Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between July 31 and Aug. 3:

Speedway #1099, 1321 Park Avenue West, Mansfield, July 31. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed employee illness agreement posted on bulletin board but no signatures to verify employees are informed in a verifiable manner. Provided copy of Richland Public Health's employee reporting agreement. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed the upper interior food contact portion of ice dispensers on Big Gulp fountain beverage machine and Follet ice machine on coffee counter to contain visible residue. Missing and/or incomplete allergen declaration(s) (critical, corrected). Observed blueberry muffins in self-service donut case to not have ingredients/allergens declaration. Person in charge (PIC) removed donuts from case during inspection.

Marco's Pizza #1065, 888 Ashland Road, Mansfield, July 31. Presence of live insects, and other pests (critical). Observed flies during inspection. Correct by Aug. 23.

O'Charley's (store #383), 1335 N. Lexington Springmill Road, Mansfield, July 31. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed cod filets in walk-in cooler with internal temperature of between 51-52 degrees. PIC discarded five metal pans (40 pounds) of cod filets. Observed cole slaw on prep cooler to have internal temperature of 43 degrees. PIC placed in cooler to rapidly cool to 41 degrees.

500 Park Ave. Café, 500 Park Avenue West, Mansfield, Aug. 1. PIC unable to demonstrate knowledge by having no critical violations (critical). Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cheesecake and meat-on-a-stick in the freezer to be uncovered. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed deli slicer blade and holder prong to have visible residue. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed burger patties in the make line that were opened two days before inspection without a date mark. Presence of live insects (critical repeat). Observed black flying insects throughout the facility. Rid facility of evidence of insects by re-inspection on Aug. 8.

IHOP, 2190 Walker Lake Road, Ontario, Aug. 1. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Education provided and all critical violations were corrected during inspection. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed raw bacon stored above batter mix in reach-in cooler along cook line. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed multiple foods (potato chunks, fried steak, batter, gravy, cheese sauce, fries) in reach-in freezers and cooler uncovered/unprotected. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed multiple metal spatulas in clean storage area to contain visible dried residue. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed multiple TCS foods (whipped cream, pancake batter, waffle batter) held above 41 degrees for undetermined amount of time. PIC discarded all listed foods.

Bob Evans Restaurants LLC #219, 30 W. Hanley Road, Mansfield, Aug. 1. PIC unable to demonstrate knowledge by having no critical violations (critical). Presence of live insects (critical repeat). Observed small black flying insects in the following locations: under soda dispensers in the server station, behind piece of broken FRP paneling by the mop sink, the warewashing area, and in the dry storage room. At re-inspection, observed no change. PIC stated pest control has been contacted but has yet to arrive. Correct by Aug. 8 re-inspection.

Panda Express, 994 N. Lexington-Springmill Road, Ontario, Aug. 2. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed garlic in oil with label specifically indicating "keep refrigerated" held on cart at wok area with internal temperature of 66 degrees for an undetermined amount of time. Employee discarded garlic in oil.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, Aug. 2. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Handwashing sink not accessible (critical repeat, corrected during inspection). Observed handwashing sink in the sushi area to be blocked with rolling push cart. Presence of live insects (critical repeat). During re-inspection observed some improvement, yet there are still insects present in the deli area. Correct by follow-up.

Kroger Company #N518, 1240 Park Avenue West, Mansfield, Aug. 2. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed packages of ground turkey on a shelf above ground beef. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed packages of partially cooked smoked turkey on a shelf above packages of fully cooked ham and sausage. Presence of live insects (critical). Observed flies and gnats in the deli and bakery area. Correct by Aug. 9.

The Golden Burrito Mexican Grill, 574 Park Avenue East, Mansfield, Aug. 3. TCS foods not being cold held at the proper temperature (critical). Observed True reach-in cooler reading 54 degrees. Advised PIC to throw out perishable foods. Correct by Aug. 4. Presence of live insects, rodents and other pests (critical). Observed fly present in kitchen area. Correct by Aug. 4.

Wiener King/Sunset Fast Foods Inc., 118 Lexington Ave., Mansfield, Aug. 3. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Presence of live insects (critical repeat). Observed flying black insects throughout the facility. During re-inspection, observed less insects and one door with the screen attached. PIC stated plans to have screen for second door installed soon. Eliminate all evidence of insects by re-inspection Aug. 10.

Tamara K. King, 4218 Park Avenue West, Mansfield, Aug. 3. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by Aug. 17. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). No employee illness reporting agreement observed. Handwashing sink not accessible (critical repeat). Facility observed to have no running water rendering handwashing sinks inaccessible. Correct by Aug. 17. CS foods not being cold held at the proper temperature (critical). Observed orange juice and whipped cream in Beverage-Air reach-in cooler to be held with internal temperatures above 41 degrees for an undetermined amount of time. PIC discarded orange juice and whipped cream. No running water observed in facility at time of inspection (critical). Observed large store-bought bottles of water being used. Facility has switched to serving only bottled/canned beverages, disposable utensils\equipment, and serving only pre-packaged food. Correct by Aug. 17. No air gap or approved backflow prevention device on the plumbing system (critical). No backflow observed on plumbing system for facility. Correct by Aug. 17.

Forest Hills Golf Course, 2235 Ohio 430 E., Mansfield, Aug. 3. TCS foods not being cold held at the proper temperature (critical). Observed White Refrigeration reading 54 degrees. Advised PIC to remove any perishable food immediately and adjust/monitor food for proper temperature is achieved.

Hidden Hollow Camp, 5127 O'Possum Run Road, Bellville, Aug. 3. Food contact surfaces not easily cleanable (critical repeat, corrected during inspection). Observed rubber spatula with large crack/break, rendering it no longer easily cleanable. PIC discarded spatula.
