Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health July 24 and 25:

Logan's Roadhouse Inc., 2170 W. Fourth St., Ontario, July 24. Person in charge (PIC) did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical). Observed multiple violations. Correct by July 31. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by July 31. Handwashing sink is being used for purposes other than handwashing (critical). Observed employee dump bucket of sanitizer water into hand sink in back prep area. Correct by July 31. Chemical sanitizer other than chlorine, iodine, or a quaternary ammonium applied improperly (critical repeat, corrected during inspection). Observed working bucket of sink and surface sanitizer at 0 ppm. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed TCS foods in steamer hot holding below 135 degrees (rice, onions, mushrooms, corn and minestrone soup). PIC had employee reheat listed foods to 165 degrees. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple foods in walk-in cooler with internal temperatures above 41 degrees (sliced American cheese, shredded mozzarella cheese, shredded cheddar cheese, bleu cheese crumbles, hard boiled eggs, cooked penne pasta, cooked spaghetti pasta, cooked macaroni & cheese, baked beans, butter, cocktail sauce, cut lettuce, cooked green beans and cheesecake). All listed foods were placed into walk-in meat cooler. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple TCS foods in cold holding well near servers window of kitchen area and in appetizer prep cooler held above 41 degrees for undetermined amount of time (cooked penne, house-made rama lade, tartar sauce, sour cream, cocktail sauce, pico de gallo,, house-made ranch dressing, parmesan cheese, spinach dip, house-made petal sauce and queso cheese). PIC discarded listed foods. Ready-to-eat, TCS food not properly discarded when required (critical repeat). Observed multiple TCS foods not discarded by use by date indicated sticker (cut tomato\lettuce burger sets, marinara sauce, cooked spaghetti noodles, pico de gallo and macaroni & cheese. PIC discarded listed foods. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed open bag of shredded mozzarella cheese not date marked. PIC discarded cheese. Food thermometers are not accurate (critical). Observed food thermometer in back prep are to read 42 degrees when actual temperature of food was 39 degrees. Correct by July 31. Presence of live insects (critical). Observed multiple flying insects throughout the facility. Correct by July 31. Food contact surfaces not easily cleanable (critical, corrected). Observed multiple plastic food containers with cracks and warping on interior surfaces of containers. PIC discarded containers.

Lands Edge Beach Club, 3431 Ohio 95, Perrysville, July 25. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Proper instruction given to PIC.

Bent Spoons Bakery, 18 Mill Run, Bellville, July 25. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Half pint of buttermilk observed stored for over 24 hours in cooler without proper date marking.

Der Dutchman Restaurant, 720 Ohio 97 W., Bellville, July 25. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Discussed monitoring items held on ice to ensure cold holding of 41 degrees or below or using a time instead of temperature control for TCS items. PIC unable to demonstrate knowledge by having no critical violations (critical). Person in charge not ensuring proper cooling (critical, corrected). Discussed using shallow pans to ensure rapid cooling of 135 degrees to 70 degrees or below in two hours or less and from 70 to 41 degrees or below in four hours or less. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected). Observed uncovered pies on a rolling cart inside the walk-in freezer. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Observed dishwasher chlorine sanitizer at 0 ppm. Improper cooling of TCS food (critical repeat, corrected). Roast beef and roast pork put into the walk-in the night before had an internal temperature of 45 degrees. PIC voluntarily discarded one small pan of roast beef and approximately four small pans of roast pork. TCS foods not being hot held at the proper temperature (critical, corrected). Observed a pan of sausage links in the warmer at 115 degrees. PIC voluntarily discarded sausage. TCS foods not being cold held at the proper temperature (critical, corrected). Observed yogurt at 44 degrees and cut honeydew and cantaloupe at 46 degrees on the buffet line. PIC voluntarily discarded yogurt and melon. TCS foods not being cold held at the proper temperature (critical, corrected). Observed the top layer of sausage patties held on ice at 52 degrees. PIC voluntarily discarded 2 pounds 5 ounces of sausage patties. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed a container of sausage with a use by date of July 24. PIC discarded sausage. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed two containers of sliced trail bologna that were not date marked. PIC voluntarily discarded trail bologna. Working containers of poisonous or toxic materials not properly labeled (critical repeat, corrected). Observed a squeeze bottle of soap that was not labeled with common name. Presence of live insects (critical). Observed flies on the ceiling in the bakery. Correct by Aug. 1. No air gap or approved backflow prevention device on the plumbing system (critical, corrected). No backflow devices observed on the hose in the warewashing area and the hose at the food prep sink. PIC disconnected hoses. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed a funnel around the end of the drain pipes on the ice machines that was eliminating the air gap. Correct by Aug. 1.

American Legion Post 535, 77 Bell St., Bellville, July 25. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Hot dogs observed stored in two-door, reach-in cooler for over 24 hours without proper date marking.

Burger King Restaurant #415, 560 N. Lexington Springmill Road, Mansfield, July 25. Did not require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule (critical, corrected during inspection). Employee illness agreements that are acknowledged online during hiring process only listed six reportable illnesses instead of 13 reportable illness per the Ohio Uniform Food Safety Code. Provided a copy of the Richland Public Health employee reporting agreement. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed open bag of cut lettuce in True prep cooler not date marked. PIC discarded lettuce.

Bob Evans Restaurants LLC #219, 30 W. Hanley Road, Mansfield, July 25. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Person in charge did not ensure their employees are properly trained in food safety (critical, corrected during inspection). Line cook could not state the proper cooking temperature for raw animal foods. Education provided. Food employee(s) did not wash hands when required (critical, corrected). Observed line cook touching forehead and hair with gloves on while cooking orders. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed employees melting ice in the handwashing sink; also observed whole green beans at the bottom of the handwashing sink. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed clean stored deli slicer to have visible residue on the blade. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected). Observed sanitizer solution buckets with a concentration between 0 ppm-150 ppm. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed wild fire ranch pitcher to have excessive visible staining. PIC had removed from service. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed TCS items between 44-46 degrees (lettuce, blue cheese and cut cantaloupe). PIC voluntarily discarded the items. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed various TCS items being cold held on the front make line to have an internal temperature between 45-52 degrees. PIC voluntarily discarded all out of temperature items. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed raw chicken, home fries and wild fire ranch past date mark. PIC voluntarily discarded items. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened milk without a date mark. Improper use of time as a public health control - 4 hours (critical, corrected). Observed no time stamp on the liquid and raw in-shell eggs. Food thermometers are not accurate (critical). Observed two thermometers on the front cook line to be out of calibration. Calibrate or replace by re-inspection Aug. 1. Presence of live insects (critical). Observed small black flying insects under soda dispensers in the server station, behind piece of broken paneling by the mop sink, the warewashing area and in the dry storage room. Contact licensed pest control operator by Aug. 1. Food contact surfaces not easily cleanable (critical repeat, corrected). Observed scissors that are not easily separable being used to open coffee packages. PIC discontinued use of scissors. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed insufficient air gap at the prep sink (white pipe). Correct by Aug. 1.
