Observed minimal rodent droppings in areas such as back stock storage area along storage racks and heavy amounts of droppings in the cleaning room on the storage shelving with the mop heads and floor polishing equipment, in rolling storage cabinets, along the compressor area of the warewashing machine and under the display case, in the electrical room, under the produce racks in grocery area, at the desk at three compartment sink where the produce wash is located and at the frozen food aisles . Observed heavy amounts of flies at the garbage compactor. Facility purchased and placed fly trap at garbage compactor.

Observed heavy debris build up in the floor drain under the display cooler. Observed significant holes in the flooring in the electrical/mechanical room leading to space below around plumbing lines and at the electrical units. Facility had sealed some of the holes from previous inspection.

Observed grout having eroded away and areas where tiles were missing. Facility was in the process of grouting during inspection. Observed significant debris build up under display case.

Observed gaps along the bottom of doors at the front doors leading to the outside. Observed significant rodent droppings in areas such as throughout back stock storage area on storage racks, and the legs, under curbside pickup coolers, cleaning room where service sink is present, at employee desk, including heavy droppings in binders, electrical room, under the bakery case and heavy droppings under bakery water system including potential nesting material. CHPD observed facility failing to follow CDC guidelines on cleaning rodent droppings, failing to soak, sweeping and nor wear protective gear. Facility was reminded multiple times to follow guidelines to clean droppings properly. Observed heavy amounts of flies throughout the facility such as at the garbage compactor.

Observed significant heavy debris build up in areas such as under display cooler, and freezer.

Observed significant holes in the flooring in the electrical/mechanical room leading to space below.

Observed rodent droppings in areas such as back stock storage area on along storage racks, under the raw meat display case and the display cooler next to the display case, and under the coffin coolers/freezers on the sales floor.

Observed plumbing leaks from under the coffin coolers/freezers with pooling water. Observed multiple gaps and holes throughout the facility into the walls under the coolers such as at the throughout northside coolers along the backwall where large areas had been in poor repair with holes and sections cut out and inside the meat cutting area. Facility was making repairs to the area during inspection. Observed significant holes in the flooring in the electrical/mechanical room leading to space below. Observed walls in poor repair bulging and holes and the flooring pitted with pooling water in the service sink room. Observed significant debris build up under display coolers and coffin coolers/freezer.

Observed minimal rodent droppings in areas such as next to the oven and the wall.

Observed significant rodent droppings in areas such as large chest freezer and coolers on the sales floor, behind spice rack and north side coolers along the back wall.

Observed significant debris build up on lights in the meat cutting room.

Observed gap leading into the wall and walk in dairy cooler with significant debris build up.
