RESULT: Downgrade

Observed salsa at 46 degrees, sliced strawberries at 47 degrees and guacamole at 48 degrees. Employee states he prepared food items at 6 a.m. to 7 a.m. Food had been held in temperature danger zone for four hours and had to be discarded. Observed stainless steel pans missing inside unit which allowed air to escape from cold unit. Facility did not have other holding units. Observed hazardous foods in reach of refrigerator. Observed green chile with no date marking. Observed raw effs stored on prep table next to stove burners. Person in charge had no standard operating procedure in place to use time as a public health control.
