Person in charge failed to ensure that employees follow necessary procedures. Facility had evidence of insect infestation. Pesticide intended for home use in the brewery area. Significant large quantities of dead cockroaches and other insects. Observed large quantity of insect egg sacs. Observed unknown larger sized animal such as cat or dog scat inside the brewery area. Gaps around outer doors at brewery area. Van parked in area between brewery and kitchen. Significant debris build up on the outsides of equipment throughout the brewery. Ceiling coatings/paint flaking off. Dead cockroaches throughout the facility.

Facility had evidence of ongoing rodent infestation. Person in charge failed to ensure employees follow necessary procedures. Hose with spray nozzel on the ground which is used to spray food contact surfaces. Garage style roll up doors partially ajar. Numerous substantial sized holes along exterior of facility. Rodent droppings throughout the facility. Numerous dead insects and fly swatter by window. Exposed rafters above food and food equipment. Observed numerous holes and penetrations in the ceiling and walls. Debris build up on floors around the processing area. Light bulbs throughout the facility had no protective shielding.
