The following restaurant inspections with critical violations were conducted by Richland Public Health between Sept. 7 and Sept. 9:

Buffalo Wild Wings, 949 N. Lexington Springmill Road, Ontario, Sept. 7. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cut celery sticks near service window in kitchen to not be covered. Food equipment surfaces not cleaned at required frequency (critical, corrected). Observed onion residue on food slicer in clean utensil/equipment storage area. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed spray bottle without label of contents/common name. Person in charge (PIC) discarded contents of bottle.

Athena Greek Restaurant, 41 N. Main St., Mansfield, Sept. 7. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a squeeze bottle of soap without a label.

Mansfield/Ontario Hampton Inn, 1051 N. Lexington Springmill Road, Mansfield, Sept. 7. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Food contact surface of probe thermometer was not sanitized before use. Alcohol swabs obtained for future use. Presence of live insects, rodents, and other pests (critical). Several flying insects observed in food prep area. Correct by Sept. 21.

Richland County Jail, 55 E. 2nd St., Mansfield, Sept. 7. Handwashing sink not accessible (critical, corrected during inspection). Observed hand wash station with miscellaneous items in sink well. Food contact surfaces not easily cleanable (critical, corrected). Observed lids on food storage tubs that are cracked, chipped. Replace lids ASAP.

Athens Greek Restaurant, 43 S. Lexington Springmill Road, Ontario, Sept. 8. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Observed the dishwasher sanitizer concentration at 10 ppm. Correct immediately. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a spray bottle of oven cleaner without a label.

Wishmaker House B & B and Winery, 116 Main St., Bellville, Sept. 8. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Worker measured temperature of tomatoes with probe thermometer without first sanitizing.

Planktown Country Market LLC, 1921 Free Road, Shiloh, Sept. 8. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee engaged in food preparation did not have where they had been informed in a verifiable manner of their duty to report. PIC had employee read and sign. Handwashing sink is being used for purposes other than handwashing (critical, corrected). Hand sink in back preparation area of smoky run grill had utensils stored in it. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Cookies in back of smoky run grill were uncovered. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Various sandwiches in ready-made cooler with internal temperatures of 44-45 degrees; beef sticks with internal temperatures of 44-52 degrees; cheese balls with internal temperatures of 48-51 degrees. PIC voluntarily discarded items. TCS foods not being cold held at the proper temperature (critical). Internal temperature of hard-boiled eggs in two-door salad cooler had internal temperature of 46 degrees for an undetermined amount of time. PIC voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Pie cooler had items with internal temperatures of 47-53 degrees for an undetermined amount of time. PIC voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Ready-to-eat meat cooler next to cheese cooler had hot dogs with an internal temperature of 46 degrees. PIC voluntarily discarded items. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Peeled hard-boiled eggs in smoky run salad prep cooler were over 24 hours old and not labeled. PIC voluntarily discarded. Improper storage of poisonous or toxic materials (critical, corrected). Chemicals stored above food preparation area in food packaging area behind deli counter. Presence of live insects (critical). Multiple flying insects observed in bakery and deli area. Please correct by re-inspection. Presence of live insects, rodents and other pests (critical). Small rodent droppings observed on floor behind mixer in smoky run, bottom shelf in dry goods storage, along wall in back equipment storage area, and along wall under shelf in cheese walk-in cooler. PIC is to have copy of most recent invoice and all sign of activity removed by re-inspection. Food contact surfaces not easily cleanable (critical, corrected). Box cutter used to open bag of flour. PIC removed from service.

Best Stop One LLC, 295 Park Avenue West, Mansfield, Sept. 9. Presence of live insects, rodents, and other pests (critical, corrected during inspection). Small rodent dropping observed on candy box. PIC cleaned at time of inspection.

TownePlace Suites, 1125 Guest Court, Ontario, Sept. 9. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC unable to locate employee illness agreement. Richland Public Health reporting agreement form provided; PIC signed and states he will have food employees sign once they return to work.

Pizza Hut, 365 N. Lexington Springmill Road, Mansfield, Sept. 9. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). All employees reviewed and signed agreement. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Four clean tongs observed with food residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Worker did not sanitize food contact surface of probe thermometer before taking temperature of tomato sauce.

Little Warriors Childcare, 2986 Park Avenue West, Mansfield, Sept. 9. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed cutting service of can opener to contain rust and residue. Resurface or replace manual can opener by next standard inspection.

Longview & Main Duchess/ BP #1183/Subway #3183, 680 N. Main St., Mansfield, Sept. 9. Presence of live insects, rodents, and other pests (critical). Observed flies during inspection. Discussed fly control methods.

Outback Steakhouse, 820 Lexington Springmill Road, Mansfield, Sept. 9. Food not properly protected from contamination by separation, packaging,and segregation (critical, corrected during inspection). Observed products in salad prep cooler not covered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed sauces and dressing along cook line not covered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed lemons near beverage station not covered. Chemical sanitizer other than chlorine, iodine, or a quaternary ammonium applied improperly (critical, corrected). Observed sink and surface cleaner for bar three-compartment sink not kept at concentration as specified on product label. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on food contact surface of a pan in the clean utensil area. Improper use of time as a public health control - 4 hours (critical repeat, corrected). Observed pre-cooked asparagus and rice sitting on the counter with no temperature control. Employee verified that product was brought out of the walk-in around 10:30 a.m. and corrected time stamp. No air gap or approved backflow prevention device on the plumbing system (critical). Observed ice bin drain pipe directly in floor drain with no effective air gap. Correct by Sept. 12.

Kentucky Fried Chicken, 301 Ashland Road, Mansfield, Sept. 9. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Observed the wash water in the three compartment sink at 97 degrees.

McDonald's - Hanley Rd #5500, 25 W. Hanley Road, Mansfield, Sept. 9. Food employee(s) did not wash hands when required (critical, corrected during inspection). Employee observed handling personal phone and not wash hands before attempting to don gloves. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Brew station area containers of tea observed not in use and not covered. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Food contact surface of tongs observed to be in contact with personal cell phone. Presence of live insects (critical). Small flying insects observed by back door where refuse waiting to be taken out is stored. Please heave removed by re-inspection and have copy of most present pest control invoice.
