Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between June 27 and June 29:

Chad Worth LLC DBA Mary Anne's Meats, 38 W. Church St., Lexington, June 27. Foods from unapproved sources (critical, corrected during inspection). Observed cartons of farm raised eggs in the display case. Facility does not have a small egg producer certificate on file. Person in charge (PIC) pulled eggs from sale.

Applebee's Neighborhood Grill & Bar, 145 Stander Ave., Mansfield, June 27. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Items in clean area had visible residue. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected during inspection). Sanitizer in bar three-compartment sink was at 0 ppm.

Paul Revere Inn, 57 Ohio 97, Lexington, June 27. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed eggs over ready-to-eat products in the cook line reach-in cooler. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat, corrected during inspection). Observed sanitizer bucket at the bar to have a concentration less than 150 ppm. Ensure quaternary ammonium solution is made up between 200 ppm-400 ppm at all times. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed 3 pounds of cottage cheese at the salad make line to have an internal temperature of 44-46 degrees. PIC voluntarily discarded cottage cheese.

Domino's Pizza, 359 N. Trimble Road, Mansfield, June 28. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). PIC stated facility did not receive the report from corporate, and therefore there is nothing done. Re-inspection to occur on July 5. Presence of live insects (critical repeat). Observed several black flying insects throughout the facility. Re-inspection to occur on July 5. Food contact surfaces not easily cleanable (critical repeat). Observed scissors being used for opening food packages not easily separable, rendering them unable to be easily cleanable. Discontinue use and obtain scissors that are able to be easily separable by re-inspection on July 5.

Camp Otyokwah, 3380 Tugend Road, Butler, June 28. Person in charge did not ensure employees were effectively cleaning their hands as required (critical, corrected during inspection). Volunteer workers were entering food prep area without first washing hands. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Four volunteer workers in warewashing area had not reviewed and signed reporting agreement. Food employee(s) did not wash hands when required (critical, corrected during inspection). Workers observed donning new gloves without first washing hands and also beginning work without first washing hands. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Two pans of rice observed in walk-in cooler date marked June 20, 2023, had not been disposed of after required seven days. Listed items voluntarily discarded. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Wash solution in three compartment sink in use measured only 96 degrees.

MVP Sports Bar & Grill, 275 Park Avenue West, Mansfield, June 28. PIC unable to demonstrate knowledge of temperatures and times for safe cooling and reheating (critical, corrected during inspection). Reviewed cooling from 135 to 70 degrees or below in two hours or less and from 70 to 41 degrees or below in four hours or less. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed nacho cheese in the warmer at 120 degrees. PIC voluntarily discarded cheese. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed the top layer of ground beef patties in the make table cooler at 54 degrees. PIC voluntarily discarded patties.

KV Market, 359 W. Fourth St., Mansfield, June 29. Food package(s) received in poor condition (critical, corrected during inspection). Observed several cans dented on the top and bottom seams. PIC removed cans from sale. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed packages of raw bacon on a shelf above ready-to-eat lunch meat in the walk-in cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed uncovered pans of food in the walk-in freezer. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed a carton of raw shell eggs on the shelf above ready-to-eat foods inside the make table cooler in the kitchen. Missing and/or incomplete allergen declaration(s) (critical, corrected during inspection). No list of ingredients on the label of banana bread packed in the facility.

Leaning Tower of Pizza, 180 Lexington Ave., Mansfield, June 29. Presence of live insects and rodents (critical repeat, corrected during inspection). Observed rodent droppings remaining in the tool room. PIC provided most recent pest control invoice. Facility is to provide EHSIT with pest control summaries or invoices for next 3 months. PIC cleaned storage room at time of inspection.
