Restaurant inspections: La Fiesta Grande, Pizza Hut, V and M Discount, more

The Food Establishment Inspection Report is a weekly report that appears Sundays. Information is taken from reports provided by the Environmental Health Department, and individual reports can be viewed at its website, http://amarillo.gov/departments/community-services/environmental-health/foodinspections. A numerical grading system is now being used, with a score of 100 points being equivalent to zero demerits.

  • 100 points – No major violation at the time of inspection

  • 85 points – Considered an average score

  • 70 points or below – Requires re-inspection within 24 hours

  • COS – Corrected on site during inspection

These points correspond to the following letter grades:

  • A – 90 to 100

  • B – 80 to 89

  • C – 70 to 79

  • F – 69 and below

These establishments received the following scores for being out of compliance as stated:

(B/88) Amarillo Gas Station Inc., 1901 E. I-40. Frozen foods thawed in cooler. COS. No hot water in women’s restroom; soda dispenser nozzles dirty. Correct by 12/08. Grease interceptor needed (repeat violation); no Certified Food Manager; food handler certification needed and certifications maintained on premises. Correct by 12/28. Back door needs to be sealed (repeat violation); hole in wall around sinks with exposed plumbing must be sealed (repeat violation); signs needed in restrooms instructing employees to wash hands before returning to work. Correct by 02/26/2024.

(A/99) Avondale Elementary, 1500 S. Avondale St. Heavy dust accumulation on return air vents. COS.

(A/99) Belmar Elementary, 6342 Adirondack Trail. Heavy accumulation of dust and dirt on return vents. COS.

(A/99) Chick-Fil-A, 5544 W. Amarillo Blvd. Debris on floor of walk-in freezer and door handles; raw chicken juice on bottom of chicken reach-in cooler (repeat violation). Correct by 03/28/2024.

(A/97) Crockett Middle School, 4720 Floyd Ave. Raw eggs stored above ready-to-eat vegetables. COS.

(A/94) Eighty Twenty, 2511 S. Georgia St. All personal items and medication must be stored in a designated area. COS. Debris and dust on shelves in walk-in cooler; grease accumulation on vent hood filters and fire suppression lines; clutter behind counter near kiosk needs to be removed; Certified Food Manager certificate and establishment permit must be posted in view of customers. Correct by 03/24/2024.

(A/94) Greenways Intermediate School, 8100 Pineridge Drive. Sanitizer in three-compartment sink too weak; all employee medication must be stored in a designated area. COS.

(A/95) Holiday Inn Express AMA West, 8330 W. Amarillo Blvd. No soap at hand sink; mold inside juice dispenser. Correct by 12/07. Dust on walls and vents. Correct by 05/28/2024.

(A/98) Humphreys Highland Elementary, 3901 S.E. 15th Ave. Cut tomatoes and onions in walk-in cooler without date marks. COS.

(A/97) Johnny N Allen 6th Grade Campus, 700 N. Lincoln St. Dust and debris build-up on ceiling tiles (repeat violation); ceiling tiles in kitchen must be non-absorbent (repeat violation). Correct by 01/08/2024. Heavy discoloration and scoring on cutting board. Correct by 02/25/2024.

(B/86) La Fiesta Grande, 7415 S.W. 45th Ave. Chile relleno container sitting on top of ice at improper temperature; TCS (time/temperature control for safety) foods in cold top unit at improper temperature; items in cooler without date marks or marked improperly; raw chicken stored above raw shrimp in pullout drawer under grill. COS. Rust inside ice machine; build-up on filters on outside of ice machine; cold top unit holding at improper temperature and pooling water in bottom of unit. Correct by 12/04. Wall rack holding clean bowls and tops of bulk storage containers need to be cleaned to sight and touch (repeat violation); light bulb out in vent hood; wall in employee restroom needs sealing to be smooth, easily cleanable and non-absorbent (repeat violation). Correct by 02/20/2024.

(A/90) Lins Grand Buffet, 8440 W. I-40. Raw chicken stored above multiple ready-to-eat items in multiple locations; hand washing station used as dump sink. COS. Dead roaches in empty cabinets; employees in kitchen without hair restraints; employee food stored in multiple areas; scoops stored incorrectly; multiple areas, including drains, behind grills and others with large amounts of food debris, grease and dirt/dust; signs needed at hand sinks stating for hand washing only. Correct by 02/26/2024.

(A/93) Little Nest, 1400 S.W. 10th Ave. Multiple food items past “best by” and “use by “ dates; multiple dented cans on shelves. COS. Debris build-up on multiple surfaces. Correct by 02/26/2024.

(A/99) Oakdale Elementary, 2711 S. Hill St. Frozen condensation dripping in walk-in freezer (repeat violation). Correct by 02/26/2024.

(A/99) Olsen Park Elementary, 2409 Anna St. Heavy dirt/dust build-up on return vents. COS.

(A/95) Palios Pizza Café, 3562 S. Soncy Road, Suite 301. No paper towels at numerous hand sinks. COS. No quaternary test strips. Correct by 12/07. Bowl stored in flour bin. Correct by 02/25/2024.

(A/99) Pinkies #95, 2751 Civic Circle. Dust accumulating on shelves throughout establishment. Correct by 11/27/2024.

(B/89) Pizza Hut, 5611 Gem Lake Road, Suite 100. Employees did not wash hands between cashing out customers and making pizzas; broken hand dispensers at all hand sinks; heavy build-up on inside dishwashing machine, under water lines and on top of machine. Correct by 12/04. Two scoops with handles touching food items; heavy oil build-up dripping on shelves near fryer; dust build-up on/near air vents throughout establishment; visible build-up on vent hood above fryers; wall under dishwashing machine heavily soiled with grime and oil; grease build-up on faucet for three-compartment sink (repeat violation). Correct by 05/28/2024.

(A/98) Salvation Army, 400 S. Harrison St. Mold in walk-in cooler by door; dust on fan in reach-in cooler and wall behind three-compartment sink; old food debris in bottom of same reach-in cooler (repeat violation). Correct by 05/28/2024.

(A/97) Tacos Guadalajara, 2003 Living Water Drive. Items without date marks. COS. Fresh water and waste water on exterior of mobile must be labeled (repeat violation). Correct by 02/26/2024.

(A/93) Tascosa High School, 3921 Westlawn Ave. Quaternary sanitizer concentration too high. COS. Frozen condensate dripping onto food in walk-in cooler (repeat violation); paint beginning to chip off tables (repeat violation); dust accumulating on exhaust vents (repeat violation); dust build-up on decorative ceiling above line (repeat violation). Correct by 02/25/2024.

(A/98) The Clairmont, 4707 Bell St. All utensils used as scoops must have a handle and stored so handle doesn’t touch the food. COS. Several surfaces in need of cleaning, including floors and standing cooler doors (repeat violation). Correct by 02/20/2024.

(A/99) Toot ‘n Totum #63, 4420 Bell St. Debris build-up on raw wood in walk-in cooler. COS.

(A/97) Treehouse Childrens Academy, 7701 S. Coulter St. Sanitizer in dishwasher too weak. COS.

(B/82) V and M Discount Store, 1000 S.W. 10th Ave. Chest freezer needs to be removed from facility. COS. Damaged and soiled cans and drink cans; heavy build-up on soda nozzles. Correct by 12/01. Cans and toothpaste stored inside hand sink. Correct by 12/08. Heavy evidence of pests throughout facility, including droppings and mice bodies (repeat violation). Correct by 12/12. Back door in storage area needs to be sealed; pickle tongs stored on cardboard Coke box; single-use cups stored in boxes on floor in storage room and under an unfinished ceiling; single-use cups stored on tray with paint chipping; several areas in the establishment and storage rooms need to be cleaned to sight and touch (repeat violation); dirt, trash and debris throughout facility. Correct by 12/28. Holes in floor and walls must be repaired. Correct by 02/26/2024. Due to non compliance and repeat violations a re-inspection fee is being issued.
