet nesting).

Wilson School District Senior High School, 2601 Grandview Blvd., West Lawn, April 18: three violations. Mechanical warewashing equipment had a pink slime residue buildup observed on the upper area above the rack intake opening. Also blower to dry clean dishes has dust/debris build up and must be cleaned. Food facility person in charge does not have records to demonstrate routine inspection and service of backflow prevention devices and other water treatment devices. Filters for steam oven equipment were not dated with date of service. Corrected. Pipe at wall where clean dishes exit machine is missing paint and has a rust build up that must be repaired.

Wilson School District Lower House, 2601 Grandview Blvd., West Lawn, April 18: no violations.

Gulf Station Oley, 1141 Memorial Highway, Oley, April 17: six violations. The person in charge does not have adequate control of food safety in this food facility as evidenced by this repeat non-compliant inspection. Again, observed multiple containers (1 half gallon, 3 quarts, 1 pint) of milk offered for sale beyond the sell-by dates, which ranged from April 14-16. All containers were removed from the sales shelves. Consumer self-service display of hot dogs still does not have sneeze guard or other effective protection. Ice bagged in facility does not have the facility information on bag. Labels are still not available. No chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Facility still does not have any sanitizer product available. A Food Employee Certification is displayed, but the individual is no longer employed by the retail food facility.

Iezzi's On 3rd, 61 S. Reading Ave., Boyertown, April 17: seven violations. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Corrected. Soda gun holder is clogged and has standing water in holder where soda gun nozzle rests. Vacuum packaged tuna, salmon thawing in packaging clearly labeled to remove from packaging to thaw was in cook line refrigerator in packaging which is not an approved thawing method. Tuna just removed from packs and Salmon discarded. Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Sanitizer bucket was empty. Everything to be sanitized in 3 bay warewashing sink. Observed clean food equipment and/or utensils in dish room area, stored uncovered or not inverted. Also Ice scoop observed laying directly on top of ice machine. Observed clean food equipment and/or utensils in kitchen area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Also Ice buckets are not inverted and stacked wet. Toilet rooms do not have a self-closing door. Womens room door closer is broken and must be repaired.

Wire wisk on rack has tape around handle and also duct tape on bain marie lid on cook line in kitchen, which is not an approved easy to clean material for repair. Raw shell eggs stored above ready to eat foods in reach in refrigerator. Corrected. Desserts (puddings, etc) was held at 50°F, in the counter area, rather than 41°F or below as required. Items placed in cooler until repaired. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Certificate just renewed and to be replaced. Water, as observed at hand sink in bakery area, is not under pressure during inspection due to a leak. Walls in the warewashing machine area have plastic hanging up and is not an easily cleanable material. Wall is scheduled to be repaired.

Conrad Weiser School District East Elementary, 200 Lincoln Drive, Wernersville, April 13: one violation. Mechanical warewashing machine has a pink residue build up at the entrance to machine where dish racks are placed in machine.

Lyons Valero, 1 W. State Ave Po Box 126, Lyons Station, April 13: five violations. Food facility person in charge not able to provide documentation that prepackaged hoagies are from an approved source. Disposed of 11 hoagies from Tacks Deli. Corrected. Boxes of eggs were stored above milk products in walk-in cooler. Observed build-up of dust and dirt on ceilings and fans in walk-in cooler. Door located in the back/exit area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. The light intensity in the back storage area is not at least 10 foot candles. Both lights are out.

Hamburg Area High School, 701 Windsor St., Hamburg, April 12: two violations. Raw shell eggs stored above foods on shelf in walk in cooler. Corrected. Customer self-service single-use tableware is not displayed in a manner so that only the handles are touched by customers. Corrected.

Hamburg Area Middle School, 701 Windsor St., Hamburg, April 12: two violations. The light intensity in the walk-in cooler / dry storage area is not at least 10 foot candles. Half the lights are not working in the walkin cooler. Food facility is using chlorine Sanitizer at an extremely high concentration of 200ppm, not approved in the Code of Federal Regulations for food contact sanitizing at this level. Corrected.

Love's Travel Stop #358&Subway#538, 3700 Mountain Road, Hamburg, April 12: four violations. Various food observed thawing at room temperature on the cart in Subway which is not an approved thawing method. Interior of ice machine in Subway has mold/slime build up around top of bin and ice drop chute area. Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying. Pans stacked on clean dish rack while wet. Love's walk-in cooler has dirt/debris build up on racks at bottom wheels of corner rack and floor area in corner.

Bojangle's, 3215 N. Fifth St., April 22: two violations. Green plastic shelving in walk-in refrigerator observed to be soiled. Bain marie interior/gaskets observed to be soiled. Utensils observed stored with handles in different directions and uncovered. Utensils corrected on inspection. Lids or covering to be corrected by 4/29/23.

JT's Gourmet Delivery LLC, 3801 Hilltop Ave., April 22: no violations.

Schell's Drive In, 4625 N. Fifth St., April 22: one violation. Observed mop head in mop bucket. Mops are not being hung to air dry.

Wal-Mart Store # 2614, 5370 Allentown Pike, April 22: one violation. Observed chunks of ice and other particles in the handwashing sink not related to handwashing. Handwashing sinks may only be used for handwashing. Corrected on inspection.

Dutch Maid Bakery, 4925 Kutztown Road, April 20: two violations. Ceiling exhaust openings above oven have some chipped paint which is not a smooth cleanable surface. Ceiling exhaust openings above oven have some dust build-up, and are in need of cleaning.

Jambalaya J's Dixie Kitchen & The Soda Shoppe, 2910 N. Fifth St., April 15: two violations. Jambalaya (1 of 2 pans) was held at 119.8°F, in the steam table area, rather than 135°F or above as required. Corrected on inspection. A working container of cleaner was stored on the same shelf with food, equipment, and/or single service articles in the food prep area. Corrected on inspection.

McDonald's of Muhlenberg, 430 Elizabeth Ave., April 15: no violations.

McDonald's of Temple, 5372 Allentown Pike, April 15: no violations.

The Hot Spot, 2934 N. Fifth Street Highway, April 15: two violations. Shepherd's Pie was held at 45.9°F, in the display case, rather than 41°F or below as required. Corrected on inspection. Two rubber spatulas observed with cracks and rough edge; unable to be cleaned properly. Discarded on inspection.

Reiffton Fire Company, 46 W. 33rd St., Reading, April 7: four violations. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration for the glass washer (chlorine test strips). There is a significant amount of grease on the floor under the Autofry unit. Side exit door located in the bar area of the food facility has a gap at the bottom and does not protect against the entry of pests. Also, the frame for the outer door in the side room is deteriorated near the bottom leaving a gap that could allow the entrance of pests. Soap was not available at the handwash sink in the bar area. Corrected.

GNA Ristorante Pizzeria, 421 Penn Ave., West Reading, April 6: two violations. Some lights are not shielded or shatter proof in the back area. Wall is damaged under the right side of the dishwasher. All wall surfaces in this area are scheduled to be repaired/replaced.

Rita's Water Ice, 850 Golden Drive Suite 12, Blandon, April 6: two violations. Floors throughout the facility are dirty and extremely sticky. Handwash sinks in front and rear are dirty and need cleaning. Paper towel dispenser empty at the handwash sink in the counter area.

Smokey Bones Bar & Fire Grill #7552, 2723 N. Meridian Blvd., Wyomissing, April 6: four violations. There is a gap along the top of the bottom bain marie drawer used to store lettuce. The metal piece between the upper and lower drawers is damaged on both sides preventing the drawer gasket from sealing properly. Facility is working to get the damaged part of this piece of equipment repaired. Floor tiles are damaged in the bar area, at the cooking line, and in the walk-in cooler. Floor is scheduled to be repaired 4/10/23. Multiple light bulbs are not shielded or shatter proof in the kitchen area. Shields are ordered and will be installed as soon as they arrive.Wall above the ice machine has a hole and is in need of repair. Facility is working to get this hole repaired.

Papa John's Pizza #2279, 2850 Saint Lawrence Ave., Reading, April 3: three violations. Plumbing system not maintained in good repair — observed water leaking at the drain for the handwash sink in the back area. Paper towel dispenser empty at the handwash sink in the back area. Corrected. Mops are not being hung to air dry.

IHOP #5502, 2701 Meridian Blvd., Wyomissing, March 30: no violations.

Laxmi's Indian Grille, 1806 State Hill Road, Wyomissing, March 30: five violations. There is an accumulation of dust/dirt on the cables above the food preparation table and on the wire shelving above clean equipment storage in the kitchen. The floor / wall juncture in the kitchen area is not coved and closed to 1/32 inch in several places. Door to the outside, located in the kitchen area of the food facility, is being propped open and there is no screen door. Food facility inspection indicates evidence of insect activity in kitchen and dining areas. Facility may need to increase the frequency of their pest control program. Working container (spray bottle) in kitchen area, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Mission Bbq Wyomissing, 1775 Papermill Road, Wyomissing, March 30: one violation. Observed a build-up of dirt, old food debris on the floor under some of the shelving in the dry storage area.

Wal-Mart Store #1670, 1135 Berkshire Blvd., Wyomissing, March 30: one violation. Severely dented, distressed canned items observed in the dry goods sales area and intended sale in the food facility (5 cans of soup, 1 large can of tuna). Corrected.

Wawa Food Market #8010, 2005 Museum Road, Wyomissing, March 30: one violation. There is an accumulation of dust/dirt on the wall-mounted fan in the back food preparation area.

Kutztown Fire Company #1 Social Quarters, 310 Noble St., Kutztown, March 29: no violations.
