Large gap in back door of facility. Wiping cloth stored on prep table. Improper employee drink stored on prep table. Cardboard used to cover prep table. Foil and duct tape attached to large food container. Substantial ice frost buildup inside freezer. Toilet inside employee restroom cracked at top with duct tape holding fixture together. Excessive grease buildup under stove top. Employee preparing raw chicken and then drinking from water bottle without washing hands. Large leak underneath hand washing sink. Hand cleanser pump in employee bathroom no operational. Very slow drainage at sink.
