No employee present and sign stating "Be back in 5 minutes" was on the counter. This left unmonitored access to agua frescas. Package beef jerky did not follow necessary procedures. Raw egg shells stored in the walk in cooler directly above cut watermelon and cucumbers. Solution used to clean food contact surfaces failing to meet parameters to achieve sanitation. Food items in cold holding above 41 degrees Fahrenheit. Food items such as chips stored on the ground. Food handling employees were not wearing hair restraints. Damp cloth stored in non-working sink basin. Observer food employee drink from single use coffee cup stored on food service counter next to hand washing station. Tongs inside container of sliced pickled jalapeños in brine with tongs submerged under the liquid. Single use items such as beverage cups, portion cups and bags stored on the ground. Shelving under counter had significant debris buildup. Leaks under washing stations at three sinks. No easily accessible waste receptacles. Numerous holes, gaps and crevices in the walls throughout storage area. Substantial debris buildup on the floor of the walk-in cooler. Observed food handling employee wipe nose with barehands while providing agua frescas to patrons. Food employee washed hands not in hand-washing sink.

Raw ground beef and raw chicken stored above and next to produce and other food items inside refrigerator. No probe thermometer available. Multiple live roaches crawling inside kitchen area floors and shelving. Dead roaches inside the kitchen. Ineffective sanitation. Observed substantial ice frost debris buildup inside reach-in freezer. Leak underneath three compartment sinks with substantial standing water on floors. Toilet improperly constructed, supplied and cleaned. Food debris underneath kitchen equipment such as stove top, refrigerator and freezer. Clutter in refrigerator and chemical storage area. Hand washing sinks not properly stocked.

Result: Immediate closure
