Restaurant Health Ratings: September's Inspections

BIRMINGHAM AL - The Jefferson County Department of Health inspected more than 150 eateries in the county during the month of September, and most of those passed their inspections with flying colors, so to speak.

However, a handful of restaurants failed to make the grade. Here are the Jefferson County eateries that received score of 85 or lower in their September health inspection:

Zaxby's; 4815 Bell Hill Drive, Bessemer (73): Points were deducted for failure to register all Certified Food Safety Managers with JCDH and maintain the appropriate card in a readily accessible location; failure to maintain food at a proper temperature; flies laying on clean equipment faced up at salad station and throughout kitchen; slime buildup on fountain drink machine attachment under and behind ice dispenser in lobby; failure to maintain equipment in good repair; soap dispenser does not work at hand sinks near raw chicken station and near makeline; leaking pipe under 3 compartment sink; cluster of roaches under 3 compartment sink, around drain pipes and on walls; several flies in kitchen area; heavy grease buildup on floor and walls near fryers.

Ensley Foods; 1422 20th Street North, Ensley (77): Points were deducted for failure to provide food safety training for all food handlers as required; failure to wear hair restraints; hot dogs and smoked sausage in steamer/warming cabinet below proper temperature; boxes of chips on the floor; failure to properly store utensils; accumulation of dust and grime on fan vent covers of RI and WI coolers and on RI chest freezers and RI ice cream display freezers; failure to keep dumpster lid closed; accumulation of rodent droppings in dry storage area and in restroom; failure to maintain premises free of litter; accumulation of grime and residue behind, near, and underneath equipment, underneath shelving in dry storage areas.

Steak & Shake; 1817 Montgomery Highway, Hoover (81): Points deducted for failure to provide an employee health policy that states what actions are necessary when an employee has a specific symptom or diagnosis related to diseases that are transmissible through food; failure to post current inspection report in a conspicuous place inside the food establishment; failure to maintain cold food at proper temperature; the door on the freezer in the shake station is hanging off track; roaches and flies in facility; accumulation of food, grease, and debris under the heavy equipment and on the walls throughout the facility; several blown bulbs throughout the facility.

Blue Ridge Health Care; 1028 Bessemer Road, Birmingham (83): Points were deducted for failure to designate a person to be in charge and present for each shift during all hours of operation; two gallons of spoiled milk from 09/02/18 stored inside WI cooler; failure to maintain food at proper temperature; grease residue on bottom dry storage shelf; Food spills on floor under shelves inside WI cooler; extremely dirty floor drains.

Circle K; 301 Robert Jemison Road, Birmingham (84): Points deducted for failure to keep food at a proper temperature; failure to provide appropriate, readily accessible thermometer for verifying internal food temperatures; failure to maintain dishwashing equipment in a state of good repair; bags of garbage are stored on top of the dumpster; several floor tiles are missing inside the employee restroom; accumulation of lint on the exhaust vent.

Mudtown Eat & Drink; 3144 Green Valley Road, Cahaba Heights (85): Points deducted for failure to provide readily available documentation of food safety training for all employees at all hours of operation; failure to maintain food at a proper temperature; failure to properly mark and date all food prepared in-house; ceiling tiles and ceiling vent covers in kitchen soiled with dust.
