Observed broken floor tiles and ceiling tiles in dishwasher area. All physical facilities shall be maintained in good repair. C/A: Advised PIC to fix floor and ceiling tiles.
Observed a dead pest inside container of sour cream in prep top cooler. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by routinely inspecting the premises for evidence of pests. COS: Sour cream was discarded and container was placed in dishwasher.
