Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 24 and May 31:

Mansfield/Ontario Hampton Inn, 1051 N. Lexington-Springmill Road, Mansfield, May 24. No air gap or approved backflow prevention device on the plumbing system (critical repeat). Observed no backflow device on Bunn coffee maker in kitchen area or on juice machine in self-service buffet area. Correct by June 7.

Speedway No. 9183, 907 Springmill St., Mansfield, May 24. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed various TCS items holding between 44-46 degrees in the quick eats cooler. Ensure cold items are held at 41 degrees or below. Person in charge (PIC) voluntarily discarded all out of temperature items.

Richland Correctional Institution, 1001 Olivesburg Road, Mansfield, May 25. Improper washing of hands and arms (critical, corrected during inspection). Food worker observed touching clean hand on contaminated surface of sink handle. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Food tray stored with clean trays on serve line observed with food residue. Food contact surfaces not easily cleanable (critical, corrected during inspection). Pans observed dented and not smooth and easily cleanable. Program in place to remove percentage of pans. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Three compartment sink measured only 103 degrees.

Ontario Pointe, 2124 Park Avenue West, Mansfield, May 25. Food package(s) received in poor condition (critical, corrected during inspection). Observed large 105-ounce can of sliced carrots with dent along top seal on can rack in dry storage room. PIC moved to location for returns/discard. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed deli salads with use by dates of May 21. PIC discarded one ranch seashell pasta salad and one cucumber marinade (5 pounds each). Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed opened bag of sliced American cheese date marked June 14. Food employee that opened package was able to verify package was opened on May 22 and date marked package for six days from opening date.

Wendy's, 1372 Lexington Ave., Mansfield, May 25. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Person in charge did not ensure their employees are properly trained in food safety (critical repeat, corrected during inspection). Observed employee heating hamburger for chili without taking a temperature of the product to ensure it was made to the correct temperature. Person in charge did not ensure their employees are properly trained in food safety (critical, corrected during inspection). Observed multiple TCS products between 51-62 degrees sitting on a grey cart in the prep area. PIC verified that the items had been on the cart at room temperature since 7 a.m., over 2 hours and 45 minutes before observation. PIC voluntarily discarded. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected during inspection). Observed sanitizer in the sanitizer bucket on the make line and the sanitizer sink of the warewashing area to be less than 200 ppm. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed various TCS products in the front cooler with an internal temperature of 46-50 degrees. PIC voluntarily discarded all at time of inspection. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed various TCS products in the walk-in cooler with an internal temperature of 44-46 degrees. PIC voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed various TCS products on the make line with an internal temperature of 44-57 degrees. PIC voluntarily discarded.

JB's Drive In, 265 S. Delaware St., Lexington, May 26. Food not properly protected from contamination by separation, packaging and segregation. (critical, corrected during inspection). Observed an uncovered bucket of soft serve in the walk-in cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed containers of coney sauce and shredded chicken in the cooler that were not date marked.

O'Charley's (store No. 383), 1335 N. Lexington-Springmill Road, Mansfield, May 26. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed multiples food in reach-in freezer and reach-in cooler along cook line uncovered. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed chicken in cooler drawer along cook line to have internal temperatures of between 44-46 degrees. PIC placed ice on chicken to rapidly cool. Also observed container of lime juice sitting on prep table with internal temperature of 58 degrees. PIC discarded lime juice. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed working spray bottle of cleaner stored hanging on shelf with clean utensils and equipment in bar area. Employee relocated to a separated area/shelf.

I-71 and Ohio 97 Duchess No. 1188, 860 Ohio 97, Bellville, May 31. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed TCS (time/temperature controlled for safety) foods located in deli 1 and deli 2 retail coolers to be cold held above 41 degrees for undetermined amount of time. PIC voluntarily discarded 22 sandwiches. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed French vanilla creamer (48) in refrigerated dispenser in coffee retail area to be cold held with internal temperature above 41 degrees for undetermined amount of time. PIC voluntarily discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed pepper jack cheese and pepperoni in deli prep cooler in kitchen area not date marked. PIC discarded both items.

School Specialty Inc. Mansfield Market C, 100 Paragon Parkway, Mansfield, May 26. TCS foods in a micro market did not have an automatic shut-off (critical). Correct by June 2.

Jay Industries-Kronis Coatings-MKT C, 1575 W. Longview Ave., Mansfield, May 26. TCS foods in a micro market did not have an automatic shut-off (critical). Correct by June 2.

Taco Bell No. 30674, 1194 Park Avenue West, Mansfield, May 31. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC could not locate all/current signed employee illness agreements. PIC had employees sign at time of inspection. Food employee(s) did not wash hands when required (critical, corrected during inspection). Observed PIC to wash hands, don gloves, start the process of making food, touching headset whilst wearing gloves, and attempt to go back to food preparation.
