Person in charge failed to demonstrate basic food safety procedures. Several food items being stored uncovered in the reach in freezer. Buildup in the ice machine. Meat grinder had food debris buildup. Observed multiple dead cockroaches near food prep line. Cockroach infestation in and around kitchen and dining area. Visible gap in between and underneath main doors. Grease and food debris buildup in kitchen. Hand washing sink in men's restroom was missing operating handle. Hole in ceiling above pickup area. Multiple holes in back of house. Excessive grease.
Result: Immediate closure

Sanitizer in spray bottle was below sanitizer concentration. Interior of the fryer had excessive soil buildup. Knives not in use with dry soil residue. Bulk food container holding fresh onions with no label. Substantial grease and soil debris buildup behind the make table, refrigerator, under fryer and stove. Sink faucet leaking. Three compartment sink faucet was leaking. Instructed person in charge to make repairs. Facility had no means of providing adequate cold holding. Only refrigerator was not functioning properly.
Result: Immediate closure
